Dissociation between Caloric and Video Head Impulse Tests in Dizziness Clinics.

Vestibular assessment tests such as the video head impulse test (vHIT) for the horizontal semicircular canal, and caloric test (Cal), both evaluate horizontal canal function. One would assume that the outcomes for these tests should lead to concordant results, yet several studies have suggested that dissociation can occur in certain pathological conditions. As this topic remains inconclusive, this review aims to analyze the scientific evidence regarding the patterns of hypofunction observed in vHIT and Cal in different otoneurological diseases. A comprehensive review of the literature regardin

Evolution in the Findings of Head-Impulse Tests During the Attacks of Menière's Disease.

OBJECTIVE: To determine the vestibulo-ocular reflex (VOR) performance during the attacks of Menière's disease (MD) using video head-impulse tests (video-HITs) according to each ictal phase. STUDY DESIGN: Retrospective case series review. METHODS: We analyzed the results of video-HITs in 24 patients with unilateral definite MD during and between the attacks. RESULTS: The head impulse gain of the VOR was usually normal (81%, 39 of the 48 semicircular canals [SCCs] in 16 patients) in the affected ear during the irritative or recovery phase, and did not differ from that for each SCC between the

Video head impulse testing in patients with isolated (hemi)nodular infarction.

BACKGROUND: Isolated (hemi)nodular strokes as underlying cause of acute dizziness are rare, thus there are still gaps of knowledge in the clinical presentation of affected patients. Clinical and experimental evidence has suggested that lesions involving the nodulus lead to various vestibulo-ocular deficits including prolonged velocity-storage, periodic-alternating nystagmus, positional nystagmus, abolished suppression of post-rotatory nystagmus by head-tilt and impaired verticality perception. At the bedside, the angular vestibulo-ocular reflex (aVOR), as assessed by the horizontal head-impuls

Video head impulse in comparison to caloric testing in unilateral vestibular schwannoma.

CONCLUSIONS: Although there was a statistically significant relationship between the results of the vHIT and the caloric test, the limited strength of this relationship suggests that, for unilateral vestibular schwannoma (UVS), caloric testing and vHIT may provide complementary information on vestibular function. OBJECTIVE: There is limited information that can be used to determine which of the video head impulse test (vHIT) and caloric test might be better used in the diagnosis and management of UVS. In this study, a group of participants with un-operated UVS was studied using both methods.

Bilateral Vestibulopathy Documented by Video Head Impulse Tests in Superficial Siderosis.

OBJECTIVE: Superficial siderosis (SS) of the CNS is associated with cerebellar ataxia, sensorineural hearing loss, and pyramidal symptoms, which result from iron depositions on CNS surfaces. SS can produce bilateral vestibulopathy as the vestibulo-cochlear nerve is particularly vulnerable. To our knowledge, however, vestibular dysfunction in SS has not been reported thoroughly in the literature. Here, we describe a case of bilateral vestibulopathy, documented quantitatively by the video head impulse test (vHIT), in a patient with SS. PATIENT: A 60-year-old man presented with slowly progressin

Background factors in patients with compensated video head impulse test gain after vestibular rehabilitation therapy.

OBJECTIVES: A few patients with chronic vestibular dysfunction have shown improved video Head Impulse Test (vHIT) gain after vestibular rehabilitation therapy (VRT), whereas others have shown either slight improvement or remained stable. Therefore, we investigated whether some specific factors affected the vHIT gain after VRT. METHODS: Six hundred and eighty-six patients with intractable vertigo/dizziness who were hospitalized for neuro-otological examinations at the Vertigo/Dizziness Center of Nara Medical University between July 2014 and October 2023 were enrolled. In addition, patients who

Video head impulse test differentiates RFC1-CANVAS, FGF14-SCA27B, and idiopathic late-onset cerebellar ataxias.

BACKGROUND: RFC1-related CANVAS and FGF14-related SCA27B are recently recognized genetic causes of late-onset cerebellar ataxia (LOCA). Despite advances in genetic testing, most panels do not detect these intronic repeat expansions, often resulting in repeated testing, delayed diagnosis, and greater patient burden. Vestibular dysfunction is common in both disorders and may help differentiate them from idiopathic LOCA (ILOCA) subtypes, guiding targeted genetic testing. OBJECTIVES: To evaluate whether vestibulo-ocular reflex (VOR) gain measured by video head impulse test (vHIT) can differentiat

Numerical simulations to determine the stimulation of the crista ampullaris during the Head Impulse Test.

The Head Impulse Test, the most widely accept test to assess the vestibular function, comprises rotations of the head based on idealized orientations of the semicircular canals, instead of their individual arrangement specific for each patient. In this study, we show how computational modelling can help personalize the diagnosis of vestibular diseases. Based on a micro-computed tomography reconstruction of the human membranous labyrinth and their simulation using Computational Fluid Dynamics and Fluid-Solid Interaction techniques, we evaluated the stimulus experienced by the six cristae ampull

Comparison of the video head impulse test results with caloric test in patients with Meniere's disease and other vestibular disorders.

Background: The caloric test has been used to evaluate the semi-circular canal function for decades. In 2009, the video head impulse test (vHIT) was introduced, which can be used to evaluate the semi-circular canal function within a short time. Although both tests examine the semi-circular canal, the stimulation methods differ and it is unclear whether the vHIT is equivocal to the caloric test.Aims/objectives: This study aimed to discern the differences between the vHIT and caloric test.Material and methods: This study comprised 112 patients with vertigo who visited the vertigo/dizziness centr
