Editorial: Global elimination of lymphatic filariasis: fact or fantasy?

In 1997, the World Health Assembly resolved to eliminate lymphatic filariasis as a public health problem by the year 2020. By the end of 2004, almost half of the 83 endemic countries had initiated national programmes, providing mass drug administration to an at risk population of approximately 435 million. This remarkable achievement is the result of an enormous amount of technical, financial and political support from public and private sectors at the community, national, regional and global level. As the global programme to eliminate lymphatic filariasis enters its second quarter of operatio

Resource allocation in Pakistan's health sector: a critical appraisal and a path toward the Millennium Development Goals.

Pakistan is trying hard to sustain its progress toward the Millennium Development Goals. However, because of a lack of political commitment to innovative solutions to improve its financing mechanism, the health system is unable to provide even essential and basic services to the people. The country, with more than 70% of the population living on less than two US dollars a day, largely depends on direct taxes for its revenue. Because of inadequate financing, the quality of government services is inexcusably poor; therefore, a majority of people seek healthcare in the private sector. This has le

Human resource shortage in India's health sector: a scoping review of the current landscape.

BACKGROUND: For healthcare delivery to be optimally effective, health systems must possess adequate levels and we must ensure a fair distribution of human resources aimed at healthcare facilities. We conducted a scoping review to map the current state of human resources for health (HRH) in India and the reasons behind its shortage. METHODS: A systematic search was conducted in various electronic databases, from the earliest available date till February 2024. We applied a uniform analytical framework to all the primary research reports and adopted the "descriptive-analytical" method from the n

Health-sector responses to address the impacts of climate change in Nepal.

Nepal is highly vulnerable to global climate change, despite its negligible emission of global greenhouse gases. The vulnerable climate-sensitive sectors identified in Nepal's National Adaptation Programme of Action (NAPA) to Climate Change 2010 include agriculture, forestry, water, energy, public health, urbanization and infrastructure, and climate-induced disasters. In addition, analyses carried out as part of the NAPA process have indicated that the impacts of climate change in Nepal are not gender neutral. Vector-borne diseases, diarrhoeal diseases including cholera, malnutrition, cardiore

Lumbar spinal fusion patients' demands to the primary health sector: evaluation of three rehabilitation protocols. A prospective randomized study.

Very few studies have investigated the effects or costs of rehabilitation regimens following lumbar spinal fusion. The effectiveness of in-hospital rehabilitation regimens has substantial impact on patients' demands in the primary health care sector. The aim of this study was to investigate patient-articulated demands to the primary health care sector following lumbar spinal fusion and three different in-hospital rehabilitation regimens in a prospective, randomized study with a 2-year follow-up. Ninety patients were randomized 3 months post lumbar spinal fusion to either a 'video' group (one-t

Healthcare system resiliency: The case for taking disaster plans further--Part 1.

To establish true healthcare resiliency, and to better position healthcare organisations to provide effective response, continuity, resumption and recovery of fundamental services and operations during serious incidents and disasters, the disaster planning process must evolve into an integrated approach of four contingency planning disciplines that holistically examine the end-to-end, all-hazard response continuum. This process also needs to incorporate and scale multifarious organisational levels and, when required, the health sector. This paper is the first component of two independent, but

Patients or customers: ethical limits of market economy in health care.

There is a move away from a market economy in health care in the United States and a move towards such a market in Germany. This article tries to make explicit what underlies the moral intuition that there is a tension between a market economy and health care. First, health care is analyzed in terms of the economic theory of the market and incompatibilities are described. The moral problem is identified as the danger of liquefying the distinction between persons and things. The basic moral intuition seems to be the classical social contract: as a functioning market is governed by the principle

Social Network Analysis and Relational Coordination: Empowering Nurse Administrators Leading Care Coordination Teams to Assess Improvement Opportunities.

OBJECTIVE: To explore cross-sector collaboration in complex care transition planning by care coordinators through discussion of a hypothetical practice application and recent research application of social network analysis and relational coordination. BACKGROUND: With growing evidence showing the impact of health-related social needs on patient outcomes, funding and regulatory agencies demand care models that link health and social organizations. Programs like medical respite for people experiencing homelessness rely on cross-sector collaboration, the structured alliance of multiple agencies

A Survey on Use of Rapid Tests and Tuberculosis Diagnostic Practices by Primary Health Care Providers in South Africa: Implications for the Development of New Point-of-Care Tests.

BACKGROUND: Effective infectious disease control requires early diagnosis and treatment initiation. Point-of-care testing offers rapid turn-around-times, facilitating same day clinical management decisions. To maximize the benefits of such POC testing programs, we need to understand how rapid tests are used in everyday clinical practice. METHODS: In this cross-sectional survey study, 400 primary healthcare providers in two cities in South Africa were interviewed on their use of rapid tests in general, and tuberculosis diagnostic practices, between September 2012 and June 2013. Engaging the private sector to increase access to HIV services and increase sustainability of the HIV response.

BACKGROUND: Public health facilities often manage high volumes of HIV clients and face challenges with long wait times, poor client satisfaction, and low retention in care. The private sector is a major provider of health services in many countries but is a relatively untapped resource for reducing congestion in high-volume public HIV facilities and increasing access to HIV services, particularly for clients who already use the private sector as their first point of care. Private banks, corporations, and clients themselves can also be a resource for generating additional financing for HIV and

Cases of mobbing activities as commonly seen in the healthcare sector in the world and in Turkey.

Service process on a site in the health sector does not only bring people and services into contact, but also all other people who share the fate of particular working medium. Although these people should cooperate and care about each other, contributing new merits to humanity with their synergy, meeting of people with the other people from time to time intertwine into a bitter, pain giving relation, like the way it happens with meeting of people with services. Owing to its controversy to humanitarian nature, religious and social values, the pain that human beings cause to one another is occas

Cost-description of a pilot parenteral vaccination campaign against rabies in dogs in N'Djaména, Chad.

In the discussion about policies and strategies for rabies prevention in developing countries, intervention costs arise as a major issue. In a pilot mass vaccination campaign against rabies in N'Djaména, Chad, 3000 dogs were vaccinated. We assessed vaccination coverage and cost, showing the cost per dog vaccinated for the public sector and for society. An extrapolation to city level calculated the approximate cost of vaccinating all 23 600 dogs in N'Djaména. In the pilot mass campaign with 3000 dogs the average cost per dog was 1.69 euro. to the public and the full societal cost was 2.45 euro.

Including mental health care in a model of European health system.

The management of a health system is a matter of economics and business administration because of the costs induced by goods and services delivered. Economics teaches us that the positive effects induced by competition in free markets cannot be expected in health care, which is a classic example of market failure from both demand and supply sides. The most sensible key concepts to refer for managing a health system are funding and provision. While the logical solution for the first variable is universal coverage through general taxation, the second one requires a deeper understanding. Integrat
