Electronic health records are replacing conventional paper-based health records. For a doctor it is a working instrument, which can significantly reduce the time spent on paper work. Patients can benefit from accessing the electronic health records even though they usually do not have a medical background. Therefore, when specifying a graphical user interface (GUI) it is necessary to take into account the requirements of the different users: e.g. the functionality for the doctors and the presentation of data in an understandable manner for the patients. The study aims to review and analyze met

Starting in 2009, the first ever Belgian nationwide data collection network using routine data extracted from primary care EPR (upload method) has been built from scratch. The network also uses a manual web-based data collection method. This paper compares these two methods by analysing missing and most recent values for certain parameters. We collected data from 4954 practices, pertaining to 29,180 patients. Mean values for the most recent parameters were similar regardless of which data collection method was used. Many missing recent values (>46%) were found for all of the parameters when

BACKGROUND: As a joint project with the Ministry of Health, Labour and Welfare (MHLW), the Research Group on Intractable Renal Diseases is examining the feasibility of utilizing its personal clinical records database. We examine the validity of the initial-treatment data from the personal clinical records of patients with new-onset rapidly progressive glomerulonephritis (RPGN).
METHODS: Personal clinical records for patients with either RPGN or anti-glomerular basement membrane (GBM) antibody nephritis were used. The data from 454 newly enrolled RPGN patients were compiled for analysis in 2 c

Electronic health records (EHRs) enable patients to access their health records anytime, from anywhere with internet connectivity. Yet not all Americans benefit from these innovations. EHRs can be hard to access for people with a range of disabilities. This lack of access perpetuates inequity and, thus, demands ethical and legal attention. Some federal laws and regulations require accessible EHRs, but even these protections can fall short. This article argues that more clearly defined obligations for EHR developers and clinicians are necessary.

OBJECTIVE: Evictions are important social and behavioral determinants of health. Evictions are associated with a cascade of negative events that can lead to unemployment, housing insecurity/homelessness, long-term poverty, and mental health problems. In this study, we developed a natural language processing system to automatically detect eviction status from electronic health record (EHR) notes.
MATERIALS AND METHODS: We first defined eviction status (eviction presence and eviction period) and then annotated eviction status in 5000 EHR notes from the Veterans Health Administration (VHA). We d

Individuals with serious mental illnesses have high rates of comorbid physical health issues and have numerous barriers to addressing their health and health care needs. The present pilot study tested the feasibility of a modified form of the "Bridge" peer-health navigator intervention delivered in a usual care setting by agency personnel. The modifications concerned the use of an electronic personal health record with individuals experiencing with housing instability. Twenty participants were randomized to receive the intervention immediately or after 6 months. Health navigator contacts and u

BACKGROUND: The availability of well-documented records during labor and delivery is essential for monitoring childbirth and ensuring positive outcomes for mothers and newborns. However, recent research in northwestern Ethiopia shows that data recording during these critical periods is inadequately performed. This study explored the barriers and enablers of data recording practices during institutional childbirth in this region.
METHODS: A descriptive qualitative study was conducted in six health centers in the Wegera district, focusing on three source documents: one facility-level log (the D

OBJECTIVE: To provide expectant mothers with a USB stick containing their maternity notes, including ultrasound images, and to assess its use and acceptability versus conventional care with exclusively hospital-held notes.
STUDY DESIGN: USB group: 200 women attending Zurich University Hospital Obstetrics Department for antenatal-to-postnatal care in 2006-2007.
CONTROLS: 200 women attending the Obstetrics Department for delivery only, after receiving conventional antenatal care elsewhere. Women were interviewed using an essentially identical postpartum questionnaire for each group, with minor

Patients increasingly expect timely, secure access to health information, a right reinforced by the Health Insurance Portability and Accountability Act and expanded under the 21st Century Cures Act. Yet millions of individuals receiving care in jails and prisons remain excluded from these advances. Incarcerated populations bear a disproportionate burden of chronic disease, psychiatric illness, and substance use disorders. For many, incarceration presents consistent contact with healthcare, though access to personal medical records during and after detention remains limited. Although the Cures

Using a multi-institutional EMR registry, we extracted housing status and evaluated the presence of several important comorbidities in order to describe the demographics and comorbidity burden of persons experiencing homelessness in northeast Ohio and compare this to non-homeless individuals of varying socioeconomic position. Of 1,974,766 patients in the EMR registry, we identified 15,920 (0.8%) as homeless, 351,279 (17.8%) as non-homeless and in the top quintile of area deprivation index (ADI), and 1,607,567 (81.4%) as non-homeless and in the lower four quintiles of area deprivation. Two prospective surveys of the immunisation state and its documentation were conducted among children under 2 years old attending a children's hospital. A survey of 111 children attending the casualty or outpatient departments showed that, according to reliable records, 106 (95%) were fully immunised for age with oral polio vaccine, 93 (84%) with triple antigen, and 26 of 33 children greater than or equal to 16 months of age (79%) with measles-mumps vaccine. A survey of 204 inpatients showed that, according to verified records, significantly fewer inpatients than outpatients were fully immunis

INTRODUCTION: The CONDUIT-HID intervention integrates patients' electronic blood pressure measurements directly into the clinical EHR using Microsoft HealthVault as an intermediary data store. The goal of this paper is to describe generalizable categories of patient and technical challenges encountered in the development and implementation of this inexpensive, commercial off-the-shelf consumer health informatics intervention, examples of challenges within each category, and how the example challenges were resolved prior to conducting an RCT of the intervention.
METHODS: The research team logg

We used record linkage to create a data repository of health information of persons who were federally incarcerated in Ontario and Canada. We obtained records from 56,867 adults who were federally incarcerated between January 1, 1998 and December 31, 2011 from the Correctional Service of Canada; 15,248 records belonged to individuals residing in Ontario, Canada. We linked these records to the Registered Persons Database (RPDB) which contained records from 18,116,996 individuals eligible for health care in Ontario. Out of 56,867 OMS records, 22,844 (40.2%) were linked to the RPDB. Looking only

In this paper, we describe our method for skin lesion classification. The goal is to classify skin lesions based on dermoscopic images to several diagnoses' classes presented in the HAM (Human Against Machine) dataset: melanoma (MEL), melanocytic nevus (NV), basal cell carcinoma (BCC), actinic keratosis (AK), benign keratosis (BKL), dermatofibroma (DF), and vascular lesion (VASC). We propose a simplified solution which has a better accuracy than previous methods, but only predicted on a single model that is practical for a real-world scenario. Our results show that using a network with additio

OBJECTIVE: The goal was to determine whether patient-held vaccination records improve vaccination rates.
METHODS: The public-use files of the 2004-2006 National Immunization Survey, a national, validated survey of households with children 19 to 35 months of age, were used. The main outcome was up-to-date (UTD) vaccination status (4 diphtheria-tetanus-acellular pertussis/diphtheria-tetanus vaccine, 3 poliovirus vaccine, 1 measles vaccine, 3 Haemophilus influenza type B vaccine, and 3 hepatitis B vaccine doses), and the main predictor was the use of a vaccination record. Control variables were

OBJECTIVES: Diagnosis is ubiquitous in Psychiatry, and whilst it does bring benefits; adverse effects of 'labelling' may also be possible. This study aimed to evaluate experimentally whether clinicians' judgements about a patient with panic disorder were influenced by an inappropriately suggested diagnosis of comorbid borderline personality disorder (BPD).
DESIGN: An experimental design was used to evaluate clinician's judgements when the nature of the information they were given was varied to imply BPD comorbidity.
METHODS: Two hundred and sixty-five clinicians watched a video-recorded asse

Gestational diabetes is a condition occurring in up to 18% [1] of pregnant women that results in an increase in blood glucose levels due to the body's inability to produce sufficient insulin given the additional needs of the baby, and/or hormonal changes that lower the body's sensitivity to insulin. If left untreated, the growing baby may become too large, increasing the risk of injury to the mother and baby during delivery. Controlling blood glucose can be a challenging task, especially for women with no previous experience and who may have unhealthy diets. An opportunity exists to further en

BACKGROUND: Systematic review of amended reports in surgical pathology has been recommended as a valuable exercise in promoting quality assurance and improvement. Examination of report amendments can identify defects in the surgical pathology process and inspire new approaches to decreasing error rates and improving overall patient care.
METHODS: We performed a retrospective review of all amended dermatopathology reports over a 1.5-year period at a large academic institution.
RESULTS: During the study period, 86 amended reports out of a total 7950 skin-specific reports were issued (1.08%). O

Large language models are enabling a new generation of health agents, yet their ability to address the diverse needs of individuals in everyday health settings remains underexplored1,2. Here we developed a comprehensive personal health agent that reasons over multimodal data from consumer wellness devices and medical records to provide personalized health support. Through analysis of web search and health forum queries, together with user-centred research involving end users and health experts, we identified four key user journeys: general health knowledge, personal data interpretation, wellne
