Outcomes of evaluation and testing of 660 individuals with hearing loss in a pediatric genetics of hearing loss clinic.

Hearing loss is a relatively common condition in children, occurring in approximately 2 out of every 1,000 births with approximately 50% of reported diagnoses having a primary genetic etiology. Given the prevalence and genetic component of hearing loss, coupled with a trend toward early diagnosis with the institution of universal newborn hearing screening, The Genetics of Hearing Loss Clinic was established at The Children's Hospital of Philadelphia to manage the diagnosis, testing, and genetic counseling for individuals and families. This paper described a cohort of 660 individuals with a dia

Rotatory and collic vestibular evoked myogenic potential testing in normal-hearing and hearing-impaired children.

OBJECTIVES: Vertigo and imbalance are often underestimated in the pediatric population, due to limited communication abilities, atypical symptoms, and relatively quick adaptation and compensation in children. Moreover, examination and interpretation of vestibular tests are very challenging, because of difficulties with cooperation and maintenance of alertness, and because of the sometimes nauseatic reactions. Therefore, it is of great importance for each vestibular laboratory to implement a child-friendly test protocol with age-appropriate normative data. Because of the often masked appearance

Development and Validation of a Self-Administered Online Hearing Test.

We describe the development and validation of a self-administered online hearing test, which screens for hearing loss and provides an estimated audiogram. The hearing test computes test results from age, self-reported hearing abilities, and self-assessed pure-tone thresholds. It relies on regression, Bayesian and binary classification, leveraging probabilistic effects of age as well as interfrequency and interaural relationships in audiograms. The test was devised based on development data, collected prospectively in an online experiment from a purposive convenience sample of 251 adult America

Assessment of hearing sequelae in individuals tested post-trauma.

BACKGROUND: Over 5% of the global population (430 million people) require rehabilitation for hearing loss. Individuals with hearing impairments face significant challenges in business, daily life, and social participation. Hearing loss (HL) and other permanent physical and sensory disabilities escalate dramatically in cases with brain damage and temporal bone trauma associated with head injuries. This study aims to identify the significant risk factors for hearing loss following head trauma, utilizing current data, and discuss the findings in the context of the literature. This could contribut

Development and validation of a digits-in-noise hearing test in Persian.

OBJECTIVE: The prevalence of unrecognised and late-diagnosed hearing loss is higher in low- and middle-income than in high-income countries, due in part to lack of access to hearing services. Because hearing screening is important for early identification of hearing loss, development of an accessible, self-screening test that can detect hearing loss reliably and quickly would provide significant benefits, especially for underserved populations. This study aimed to develop and validate a new version of the digits-in-noise (DIN) test for Persian speaking countries. DESIGN: Recordings of Persian

Efficacy of serologic testing in asymmetric sensorineural hearing loss.

PURPOSE: The goal of this study was to determine the efficacy of a detailed questionnaire, auditory brain stem response testing (ABR), MRI, and an extensive battery of serologic tests in diagnosing asymmetric sensorineural hearing loss (ASNHL). METHODS AND MATERIAL: Patients with audiograms demonstrating ASNHL of 10 dB or greater in 2 consecutive frequencies or 15 dB in any 1 frequency between 250 and 6000 Hz were asked to participate. Patients underwent MRI scanning of the cerebellopontine angle, internal auditory canals, and posterior fossa with gadolinium contrast, ABR, and an extensive ba

[Stapedial reflex test for gerontal sensorineural hearing loss].

The purpose of this investigation was to find a method that can diagnose sensorineural hearing loss early. Fifty subjects, aged more than 60 years, with hearing level ranged from normal to profound deafness, were examined. The noise-tone difference (NTD) were calculated at 0.5, 1, 2, and 4kHz from these data. The results indicated that NTD at 1kHz was more sensitive than others for diagnosing gerontal sensorineural hearing loss in early stage.

The use of diagnostic testing in asymmetric sensorineural hearing loss.

The etiology of an asymmetric sensorineural hearing loss can often be difficult to determine. Because a wide variety of pathologic processes may be responsible for the hearing loss, numerous diagnostic tests are usually used in the initial evaluation, including pure-tone audiometry, acoustic reflex testing, imaging, serologic testing, and auditory brainstem response testing. The diagnostic evaluations of 225 consecutive cases of asymmetric sensorineural hearing loss are reviewed. A cochlear site-of-lesion was demonstrated in the majority (194) of patients. Because all retrocochlear lesions (31

Tympanometry as a screening test for treatable hearing loss in the elderly.

One hundred and fourteen adults with hearing loss, but without otorrhoea, were studied with the aim of establishing whether tympanometry could be used as a screening test to identify potentially treatable aural pathology. Tympanometry was compared with independent otoscopy. No middle ear pathology likely to be of significance in an elderly person was found in ears with a normal tympanogram. All ears with significant pathology gave an abnormal tympanogram, and in addition there was a false positive rate of 6.8%. We suggest that elderly patients with dry ears and a normal tympanogram could be fi

Smartphone-based hearing test as an aid in the initial evaluation of unilateral sudden sensorineural hearing loss.

Sudden sensorineural hearing loss (SSNHL) can cause significant morbidity. Treatment with steroids can improve outcome. Delay in initiation of treatment reduces the chance to regain hearing. For this reason SSNHL is considered an emergency. Diagnosis is based on history, physical examination and a standard audiogram, the latter requiring specialized equipment and personnel. Standard audiogram may not be available at the time and place of patient presentation. A smartphone or tablet computer-based hearing test may aid in the decision to prescribe steroids in this setting. In this study the uHea

Genetic testing for pediatric hearing loss: no time to waste.

Since the discovery of the first human deafness gene a quarter of a century ago, our approach to clinical evaluation of children with hearing loss has changed dramatically. What was once a low-yield scattershot approach has changed to a clearly definable pathway involving genetic testing, imaging, and congenital cytomegalovirus testing. There still however is a great deal of work to be done to expand the correct use of this testing, particularly genetic testing.

[Caloric test in low-tone sensorineural hearing loss].

Acute low-tone sensorineural hearing loss (ALHL) is generally has a relatively good prognosis. We have often found in long-term following-up, however, that ALHL relapses, recurs or develops into Meniere's disease. Diagnostic criteria of the Acute Altitude Deafness Research Group of the Ministry of Health, Labor, and Welfare of Japan, define ALHL as low-tone-disorder sensorineural hearing loss without vertigo in which cochlear symptoms -ear fullness, tinnitus, and deafness- develop suddenly. Over the last five years, we have treated 31 cases of ALHL, in about half of which neurotological examin

Use of the pinna reflex as a test of hearing in mutant mice.

Although it is a gross measure, the pinna reflex test is easily administered and is, therefore, incorporated as a general screening tool in mutagenesis programs. Our recent application of this approach indicated that mutant mice lacking one of the small Maf proteins, in this case MafG, failed to exhibit a pinna reflex. In contrast, littermate controls, with the same mixed 129/CD1 background, and including both wild type and heterozygous mutant animals, passed the test. Because previous studies indicate that mafG is expressed in both cochlear and vestibular parts of the mouse inner ear, the sou

[Results of screening for hearing disorders using the Ewing test].

Late developments support that deaf children have an urgent need for rehabilitation and hence for early search for hearing defects. One of the modalities to achieve this goal is mass screening of populations. In Belgium the long established well-baby-clinics provide a commodity for these programs. As most infants attend regularly these clinics, it was evident to perform here the testing procedure. The Ewing-test was chosen as the tool for the screening. We started about 1980. In this discussion the statistics from 1984 to 1986 from one province (West-Vlaanderen) are used. It appeared that appr

[Significance of the glycerol test for the diagnosis of sensorineural hearing disorders].

140 patients with various forms of sensorineural hearing loss were examined via the glycerol test (Klockhoff). In 66% of the cases with Menière's disease, a typical temporary threshold shift could be seen. 17 patients with fluctuating hearing loss of unknown etiology without vestibular symptoms showed a similar effect; they were classified as belonging to the group of the cochlear type of Menière's disease. In 34% of cases with clinically assured diagnosis of Ménière's disease, glycerol did not induce improvement in hearing. Repeated glycerol tests revealed varying results; shortly after a Men
