Mid-term result of one and one half ventricular repair in a patient with pulmonary atresia and intact ventricular septum.

A 10-year-old girl was referred to us with a diagnosis of pulmonary atresia and intact ventricular septum with a severely hypoplastic right ventricle and atrial septal defect. Her tricuspid valve diameter (TVD) was 48.5% of normal, with a TVD Z-score of -6.7, while right ventricular end-diastolic volume was 53.4% of normal. As a definitive procedure, one and one half ventricular repair using a bidirectional cavopulmonary shunt (BCPS) was performed instead of a Fontan procedure. The postoperative course was uneventful without episodes of low cardiac output or elevated right atrial pressure. She

An Alternative Technique of Performing the Bidirectional Superior Cavopulmonary Connection without Cardiopulmonary Bypass.

There is an increasing trend of performing the bidirectional superior cavopulmonary anastomosis or bidirectional Glenn (BDG) without cardiopulmonary bypass (CPB). However, there are concerns of neurological dysfunction due to cerebral congestion and systemic oxygen desaturation due to clamping the superior vena cava and pulmonary artery, respectively, required for the off-pump anastomosis. In this report we present our alternative technique of performing the procedure without CPB in 13 patients. This technique may be a useful alternative in a select anatomic group of patients.

Hemodynamic mechanisms of neurogenic pulmonary edema.

Acute lung injury has been reported in man and animals with intracranial disorders, head trauma or cerebral compression. Early studies in our laboratory demonstrated that pulmonary hemorrhagic edema (PHE) of acute and fulminating type occurred accompanying severe hypertension and bradycardia (Cushing responses) in animals following cerebral compression (CC) or intracranial hypertension (ICH). The lung pathology was prevented by spinal transection and sympathoadrenergic blocking agents, but was not affected by decerebration, adrenalectomy, vagotomy and atropine. The effects of central sympathet

Revival of right thoracotomy to approach atrio-ventricular valves in reoperations.

A right thoracotomy was used to approach the atrio-ventricular (AV) valves in 8 patients who had previously undergone a cardiac operation through a midline sternotomy. Due to an extremely enlarged right heart, to a dilated ascending aorta, or to the presence of anteriorly placed aorto-coronary bypass-grafts, a repeated midline sternotomy was considered to involve the risk of massive hemorrhage in this group of patients. Cardiopulmonary bypass was instituted after cannulation of the right femoral artery and of the right atrium or venae cavae. The ascending aorta was controlled when deemed neces

The impact of newly diagnosed patent foramen ovale in patients undergoing off-pump coronary artery bypass grafting: case series of eleven patients.

UNLABELLED: The increased use of transesophageal echocardiography (TEE) by anesthesiologists may lead to an increase in the intraoperative detection of previously undiagnosed patent foramen ovale (PFO). The impact of heart manipulation on interatrial shunting through a PFO during off-pump coronary artery bypass graft (CABG) has not been studied. We retrospectively studied 11 patients with PFOs who underwent off-pump CABG. TEE contrast studies and blood gas analyses were performed at baseline, during heart elevation for distal coronary arteries anastomoses, and at the end of the surgery. At bas

Right ventricular performance during left ventricular unloading conditions: the contribution of the right ventricular free wall.

AIM: Right ventricular (RV) dysfunction is a significant complication following implantation of left ventricular assist device (LVAD). However, RV performance after LVAD implantation remains unclear. We have studied the effects of preload and afterload on RV performance under left ventricular (LV) unloading. METHODS: Six adult mongrel dogs were subjected to cardiopulmonary bypass. RV preload and afterload were independently regulated. Dynamic pressure-length analysis of RV free walls was performed using micromanometer catheter and sonomicrometric dimension transducers. Global RV systolic func

Effects of halothane and morphine sulfate on myocardial compliance following total cardiopulmonary bypass.

We evaluated the effect on diastolic myocardial compliance of halothane and morphine sulfate using 15 swine placed on total cardiopulmonary and right heart bypass with controlled aortic pressure, heart rate, and left ventricular preload. The animals were divided into three equal groups: (I) regional block anesthesia, (II) morphine sulfate (10 mg/kg), and (III) halothane anesthesia at 0.5%. Myocardial performance was evaluated on right heart bypass following a 30 minute period of total cardiopulmonary bypass before and after administration of the anesthetic agent by measuring stroke volume, lef

Long-term cardiopulmonary bypass by peripheral cannulation in a model of total heart failure. The decompression of the left heart through a percutaneous helical spring positioned within the lumen of the tricuspid and pulmonary artery valves.

We performed long-term closed-chest cardiopulmonary bypass in an animal model of total heart failure (induced ventricular fibrillation). The extracorporeal system included a venous reservoir, a roller pump, a membrane lung, and a blood pulsator system. We cannulated the right external jugular vein for venous drainage and the right subclavian artery for arterial return. To decompress the left heart we passed by percutaneous cannulation a special helical spring mounted on a Swan-Ganz catheter (Baxter Edwards Divisions, Irvine, Calif.) and positioned it to rest within the pulmonary artery and tri

Type A aortic dissection following heart transplantation: A case report.

BACKGROUND: Cardiac transplantation is considered the standard treatment for refractory end-stage heart failure. Worldwide, 5074 heart transplantations were performed in 2015. About 100 heart transplants are performed at the authors' center each year. The usual complications of heart transplantation include graft rejection, infection, and graft dysfunction. Aortic dissection after heart transplantation is very rare and is a serious complication that requires a hybrid procedure. CASE SUMMARY: A 58-year-old female patient was admitted to Union Hospital Affiliated to Tongji Medical College of Hu

Right ventricular infarction with tricuspid insufficiency and chronic right heart failure.

A patient with chronic right heart failure and probable tricuspid insufficiency associated with an inferior myocardial infartion is described. Angiograms demonstrated total occlusion of the right coronary artery at its origin, a patent venous bypass graft to the mid-right coronary artery and hypokinesia of the inferior wall of both the right and left vebtricles. Clinical data indicated a greater impairment of right than of left ventricular function. It is proposed that infarction of the right ventricle resulted in chronic right heart failure and tricuspid insufficiency.

Coronary bypass grafting without use of cardiopulmonary bypass for dextrocardia.

Dextrocardia is a condition in which the heart is pointed toward the right side of the chest instead of normally pointing to the left. The rate of atherosclerotic heart disease in subjects with this condition is similar to that of the general population. We present a patient with situs inversus totalis with dextrocardia who underwent coronary artery bypass grafting without use of cardiopulmonary bypass (OPCAB). A 74-year-old man who was known to have dextrocardia with situs inversus was admitted to the hospital because of angina. Coronary angiography was performed and showed ostial occlusion o

[Myocardial protection by retrograde cardioplegic perfusion through the right atrium].

Three types of cardioplegic delivery with ink into the isolated canine hearts were compared: (1) antegrade aortic root perfusion (AARP), (2) retrograde coronary sinus perfusion (RCSP) and (3) retrograde right atrium perfusion (RRAP). Ink was not distributed in the area distal to the coronary occlusion by AARP but well distribution in the same area by RCSP or RRAP. The right ventricular wall and ventricular septum were poorly perfused by RCSP but well perfused by RRAP. During cardiopulmonary bypass, RRAP created a fairly rapid cardiac arrest and satisfactory myocardial cooling. During Perfusion

Diaphragmatic hernia post coronary artery bypass with gastroepiploic artery.

An 86-year-old man, with a medical history of coronary artery bypass grafting with the right gastroepiploic artery 20 years prior, was admitted to our hospital for right-sided heart failure. Computed tomography findings revealed an intrapericardial diaphragmatic hernia of the transverse colon compressing the right ventricle. The hernia was successfully repaired, and the patient recovered without any complications. Diaphragmatic hernia is rare but may be a lethal complication following coronary artery bypass grafting. We should be aware of the possibility of this condition.

Cava endoluminal balloon occlusion as an adjunct technique to perform less invasive cardiopulmonary bypass procedures.

OBJECTIVES: A cardiopulmonary bypass technique was developed with the aim of achieving a less invasive approach to right-sided lesions of the heart or to avoid extensive dissection in reoperations. METHODS: Cavae endoluminal balloon occlusion (CEBO) was achieved with balloon catheters, introduced through cannulas inserted in the right jugular and one of the femoral veins. This procedure can be used in association with a standard femoral cardiopulmonary bypass or with the port-access technique. RESULTS: Twenty-two patients were operated, through a right thoracotomy, for atrial septal defect (

Evolution, coronary artery bypass surgery and outcomes of right ventricular dysfunction in ischaemic cardiomyopathy.

OBJECTIVES: Whether the presence or evolution of right ventricular dysfunction (RVD) affects the prognosis and the therapeutic choice between coronary artery bypass grafting (CABG) or medical therapy alone in patients with ischaemic cardiomyopathy (ICM) remains unclear. We investigate the prognostic and therapeutic implications of RVD in patients with ICM. METHODS: Patients with baseline echocardiographic right ventricular (RV) assessment were included from the Surgical Treatment of Ischaemic Heart Failure trial. The primary outcome was all-cause mortality. RESULTS: Of 1212 patients enrolled

Mechanical Ventilation After Bidirectional Superior Cavopulmonary Anastomosis for Single-Ventricle Physiology: A Comparison of Pressure Support Ventilation and Neurally Adjusted Ventilatory Assist.

We evaluated the effects of different respiratory assist modes on cerebral blood flow (CBF) and arterial oxygenation in single-ventricle patients after bidirectional superior cavopulmonary anastomosis (BCPA). We hypothesized that preserved auto-regulation of respiration during neurally adjusted ventilatory assist (NAVA) may have potential advantages for CBF and pulmonary blood flow regulation after the BCPA procedure. We enrolled 23 patients scheduled for BCPA, who underwent pressure-controlled ventilation (PCV), pressure support ventilation (PSV), and NAVA at two assist levels for all modes i

[Staged total cavopulmonary connection following Starnes operation].

A three-year-old boy with Ebstein's anomaly, who had received Starnes operation in the neonate state, successfully underwent staged total cavopulmonary connection. Despite of hypoplasticity of the pulmonary artery, the postoperative cardiac catheterization showed no elevation of pulmonary artery pressure and good cardiac performance. Although he suffered from supraventricular tachycardia in the early postoperative period, subsequent course was uneventful.
