Head turning sign.

The head turning sign was first described as such more than 20 years ago but only recently have empirical studies of the sign in large cohorts of patients being assessed for cognitive disorders been reported. Although precise operationalisation of the sign has differed between studies, nevertheless it appears to be frequently observed in cognitive clinic patient cohorts, more so in Alzheimer's disease than in other dementias and with an intermediate frequency in mild cognitive impairment. Head turning is an easily observed and categorised sign and may raise suspicion of the presence of a cogni

[Mild head injury].

INTRODUCTION: Mild traumatic brain injury (TBI) represents a major health concern, because a sizeable number of patients with mild TBI will develop potentially life-threatening complications. The target of this study was to describe a large series of adult patients suffering from mild TBI, treated at University Hospital of Getafe, between 2010 and 2015 (n = 2480). We examined the patients' epidemiological and baseline clinical profile, diagnosis, treatment and ultimate outcomes, to identify major prognostic factors that influence the final result. METHODS: We retrospectively extracted patient

Radial Head Fractures.

BACKGROUND: Radial head fractures are common elbow injuries in adults and are frequently associated with additional soft tissue and bone injuries. METHODS: A literature search was performed and the authors' personal experiences are reported. RESULTS: Mason type I fractures are treated non-operatively with splinting and early mobilisation. The management of Mason type II injuries is less clear with evidence supporting both non-operative treatment and internal fixation. The degree of intra-articular displacement and angulation acceptable for non-operative management has yet to be conclusively

Head-Up Tilt.

Autonomic dysfunction has been associated with paroxysmal atrial fibrillation (PAF). The head-up tilt test (HUTT) is an important diagnostic tool for autonomic dysfunction. The aim of this study was to examine atrial fibrillation recurrence after RFCA by performing HUTT. A total of 488 consecutive patients with PAF who underwent RFCA were prospectively enrolled. HUTT was positive in 154 (31.6%) patients after a mean follow-up of 22.7 ± 3.5 months, and 163 (33.4%) had a recurrence. HUTT positive was significantly higher in PAF patients with recurrence compared to those without (68 (41.7%) versu

Myofascial Head Pain.

Muscle nociception is mainly characterized by local tenderness and referred pain. The neurophysiological basis of muscle pain supports a role of sensitization mechanisms. From a clinical viewpoint, muscle pain is represented by the presence of myofascial trigger points (TrPs). Evidence suggests that TrPs are able to start a peripheral nociceptive mechanism and hence contributing to changes in the central nervous system. Several studies demonstrated that the referred pain elicited by TrPs reproduces the headache pattern in patients with tension-type headache (TTH), migraine, cervicogenic headac

Radial head fractures.

The radial head is the most commonly fractured bone of the elbow, with most fractures occurring in women older than 50 years. The radial head is an important stabilizer for valgus, axial, and posterolateral rotational forces. Loss of articular segments of the radial head negatively affects elbow kinematics and stability. Most fractures are treated nonsurgically. Indications for nonsurgical treatment include nondisplaced and isolated displaced fractures without a block to motion. Fragment excision is indicated when a mechanical block from a small, displaced fragment impedes elbow motion. If tec

Closed head injury.

Closed head injuries vary from the very minor to the catastrophic. It is often difficult to differentiate the severity at initial presentation. Serial assessment is very valuable. Awareness of facial injuries is aided by familiarity with facial bone anatomy and the clinical presentation of orbital, zygomatic, maxillary, and mandibular fracture. Functional injury such as concussion may coexist with other injuries. This article will discuss closed head trauma and outline specific injuries to the face, brain, skull, and its surroundings.

Radial head arthroplasty.

In conclusion, radial head fractures with 3 or more fragments have a high incidence of complications when treated with ORIF including hardware failure, malunion, nonunion, and the need for re-operation. Radial head arthroplasty has demonstrated good success in the treatment of complex, comminuted radial head fractures which are not amenable to non-opeative treatment or ORIF. Success can be optimized by diligent surgical dissection, avoiding inadvertent nerve injury, placement of an appropriately sized implant, repair of associated injuries, and early protected motion.

Minor head trauma.

This article reviews the persisting difficulty and the importance of the diagnosis of minor head trauma. The diagnosis has been complicated by pervasive disagreement regarding diagnostic criteria. This is primarily a result of the fact that evidence for actual injury is hard to obtain in minor cases because most symptoms tend to be subjective and have high base rates in the normal, uninjured population. At the same time, the diagnostic decision has important implications for patients in terms of treatment, expectancy for future function and lifestyle, and compensation for injuries. Decision th

Pediatric head injury.

Pediatric head injury is a public health problem that exacts a high price from patients, their families and society alike. While much of the brain damage in head-injured patients occurs at the moment of impact, secondary injuries can be prevented by aggressive medical and surgical intervention. Modern imaging devices have simplified the task of diagnosing intracranial injuries. Recent advances in monitoring technology have made it easier to assess the effectiveness of medical therapy. These include intracranial pressure monitoring devices that are accurate and safe, and jugular bulb monitoring

Slipped capital femoral epiphysis could be regarded as exceeding of a physiological process in puberty in which overweight of the patients seems to be the essential factor. Early clinical and radiological diagnosis are important to perform simple surgical procedures stopping further lysis and avoiding later osteoarthritis. Fixation of the femoral epiphysis with K.-wires are emphasized worldwide because of the lower complication rate. In case of severe slip osteotomies for realignment are more problematic. Long term follow-up studies have shown poor results mostly because of chondrolysis and fe

Abnormal head movements.

Abnormal head movements have been studied in a variety of diseases using objective recording techniques and the data analysed with respect to the frequency content of the movement. Flopping, nodding, tic, chorea, myoclonic jerks, and most head tremors involve frequencies of approximately 2 and 4 Hz which correspond to the natural fundamental and second harmonic resonances of the head as determined by the mechanical properties of the head/neck system. These findings provide a basis for classification of abnormal head movements as well as an explanation of the characteristics of those arising fr

Head-to-head relief: ubrogepant, rimegepant, and zavegepant in migraine treatment.

Migraine, a significant cause of disability worldwide, heavily impacts daily functioning and quality of life. Despite various acute treatment options, including nonsteroidal anti-inflammatory drugs (NSAIDs) and triptans, patients experience limited relief or adverse effects. This review examines the efficacy and safety of gepants - ubrogepant, rimegepant, and zavegepant - in the acute treatment of migraine. We assessed phase II and III clinical trials, focusing on clinically relevant endpoints such as pain freedom and freedom from the most bothersome symptom at two hours post-treatment. We cal

The Sheep Head Versus the Pig Head as a Training Model for Sialendoscopy.

OBJECTIVE: Sialendoscopy is a minimally invasive technique for treating inflammatory salivary gland diseases, requiring hands-on training due to its steep learning curve. While pig heads have been the standard training model for over two decades, sheep heads have recently emerged as an alternative. This study evaluates the efficiency of sheep heads for sialendoscopy training in newcomers. METHODS: Participants of the 32nd International Sialendoscopy Hands-on Course (Geneva, 2023) performed sialendoscopies on fresh pig and sheep heads. Self-assessment questionnaires were completed pre- and pos

'Falling heads': investigating reflexive responses to head-neck perturbations.

BACKGROUND: Reflexive responses to head-neck perturbations affect the injury risk in many different situations ranging from sports-related impact to car accident scenarios. Although several experiments have been conducted to investigate these head-neck responses to various perturbations, it is still unclear why and how individuals react differently and what the implications of these different responses across subjects on the potential injuries might be. Therefore, we see a need for both experimental data and biophysically valid computational Human Body Models with bio-inspired muscle control s

Head injury: audit of a clinical guideline to justify head CT.

INTRODUCTION: Head injury causes significant morbidity and mortality, and there is contention about which patients to scan. The UK National Health Service Clinical Guideline (CG) 56 provides criteria for selecting patients with clinically important brain injury who may benefit from a head CT scan, while minimising the radiation and economic burden of scanning patients without significant injury. This study aims to audit the documentation of the use of these guidelines in a busy UK trauma hospital and discusses the comparison with an Australian (New South Wales (NSW) ) head injury guideline.
