A global analysis of the determinants of maternal health and transitions in maternal mortality.

The reduction of maternal mortality and the promotion of maternal health and wellbeing are complex tasks. This Series paper analyses the distal and proximal determinants of maternal health, as well as the exposures, risk factors, and micro-correlates related to maternal mortality. This paper also examines the relationship between these determinants and the gradual shift over time from a pattern of high maternal mortality to a pattern of low maternal mortality (a phenomenon described as the maternal mortality transition). We conducted two systematic reviews of the literature and we analysed pub

The Consequences of Contact with the Criminal Justice System for Health in the Transition to Adulthood.

A rapidly growing literature has documented the adverse social, economic and, recently, health impacts of experiencing incarceration in the United States. Despite the insights that this work has provided in consistently documenting the deleterious effects of incarceration, little is known about the specific timing of criminal justice contact and early health consequences during the transition from adolescence to adulthood-a critical period in the life course, particularly for the development of poor health. Previous literature on the role of incarceration has also been hampered by the difficul

Transitional health care for patients with Hirschsprung disease and anorectal malformations.

BACKGROUND: Hirschsprung disease (HD) and anorectal malformations (ARM) are congenital disorders with potentially lifelong consequences. Although follow-up is performed in most pediatric patients, transfer to adult health care is often problematic. This study assesses transitional care with the help of questionnaires in consultation with adult patients. METHODS: This study was conducted in an outpatient clinic of a pediatric surgical center in the Netherlands. All patients born and treated for ARM or HD before 1992 were invited to visit our clinic. Patients completed questionnaires concerning

Mental Health and Self-Worth in Socially Transitioned Transgender Youth.

OBJECTIVE: Social transitions are increasingly common for transgender children. A social transition involves a child presenting to other people as a member of the "opposite" gender in all contexts (e.g., wearing clothes and using pronouns of that gender). Little is known about the well-being of socially transitioned transgender children. This study examined self-reported depression, anxiety, and self-worth in socially transitioned transgender children compared with 2 control groups: age- and gender-matched controls and siblings of transgender children. METHOD: As part of a longitudinal study

Effects of Velusetrag (TD-5108) on gastrointestinal transit and bowel function in health and pharmacokinetics in health and constipation.

Velusetrag (TD-5108) is a potent, selective high intrinsic activity serotonin 5-HT(4) receptor agonist. We assessed effects of Velusetrag on gastrointestinal transit and compared its pharmacokinetics in healthy volunteers (HV) and chronic constipation (CC) patients. Sixty HV were randomly assigned, double-blind to placebo, 5, 15, 30 or 50 mg Velusetrag (single and 6-day dosing). Primary endpoints were colonic transit (geometric centre at 24 h, GC24) and ascending colon emptying (ACE) T(1/2) after first dose. Secondary endpoints included gastric emptying (GE) T(1/2) and colonic filling at 6 h (

Co-occurrence patterns of multiple health risk behaviors and transition dynamics across adolescence: a five-wave latent class and transition analysis of the Korea youth health panel survey (2019-2023).

PURPOSE: Multiple health risk behaviors (MHRBs) co-occur during adolescence and evolve across developmental stages, yet longitudinal person-centered analyses remain limited in Asian contexts. This study aimed to identify latent classes of co-occurring MHRBs across five annual waves and examine transition dynamics and predictors of favorable transitions among Korean adolescents. METHODS: The data from 5051 adolescents (25,255 person-waves) in the Korea Youth Health Panel Survey (KYHPS, 2019-2023) were analyzed for alcohol use, physical inactivity, poor diet, and short sleep duration. Latent cl

Women in Transition: Experiences of Health and Health Care for Recently Incarcerated Women Living in Community Corrections Facilities.

Health priorities of women after incarceration remain poorly understood, constraining development of interventions targeted at their health during that time. We explored the experience of health and health care after incarceration in a focused ethnography of 28 women who had been released from prison or jail within the past year and were living in community corrections facilities. The women's outlook on health was rooted in a newfound core optimism, but this was constrained by their pressing health-related issues; stress and uncertainty; and the pressures of the criminal justice system. These

Three Decades of Health Care Transition in Rheumatology: Progress, Challenges, and Future Directions.

The revolution of treatment advances for children with rheumatic diseases since the advent of pediatric rheumatology has coincided with the growing recognition that many of these children will continue to require long-term rheumatology care after reaching adulthood. Moreover, it has become evident that effective, systematic approaches are needed to help adolescents and young adults (AYAs) with pediatric rheumatic diseases learn to manage their health independently and succeed in an adult-oriented model of health care. In this narrative literature review, we provide a historical perspective on

Oral Health Across the Menopausal Transition: Biological Pathways, Clinical Implications, and Future Perspectives.

Background/Objectives: Menopause is a complex physiological transition characterized by progressive estrogen deficiency and systemic biological changes that can affect multiple organs and tissues, including the oral cavity. Growing evidence suggests that hormonal fluctuations during the menopausal transition may influence periodontal health, salivary function, oral sensory perception, and overall oral health-related quality of life. Objective: This narrative review aims to provide a comprehensive overview of the biological mechanisms and clinical manifestations associated with the relationship

Nutrition Transition, Processed Foods, and Cardiometabolic Risk in Somalia: A Public Health Commentary.

BACKGROUND: Somalia is undergoing a nutrition transition in which traditional camel milk, sorghum, and legume-centred diets are being progressively displaced by imported processed foods high in refined sugars, saturated fats, and dietary cholesterol. This shift is occurring in a population already burdened by undernutrition and a fragile health system. PURPOSE: This narrative commentary synthesises published evidence on the biochemical mechanisms linking excess sugar and cholesterol intake to dyslipidaemia, type 2 diabetes mellitus (T2DM), and cardiovascular disease, and considers how these m

Transition cow health and management in pasture-based dairy herds: A farmers' survey.

Seasonal-calving pasture-based systems characterize Irish dairy production. During the dry period, cows are housed and offered predominantly grass silage, providing unique transition cow management opportunities. This study aimed to describe transition period disease incidence and management strategies reported by farmers, and to evaluate their associations with herd size and calving pattern to inform and guide research activities and national advisory. An online survey distributed amongst 3,899 Teagasc Technical Dairy Advisory clients yielded 525 responses suitable for analysis. Results are p

Engagement in Household Chores in Youth With Chronic Conditions: Health care Transition Implications.

This study examined associations between chores engagement, self-management, and transition readiness in youth with chronic conditions. Youths with various chronic conditions attending a therapeutic camp, and their parents participated. Responses of 165 campers and their parents were analyzed (mean camper age 12.3 ± 2.6 years, 47.9% males, 79.4% White). The most common diagnoses were diabetes, spina bifida, cerebral palsy, and sickle cell anemia. Youth who completed chores manifested higher overall health care transition readiness (β^ = 5.17, p = .026) and better communication with providers (

Spontaneous Remission in Paroxysmal Nocturnal Hemoglobinuria-Return to Health or Transition Into Malignancy?

Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired syndrome characterized by intravascular hemolysis, thrombosis, and bone marrow failure. The disease is caused by a mutation in the PIG-A gene that leads to the lack of glycosylphosphatidylinositol-anchored complement regulatory molecules CD55 and CD59 on affected blood cell surfaces. In previous studies, spontaneous clinical remissions have been described. The disease manifestations are very heterogeneous, and we wanted to examine if true remissions and disappearance of the clone occur. In a follow-up of a nation-wide cohort of 106 Finni

Assessment of transition readiness in adolescents and young adults with chronic health conditions.

BACKGROUND: Transition from pediatric to adult health care is a vulnerable period for adolescents and young adults. Challenges include paucity of validated measures to assess patients' transition readiness. We evaluated the Transition Readiness Assessment Questionnaire (TRAQ) in adolescents and young adults with rheumatic, gastrointestinal, and endocrine disorders. We examined whether baseline TRAQ scores and other demographic variables predicted transition to adult care over a three year follow up period. METHODS: In this descriptive study at a single institution, eighty-nine adolescents at

Marijuana use trajectories during the post-college transition: health outcomes in young adulthood.

BACKGROUND: Despite the relatively high prevalence of marijuana use among college students, little information exists regarding health outcomes associated with different use patterns or trajectories. METHODS: Seven annual personal interviews (years 1-7) were administered to 1253 individuals, beginning in their first year in college. Growth mixture modeling was used to identify trajectories of marijuana, alcohol, and tobacco use frequency during years 1-6. Logistic regression was used to evaluate the relationship between marijuana use trajectories and several year 7 health outcomes, holding co

Patterns of childhood and adolescent overweight and obesity during health transition in Vanuatu.

OBJECTIVE: Rapid economic development and subsequent changes in lifestyle and disease burdens ('health transition') is associated with increasing prevalence of obesity among both adults and children. However, because of continued infectious diseases and undernutrition during the early stages of transition, monitoring childhood obesity has not been prioritized in many countries and the scope of the problem is unknown. Therefore we sought to characterize patterns of childhood overweight and obesity in an early transitional area, the South Pacific archipelago of Vanuatu. DESIGN: We completed an

Effect of transition home from combat on risk-taking and health-related behaviors.

Transition home following a combat deployment involves a period of adjustment. Exploratory and confirmatory factor analyses of a new 16-item transition scale were conducted with 2 samples and resulted in 4 factors (Benefit, Appreciation, Anger/Alienation, and Guilt/Remorse). In Study 1 (N = 1,651), the number of combat events was positively related to Anger/Alienation 4 months later even after controlling for posttraumatic stress disorder (PTSD) symptoms, partial r = .18, p < .001. In Study 2 (N = 647), after controlling for PTSD symptoms, Anger/Alienation assessed at 4 months postdeployment p

Epidemiological and nutrition transition in developing countries: impact on human health and development.

Whereas common infectious and parasitic diseases such as malaria and the HIV/AIDS pandemic remain major unresolved health problems in many developing countries, emerging non-communicable diseases relating to diet and lifestyle have been increasing over the last two decades, thus creating a double burden of disease and impacting negatively on already over-stretched health services in these countries. Prevalence rates for type 2 diabetes mellitus and CVD in sub-Saharan Africa have seen a 10-fold increase in the last 20 years. In the Arab Gulf current prevalence rates are between 25 and 35% for t
