Variations in risk perceptions: a qualitative study of why unnecessary urinary catheter use continues to be problematic.

BACKGROUND: Catheter associated urinary tract infection (CAUTI) is one of the most commonly acquired health care associated infections within the United States. We examined the implementation of an initiative to prevent CAUTI, to better understand how health care providers' perceptions of risk influenced their use of prevention practices and the potential impact these risk perceptions have on patient care decisions. Understanding such perceptions are critical for developing more effective approaches to ensure the successful uptake of key patient safety practices and thus safer care for hospita

Misuse of Prescription Pain Relievers, Stimulants, Tranquilizers, and Sedatives Among U.S. Older Adults Aged ≥50 Years.

INTRODUCTION: Misuse of prescription medications has led to higher mortality and morbidity in the U.S. This study investigates medical and nonmedical use of prescription medications (prescription pain relievers, stimulants, prescription tranquilizers, and sedatives) among adults aged ≥50 years. METHODS: Data were from the 2015-2017 National Survey of Drug Use and Health. For each prescription medication, medical and nonmedical use were mutually exclusive (prescription pain relievers, stimulants, tranquilizers, and sedatives). Nonmedical use (misuse) was defined as the use of any prescription

Treatment of Attention-Deficit/Hyperactivity Disorder With Stimulants: Reducing Harms.

An increase in attention-deficit/hyperactivity disorder (ADHD) diagnoses since 2020, along with shortages in prescription stimulants and an increase in overdose deaths from stimulants, has triggered alarm within the public sector, regulatory agencies, and health care systems. Stimulants are classified as Schedule II drugs by the Drug Enforcement Administration due to the potential for misuse and physiological and psychological dependence. Despite concerns, stimulants are effective first-line treatments for children and adults with ADHD. The majority of deaths related to stimulants are linked t

Nigerian medical students' opinions about individuals who use and abuse psychoactive substances.

Substance use disorders are prevalent in Nigeria. The number of available specialist health providers is inadequate to fill the treatment gap. Interventions can be provided by nonspecialist health providers and have been found to be beneficial. However, attitudes toward substance misuse and misusers can impede the provision of this service. We aimed to determine attitudes of medical trainees toward substance use by utilizing a modified form of the Substance Abuse Attitude Scale (SAAS). Medical students (n = 200) had positive attitudes toward individuals who misuse psychoactive substances. The

State targeted response to the opioid Crisis grants (opioid STR) program: Preliminary findings from two case studies and the national cross-site evaluation.

Opioid misuse is a national health crisis that requires sustained treatment, prevention, and recovery efforts. This study evaluates the innovative treatment approaches that two states - Kentucky and Missouri - implemented in their states using State Targeted Response to the Opioid Crisis Grant (Opioid STR) program funds from the Substance Abuse Mental Health and Services Administration (SAMHSA), as well as preliminary findings from the Opioid STR national, cross-site evaluation that is funded and managed by SAMHSA. The Kentucky approach discusses the Emergency Department (ED) bridge model, whi

Offending behaviour, health and wellbeing of military veterans in the criminal justice system.

BACKGROUND: A small but significant proportion of military veterans become involved in the criminal justice system (CJS) after leaving service. Liaison and Diversion (L&D) services aim to identify vulnerable offenders in order to provide them with the health/welfare support they need, and (where possible) divert them away from custody. An administrative database of L&D service-users was utilised to compare the needs of veterans with those of non-veteran L&D service-users. METHOD: National data collected from 29 L&D services in 2015-2016 was utilised. Of the 62,397 cases, 1,067 (2%) reported p

Do Junior Entrants to the UK Armed Forces have worse outcomes than Standard Entrants?

INTRODUCTION: The UK is the only permanent member of the UN Security Council that has a policy of recruiting 16 and 17 year old individuals into its regular Armed Forces. Little is known about the consequences of enlisting as a Junior Entrant (JE), although concerns have been expressed. We compare the mental health, deployment history, and pre-enlistment and post-enlistment experiences of personnel who had enlisted as JEs with personnel who joined as Standard Entrants (SEs). METHOD: Participants from a large UK military cohort study completed a self-report questionnaire between 2014 and 2016

Substance use disorders in the U.S. Armed Forces, 2000-2011.

Drug misuse is associated with serious health consequences and has detrimental effects on military readiness. During 2000 to 2011, 70,104 service members received an incident diagnosis of a substance use disorder (SUD) (excluding alcohol and tobacco-related disorders). Incidence rates declined with increasing age, time in service, rank, and number of combat deployments. Service members in a combat occupation had 1.2 times the rate of individuals in a health care or administation/supply occupation. The median time to discharge after an SUD diagnosis was longest in the Air Force (327 days) and s

Patterns and characteristics of 'calls of despair': a service evaluation using Yorkshire Ambulance Service data.

INTRODUCTION: 'Deaths of despair' (DoD) - encompassing fatalities from suicide, drug overdoses and alcohol-related causes - represent a growing public health crisis. Socioeconomic vulnerability and healthcare disparities are well-documented drivers of DoD. While healthcare contacts preceding despair-related deaths have been studied extensively, the role of ambulance services is underexplored. This study aimed to address this gap by utilising ambulance service data to provide insights into 'calls of despair' received by a UK ambulance service over a 12-month period. METHODS: This exploratory,

Prospective Associations Among Loneliness and Health for Servicemembers: Perceived Helplessness and Negative Coping Appraisal as Explanatory Mechanisms.

Links between loneliness and health are robust, though evidence for associations with alcohol use is mixed. Previous research has supported perceived stress as a predictor of alcohol use and as a pathway through which loneliness impacts health over time. Yet findings are primarily limited to civilian samples, and less is known about how loneliness relates to stress and health among service members. The current study explores prospective associations among loneliness, stress, and health (i.e., sleep, alcohol misuse, and psychological distress) within a sample of mostly male service members. We
