Guanfacine treatment of comorbid attention-deficit hyperactivity disorder and Tourette's syndrome: preliminary clinical experience.

OBJECTIVE: Many children with Tourette's syndrome (TS) are handicapped more by difficulties with inattention, impulsivity, and hyperactivity than by their tics. However, stimulant medications used to treat attention-deficit hyperactivity disorder (ADHD) can exacerbate tics. Guanfacine is an alpha 2-adrenergic agonist that may have beneficial effects on attention, without the hypotensive or sedative effects of clonidine, which is often used as an alternative to stimulants. METHOD: An open-label study of guanfacine was performed in 10 children with TS+ADHD, aged 8 to 16 years. The duration of f

A sensitive and green spectrofluorimetric method for determination of guanfacine hydrochloride based on NBD-Cl derivatization.

A sensitive and environmentally friendly spectrofluorimetric method was developed for the determination of guanfacine in pharmaceutical formulations. The method is based on derivatization of guanfacine with 4-chloro-7-nitrobenzofurazan in alkaline medium to yield a highly fluorescent product measured at 535 nm after excitation at 466 nm. Experimental conditions affecting the reaction were systematically optimized to achieve maximum fluorescence intensity. The method exhibited excellent linearity over the concentration range of 50-500 ng/mL with a correlation coefficient of 0.9994. The limits o

Guanfacine intoxication in a young child presenting with prolonged somnolence and sinus bradycardia: a case report.

Guanfacine, a medication used to treat attention-deficit hyperactivity disorder (ADHD), activates alpha-2A adrenergic receptors in the brain to reduce symptoms. However, its central alpha-2 agonist activity can cause adverse events, such as somnolence, bradycardia, and hypotension. We present the case of a 6-year-old boy (15 kg) with no history of regular medication use who was admitted to the hospital with unexplained prolonged somnolence and bradycardia. Initial evaluation ruled out common causes; however, ingestion of his mother's prescription medication was suspected. Liquid chromatography

Guanfacine alters the effect of stress and smoking on heart rate variability in regular daily smokers.

BACKGROUND: We had previously demonstrated that guanfacine, an α2a-adrenergic agonist, attenuated the effect of stress on smoking-lapse behavior in regular daily smokers. Heart rate variability (HRV), a measure of vagal activity, may be a potential mechanism underlying the relationship between stress, smoking, and relapse. METHODS: We examined whether guanfacine (0 mg/day vs. 3 mg/day; n = 26) altered changes in high-frequency heart rate variability (HF-HRV) following stress and ad-lib smoking using a validated laboratory analogue of smoking-lapse behavior. All participants completed a parent

A translational investigation targeting stress-reactivity and prefrontal cognitive control with guanfacine for smoking cessation.

Stress and prefrontal cognitive dysfunction have key roles in driving smoking; however, there are no therapeutics for smoking cessation that attenuate the effects of stress on smoking and enhance cognition. Central noradrenergic pathways are involved in stress-induced reinstatement to nicotine and in the prefrontal executive control of adaptive behaviors. We used a novel translational approach employing a validated laboratory analogue of stress-precipitated smoking, functional magnetic resonance imaging (fMRI), and a proof-of-concept treatment period to evaluate whether the noradrenergic α2a a

Sex differences in guanfacine effects on drug craving and stress arousal in cocaine-dependent individuals.

Currently, no FDA-approved medication exists for the treatment of cocaine use disorder. Furthermore, as women become increasingly more at risk for the consequences of cocaine addiction, the need to establish better-tailored treatment medications is paramount. We examine the effects of the alpha2 adrenergic agonist, guanfacine HCl, on responses to stress and drug cue in a group of cocaine-dependent men and women who also abuse alcohol and nicotine. Forty early abstinent treatment-seeking cocaine-dependent males and females were randomly assigned to receive either daily placebo (12 M/7 F) or gua

Guanfacine-induced lichenoid drug eruption in a child with autism and attention deficit hyperactivity disorder.

Lichenoid drug eruptions (LDEs) have a variety of medication causes. We report a case of a 5-year-old boy with autism and attention deficit hyperactivity disorder treated with guanfacine who developed pruritic lesions consistent with LDEs. Rechallenge was not attempted. There are several clinical and histopathologic clues that may distinguish LDEs from lichen planus.

A comparison of the inhibitory effects of clonidine and guanfacine on the behavioral and autonomic components of morphine withdrawal in rats.

Intravenous administration of naloxone (0.5 mg/kg) to morphine dependent rats elicited classical autonomic and behavioral symptoms of narcotic abstinence including hypertension, tachycardia, withdrawal body shakes, escape attempts, diarrhea, etc. Pretreatment of dependent rats with either clonidine (3-90 micrograms/kg) or guanfacine (3-900 micrograms/kg) produced a dose-dependent reduction in the hypertensive response to subsequent injection of naloxone. Clonidine was about 12 times more potent than guanfacine in inhibiting this autonomic symptom of withdrawal. Both drugs were less effective a

Comprehensive analysis of Guanfacine treatment in autism spectrum disorder with comorbid attention deficit hyperactivity disorder.

Autism Spectrum Disorder (ASD) often co-occurs with Attention-Deficit/Hyperactivity Disorder (ADHD), exacerbating functional challenges. Standard ADHD treatments are often less effective and less well tolerated in ASD. Guanfacine XR, a non-stimulant ADHD medication, shows promise for managing hyperactivity and irritability in ASD-ADHD but with mixed results. This retrospective study analyzed data from 29 children aged 6–18 years with ASD-ADHD treated with Guanfacine XR at three healthcare centers. Clinicians initiated Guanfacine XR in cases with persistent ADHD symptoms, partial response, or s

Guanfacine effects on stress, drug craving and prefrontal activation in cocaine dependent individuals: preliminary findings.

Cocaine dependence is associated with increased stress and drug cue-induced craving and physiological arousal but decreased prefrontal activity to emotional and cognitive challenge. As these changes are associated with relapse risk, we investigated the effects of α2 receptor agonist guanfacine on these processes. Twenty-nine early abstinent treatment-seeking cocaine dependent individuals were randomly assigned to either daily placebo or guanfacine (up to 3 mg) for four weeks. In a laboratory experiment, all patients were exposed to three 10-min guided imagery conditions (stress/stress, drug cu
