CONTEXT: Hypertension is a common and costly risk factor for cardiovascular disease, but just over half of all adults with hypertension have their blood pressure controlled nationally. In Minneapolis-St Paul, Minnesota, the rate of hypertension control is approximately 70% despite a rate of hypertension control similar to the national average as recently as the first half of the 1990s.
OBJECTIVE: The purposes of this study were to identify factors in Minneapolis-St Paul and state-level policies and programs in Minnesota that may have contributed to the more rapid increase in blood pressure co

In Oklahoma, the infant mortality rate is higher than the national rate. Fetal Infant Mortality Review (FIMR) is an effective perinatal systems intervention. FIMR gathers information from medical, social, and community sources taking a comprehensive look at how well these systems are serving women and infants. It is important to understand that FIMR is a method to develop community-based plans to reduce fetal and infant mortality. It is not a quick-fix solution to a complex problem, a research study, an institutional review, or a system for assigning blame or responsibility for a death. FIMR h

BACKGROUND: Repeated outbreaks of emerging pathogens underscore the need for preparedness plans to prevent, detect, and respond. As countries develop and improve National Action Plans for Health Security, addressing subnational variation in preparedness is increasingly important. One facet of preparedness and mitigating disease transmission is health facility accessibility, linking infected persons with health systems and vice versa. Where potential patients can access care, local facilities must ensure they can appropriately diagnose, treat, and contain disease spread to prevent secondary tra

The prevalence of anthrax in both animal and human populations has been increasing in Africa. It was therefore appropriate for this WHO meeting to be convened in an endemic area of the Western Province of Zambia in 1992. The participants reviewed anthrax epidemiology and control in some African countries, elaborated national anthrax control and research programmes in Africa, discussed international cooperation and work plans, and elaborated recommendations for anthrax control in Africa. The discussions centred on anthrax surveillance and reporting systems, diagnosis, vaccine production and imm

There is growing evidence that climate change adversely affects human health. Multiple diseases are sensitive to climate change, including cardiovascular diseases (CVDs), which are also the leading cause of death globally. Countries such as India face a compounded challenge, with a growing burden of CVDs and a high vulnerability to climate change, requiring a co-ordinated, multi-sectoral response. In this framework synthesis, we analysed whether and how CVDs are addressed with respect to climate change in the Indian policy space. We identified 10 relevant national-level policies, which were an

This paper discusses school safety mainly in relation to safety education and examines measures for the comprehensive development of systems to ensure the safety of children at school, including those requiring medical care. The number of children requiring medical care is increasing in general schools following the promotion of inclusive education. The government of Japan has established the School Health and Safety Act and Guidelines on School Curricula. Municipalities have developed education systems that cover the safety education specified in disaster preparedness plans for schools. Safet

The growing intersection between climate change and human mobility argues that migration, displacement, and immobility are increasingly shaped by both sudden-onset and slow-onset climate hazards, alongside underlying social and governance vulnerabilities. Most climate-related mobility occurs within national borders and carries considerable implications for health, livelihoods, and urban systems. Global frameworks such as the Global Compact for Migration and the UN Framework Convention on Climate Change have begun to acknowledge these dynamics; however, a major adaptation gap is identified at t

BACKGROUND: Large epidemics frequently emerge in conflict-affected states. We examined the cholera response during the humanitarian crisis in Yemen to inform control strategies.
METHODS: We conducted interviews with practitioners and advisors on preparedness; surveillance; laboratory; case management; malnutrition; water, sanitation and hygiene (WASH); vaccination; coordination and insecurity. We undertook a literature review of global and Yemen-specific cholera guidance, examined surveillance data from the first and second waves (28 September 2016-12 March 2018) and reviewed reports on airst

BACKGROUND: Many developing countries increasingly recognize and acknowledge family planning as a critical part of socio-economic development. However, with few health dollars to go around, countries tend to provide essential drugs for curative care, rather than for family planning products. Donors have historically provided free contraceptives for family planning services. Whether products are donated or purchased by the country, a successful family planning program depends on an uninterrupted supply of products, beginning with the manufacturer and ending with the customer. Emergence of many emerging or re-emerging infectious diseases have occurred over the past decade, some of which are major public threat. SARS, MERS-CoV, highly pathogenic avian influenza A(H5N1), Ebola virus disease have raised concerns because of their virulence, their mortality, and/or their modality of transmission, or their impact on maternofoetal transmission (Zika virus). The witness of these emergences have conducted health authorities to have policies and plans and to imagine new organizations for health systems in order to identify any case of highly communicable virulent disease for

BACKGROUND: Mass campaigns are a key strategy for delivering life-saving interventions under Global Health Initiatives, especially in weak health system contexts. They are frequently designed parallel to the health system to rapidly achieve programme targets such as vaccination coverage, but we lack quantitative evidence demonstrating their impact and effect mechanisms on health system performance at sub-/national level. This longitudinal study responds to this gap through an analysis of polio eradication campaigns in Nigeria.

Antimicrobial resistance (AMR) represents one of the most pressing global health and development challenges of the twenty-first century, threatening the effectiveness of antimicrobial therapies and undermining the clinical management of infectious diseases worldwide. The increasing prevalence of resistant pathogens undermines progress in disease control, surgery, and critical care, and poses severe socioeconomic and public health consequences. This narrative review synthesizes multidisciplinary evidence to examine the global trends, determinants, and implications of AMR through an integrated O

BACKGROUND: Systemic barriers contribute to challenges in implementing sustainable improvements in early care and education (ECE) health promotion practices, including fragmented state systems, limited organizational capacity, and a lack of accessible, high-quality professional development. Guided by the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) framework, this study evaluated the Better Together approach-a multi-level quasi-experimental program aimed at enhancing health promotion practices in ECE programs.
METHODS: Better Together, developed by Nemours Children

The $63 billion comprehensive global health initiative (GHI) emphasizes health systems strengthening (HSS) to tackle challenges, including child and maternal health, HIV/AIDS, family planning, and neglected tropical diseases. GHI and other initiatives are critical to fighting emerging and reemerging diseases in resource-poor countries. HSS is also an increasing focus of the $49 billion program of the US President's Emergency Plan for AIDS Relief and the Global Fund to Fight AIDS, Tuberculosis and Malaria. Laboratory systems and services are often neglected in resource-poor settings, but the fu

There is increasing demand for a systems approach within national healthcare guidelines to provide a systematic and sustainable framework for improvements in patient safety. Supported by this is the growing body of evidence within Human Factors/Ergonomics (HFE) healthcare literature for the inclusion of this approach in health service design, provision and evaluation. This paper considers the current interpretation of this within UK healthcare systems and the dichotomy which exists in the challenge to implement a systems approach. Three case studies, from primary and secondary care, present a

Wildlife and wildlife interfaces with people and livestock are essential surveillance targets to monitor emergent or endemic pathogens or new threats affecting wildlife, livestock, and human health. However, limitations of previous investments in scope and duration have resulted in a neglect of wildlife health surveillance (WHS) systems at national and global scales, particularly in lower and middle income countries (LMICs). Building on decades of wildlife health activities in LMICs, we demonstrate the implementation of a locally-driven multi-pronged One Health approach to establishing WHS in

Respiratory infections place a heavy burden on patients, providers, employers, and health care systems. The prescribing of antibiotics is common, despite the fact that many respiratory conditions are caused by viruses. The economic effect of treating respiratory tract infections with broad-spectrum antibiotics was retrospectively analyzed by means of health care claims data from six managed care health plans affiliated with a large national insurer. A regression model was used to adjust for factors that can influence treatment costs, such as age, baseline cost, retreatment, and drug cost. The

INTRODUCTION: Numerous digital approaches are being explored internationally to support the elicitation, documentation and sharing of advance care planning information. In England, Electronic Palliative Care Coordination Systems have been developed for this purpose, often as a template stored as part of an electronic clinical record system. Despite Electronic Palliative Care Coordination Systems being central to the end-of-life care policy, there has been a lack of exploration of the perspectives of healthcare professionals who use Electronic Palliative Care Coordination Systems and are critic

BACKGROUND: The establishment of a functional information system for human resource for health (HRH) was one of the major challenges for the Tanzanian health sector. In 2008, the Ministry of Health and Social Welfare developed the HRH Strategic Plan, in which establishment of computerized information systems were one of the strategic objectives. In response to this objective, the Ministry developed two information systems, namely the Human Resource for Health Information System (HRHIS) and the Training Institution Information System (TIIS), to capture information from both the public and priva
