A Technique of Forced Expiratory Noise Time Evaluation Provides Distinguishing Human Pulmonary Ventilation Dynamics During Long-Term Head-Down and Head-Up Tilt Bed Rest Tests Simulating Micro and Lunar Gravity.

Estimating the effect of microgravity/hypogravity on pulmonary ventilation function remains topical. Recently developed acoustic techniques based on the evaluation of the forced expiratory noise time (FETa) were hypothesized to be a promising tool for this aim. The aim of the protocol is to study the effect of two different modalities of bed rest space simulations (microgravity and lunar gravity) on FETa and spirometric indices. The FETa in the frequency band of 200-2000 Hz, recorded above human trachea, was evaluated. The 21st-day exposure to 6 degree head-down tilt (HDT) bed rest, simulating

Development of optic disc edema during 30 days of hypercapnic head-down tilt bed rest is associated with short sleep duration and blunted temperature amplitude.

Sleep and circadian temperature disturbances occur with spaceflight and may, in part, result from the chronically elevated carbon dioxide (CO2) levels on the international space station. Impaired sleep may contribute to decreased glymphatic clearance and, when combined with the chronic headward fluid shift during actual spaceflight or the spaceflight analog head-down tilt bed rest (HDTBR), may contribute to the development of optic disc edema. We determined if strict HDTBR combined with mildly elevated CO2 levels influenced sleep and core temperature and was associated with the development of

Head-Down-Tilt Bed Rest With Elevated CO2: Effects of a Pilot Spaceflight Analog on Neural Function and Performance During a Cognitive-Motor Dual Task.

Spaceflight has widespread effects on human performance, including on the ability to dual task. Here, we examine how a spaceflight analog comprising 30 days of head-down-tilt bed rest (HDBR) combined with 0.5% ambient CO2 (HDBR + CO2) influences performance and functional activity of the brain during single and dual tasking of a cognitive and a motor task. The addition of CO2 to HDBR is thought to better mimic the conditions aboard the International Space Station. Participants completed three tasks: (1) COUNT: counting the number of times an oddball stimulus was presented among distractors; (2

Antioxidant Supplementation Does Not Affect Bone Turnover Markers During 60 Days of 6° Head-Down Tilt Bed Rest: Results from an Exploratory Randomized Controlled Trial.

BACKGROUND: Immobilization and related oxidative stress are associated with bone loss. Antioxidants like polyphenols, omega-3 fatty acids, vitamins, and micronutrients may mitigate these negative effects on bone metabolism through scavenging of free radicals. OBJECTIVES: We hypothesized that antioxidant supplementation during 60 days of 6° head-down tilt bed rest (HDBR) would reduce bone resorption and increase bone formation compared to nonsupplemented controls. METHODS: This exploratory randomized, controlled, single-blind intervention study conducted in a parallel design included 20 healt

Optic nerve sheath diameter increases during 12◦ head-down tilt in healthy volunteers: a crossover pilot study informing the design of a spaceflight countermeasure trial.

BACKGROUND: Spaceflight-associated neuro-ocular syndrome (SANS) is hypothesized to arise from a headward fluid shift that elevates intracranial pressure and distends the optic nerve sheath. Head-down tilt (HDT) is the ground-based analog of microgravity, and optic nerve sheath diameter (ONSD) measured by transorbital B-mode ultrasound is a candidate non-invasive marker of intracranial pressure. Before a definitive trial in which lower body negative pressure (LBNP) is added to 12◦ HDT as a putative countermeasure, we conducted a pilot study to (i) confirm that 12◦ HDT detectably elevates ONSD w

Reduced cerebral blood flow does not alter concurrent or post-intervention inhibitory control: evidence from a 3-h head-down tilt protocol in healthy young adults.

A transient increase in cerebral blood flow (CBF) via exercise or hypercapnia has-in part-been linked to an executive function (EF) benefit. It is, however, unclear whether a transient reduction in CBF negatively impacts EF. In the present study, healthy young adults (n = 24) completed separate 3-h interventions involving - 12° head-down tilt (HDT) and a control (0°) supine posture. The HDT protocol was used because it induces a cephalad fluid shift that safely and reliably decreases CBF. To estimate CBF, transcranial Doppler ultrasound measured middle cerebral artery velocity (MCAv) at discre

The drainage volume control by elevation of drainage height versus head down tilt in supine position for management of cerebrospinal fluid leakage following lumbar posterior surgery.

OBJECTIVE: To compare the clinical outcomes of control of drainage volume through either elevating the drainage height or tilting the head down in a supine position for the management of cerebrospinal fluid leakage (CSFL) following posterior lumbar surgery. METHODS: A retrospective analysis was conducted to review the data of patients who underwent lumbar spine surgery at a single hospital over a 4-year period from January 2020 to December 2023. Postoperative CSFL and complications were recorded. All patients with CSFL were managed with bed rest, a 20-30° head-down tilt position, or a drainag

Head Down Tilt 15° to Increase Collateral Flow in Acute Ischemic Stroke: Rationale and Study Protocol of a Multicenter, Randomized, Proof-of-Concept, Phase 2a/b Trial in Patients Treated With Mechanical Thrombectomy (DOWN-SUITE).

BACKGROUND: Collateral blood flow is a critical determinant of successful recanalization in acute ischemic stroke caused by large vessel occlusion. Head down tilt -15° (HDT15), similar to Trendelenburg positioning, is a simple, low-cost positional therapy that may augment cerebral collateral blood flow and penumbral survival. The aim of the study is to assess the safety, feasibility, and efficacy of HDT15 in improving cerebral collateral circulation and clinical outcomes in patients with large vessel occlusion-acute ischemic stroke treated with mechanical thrombectomy (MT). METHODS: The DOWN-

Effects of antioxidant supplementation on bone mineral density, bone mineral content and bone structure in healthy men during 60 days of 6° head-down tilt bed rest: Results from a randomised controlled trial.

Dietary countermeasures to mitigate detrimental spaceflight-induced effects on bone health would alleviate the requirements and the consequences imposed by other types of countermeasures for this risk. We hypothesised that antioxidant supplementation during 60 days of 6° head-down tilt bed rest (HDBR), an analogue of spaceflight, would have a protective effect on bone mineral density (BMD), content (BMC) and bone structure parameters. An exploratory, randomised, controlled, single-blind intervention trial was conducted in a parallel design with 20 healthy male volunteers (age 34 ± 8 y, weight

Effect of a five minutes isokinetic one leg knee extension training on maximal oxygen uptake during 20 days of -6 degrees head-down tilting bed-rest in young male subjects.

Due to prolonged bed rest (BR), maximal oxygen uptake (VO2max) might be decreased by above -8%, even if a moderate supine bicycle exercise (SBE) was performed for 45 to 60 minutes every day during BR. However, the decrement of VO2max was lowered to below -5% by isometric training and using a reverse gradient garment. Further it was rather slightly increased by SBE training with gradually increasing load from 40% to 90% VO2max. The facts suggest the dependency of the decrease of VO2max during BR on exercising muscle mass (MM) and strength. In the present study, therefore, the effects of isokine

[Spread of hyperbaric local anesthetics in a spinal canal model. The influence of Trendelenburg position and spinal configuration].

BACKGROUND: The influence of Trendelenburg positions and variations in spinal canal configuration on the spread of hyperbaric spinal anesthetics was examined in two models of the subarachnoid space. METHODS: Both models included simulations of the spinal cord, filum terminale and cerebrospinal fluid. Model I had a straight shape, thus omitting replications of lumbar lordosis and thoracic kyphosis. It allowed the evaluation of fluid dynamics and the spread of 0.5% hyperbaric bupivacaine and 4% hyperbaric mepivacaine in 0° (supine position), 5° and 10° head-down tilt positions. Model II include

[A case of acute myocardial infarction during laparoscopic rectal amputation].

An 80-year-old man with chronic bronchitis and hypertension was admitted with rectal cancer and cholecystolithiasis. Under general and epidural anesthesia, laparoscopic cholecysytectomy was performed with slightly elevated EtCO2. He underwent head-down tilt for laparoscopic rectectal amputation. Twenty five minutes after positioning, he developed ventricular tachycardia and died. Autopsy showed acute myocardial infarction super-imposed on the scar of lateral wall from old infarction. The laparoscopic operating techniques have become popular, but we do not know the safety limits of the pneumope

[Occupational work capacity and functional status of an operator during repeated exposure to optokinetic and antiorthostatic forces].

It has been shown that optokinetic stimulation that elicits sensory-autonomic manifestations of motion sickness and head-down tilt that simulates fluid redistribution in the cranial direction decrease professional abilities of the operator even after he has developed good skills. Repeated exposures to optokinetic stimulation or head-down tilt have some training effect. However, control tests that include both optokinetic stimulation and head-down tilt give evidence that their training effect is not adequate.

Changes in cerebral blood volume with changes in position in awake and anesthetized subjects.

UNLABELLED: Changes in posture affect cerebral blood volume (CBV), and moderate head-up tilt is used as a therapeutic maneuver to reduce CBV and intracranial pressure. However, CBV is rarely measured in the clinical setting. Near-infrared spectroscopy allows real-time bedside monitoring of cerebral hemodynamics, and we have used this technique to measure changes in CBV with changes in posture in 10 normal subjects and 10 propofol-anesthetized patients. In the awake subjects, changes in CBV were correlated with the degree of table tilt. CBV decreased with 18 degrees head-up tilt and increased w

Reversal of Coma With Trendelenburg Position in Spontaneous Intracranial Hypotension.

Spontaneous intracranial hypotension (SIH) is a clinical and radiologic syndrome caused by spinal leakage of cerebrospinal fluid due to a dural tear, leaking meningeal diverticulum, or cerebrospinal fluid-venous fistula. Whereas the hallmark clinical feature of SIH is orthostatic headache, in rare instances, life-threatening complications may include altered consciousness and even coma as a result of extreme downward displacement of the midbrain and brainstem. We describe the clinical features, neuroimaging findings, management strategies, and short-term outcomes of 2 unique cases of severe SI

Positional asphyxia: accidental death due to head-down position in a badger sett.

Death from positional asphyxia occurs when the victim is found in a position that interferes with adequate breathing. We present an unusual case of a 67-yearold man who was found deceased trapped in a badger sett. The autopsy revealed marked congestion, cyanosis and a multitude of pinpoint hemorrhages in the face, neck and upper chest. Confluent scleral and conjunctival hemorrhage were observed and numerous petechial bleedings were found in the oral mucosa and within the reflected scalp. Death was attributed to positional asphyxia due to head-down position in a badger sett.

Optimal Insertion Depth of Gastric Decompression Tube with a Thermistor for Patients Undergoing Laparoscopic Surgery in Trendelenburg Position.

Monitoring core temperature is crucial for maintaining normothermia during general anesthesia. Insertion of a gastric decompression tube (GDT) may be required during laparoscopic surgery. Recently, a newly designed GDT with a thermistor for monitoring esophageal temperature has been introduced. The purpose of the present study was to evaluate the optimal insertion depth of a GDT with a thermistor. Forty-eight patients undergoing elective laparoscopic surgery in the Trendelenburg position were included in the study. The GDT was inserted to a depth of nose-earlobe-xiphoid distance (NEX) + 12 cm
