Decreased Yes-Associated Protein-1 (YAP1) Expression in Pediatric Hearts with Ventricular Septal Defects.

BACKGROUND: Ventricular septal defects (VSDs) are the most common and simplest type of congenital heart diseases (CHDs). Animal studies have suggested that the downregulation of Yes-associated protein 1 (YAP1) during embryonic development causes VSD-associated CHDs. However, how YAP1 contributes to isolated VSD (iVSD) is unclear. METHODS AND RESULTS: Twenty right atrial specimens were obtained from iVSD patients during routine congenital cardiac surgery and we assessed YAP1 expression in these specimens. For controls, six right atrial specimens were obtained from normal hearts of children wit

Gunshot wounds: causing myocardial infarction, delayed ventricular septal defect, and congestive heart failure.

Penetrating chest trauma can cause a wide variety of cardiac injuries, including myocardial contusion, damage to the interventricular septum, laceration of the coronary arteries, and free-wall rupture. Herein, we describe the case of a 21-year-old man who presented with congestive heart failure, which was secondary to an old myocardial infarction and complicated by the delayed formation of a ventricular septal defect. All of these conditions were attributable to multiple gunshot wounds that the patient had sustained 6 months earlier. Left ventricular angiography showed an apical aneurysm; a la

B-type natriuretic peptide and heart failure in patients with ventricular septal defect: a pilot study.

In adults without congenital heart disease, B-type natriuretic peptide (BNP) has been shown to be a very sensitive and specific marker of heart failure. The utility of BNP as a marker of clinical heart failure in children with a ventricular septal defect (VSD) has yet to be determined. A prospective, observational study evaluated BNP levels and other measures of heart failure. Eligible patients were <2 years old, scheduled to undergo surgical repair of a VSD, and without other significant structural heart disease. Data collected before and after surgical repair included echocardiographic measu

[Cardiogenic shock complicating extensive infarction with ventricular septal defect. Circulatory assistance and heart transplantation].

A 47 year old man had a massive anterior myocardial infarction with cardiogenic shock with a left parasternal murmur. Coronary angiography showed occlusion of the left anterior descending artery for which angioplasty resulted in failure. There was antero-lateral-apical akinesia and a ventricular septal defect (VSD) with a left-right shunt (Qp/Qs = 1.54). Persistence and aggravation of haemodynamic instability led to intra-aortic balloon pumping with inotropic pharmacological support followed by biventricular assistance with a MEDOS device. Under transoesophageal echocardiographic monitoring, t

Characterization of SMAD3 Gene Variants for Possible Roles in Ventricular Septal Defects and Other Congenital Heart Diseases.

BACKGROUND: Nodal/TGF signaling pathway has an important effect at early stages of differentiation of human embryonic stem cells in directing them to develop into different embryonic lineages. SMAD3 is a key intracellular messenger regulating factor in the Nodal/TGF signaling pathway, playing important roles in embryonic and, particularly, cardiovascular system development. The aim of this work was to find evidence on whether SMAD3 variations might be associated with ventricular septal defects (VSD) or other congenital heart diseases (CHD). METHODS: We sequenced the SMAD3 gene for 372 Chinese

Congenital diaphragmatic hernia with congenital heart disease including ventricular septal defect: Risk factors.

BACKGROUND: Congenital diaphragmatic hernia (CDH) with congenital heart disease (CHD) has a poor prognosis. This study aimed to establish the prognostic factors of CDH associated with ventricular septal defect (VSD), which is the most commonly encountered CHD. METHODS: We conducted a single-center, retrospective observational study including neonatal cases of CDH with CHD managed at our hospital between 2000 and 2022. The risk factors for CDH in patients with VSD were established by comparing survivors and non-survivors. RESULTS: Among the 84 infants with CDH, 14 (17%) were associated with C

Infective endocarditis-induced complete closure of a ventricular septal defect and complete heart block in a child.

We hereby report rare occurrence of irreversible complete heart block in a child with tricuspid valve infective endocarditis. The tricuspid valve vegetation also caused complete closure of perimembranous ventricular septal defect, which was later discovered during surgery.

Hydatid cyst of the interventricular septum causing complete heart block and postoperative ventricular septal defect.

Cardiac involvement is seen in 0.5-2% of patients with hydatid disease, and involvement of the interventricular septum (IVS) is even rarer. We report surgical treatment of a large cardiac hydatid cyst in the IVS. A 55-year-old woman presented with dyspnea and bradycardia. Transthoracic echocardiography revealed a large cyst in the mid to apical part of the IVS. The patient was placed on cardiopulmonary bypass. Through an incision to bulging part of cyst into right atrium, and without opening adjacent cardiac chambers, we aspirated the entire contents of the cyst, removed its germinative membra

Does the technique of interventional closure of perimembranous ventricular septal defect reduce the incidence of heart block?

OBJECTIVE: To describe the difficulties and differing techniques in the transcatheter placement of amplatz ventricular septal defect devices to close perimembranous ventricular septal defects and place these in the context of the expanding literature on ventricular septal defect catheter closure. BACKGROUND: Surgery remains the established first-line therapy for closure of haemodynamically significant perimembranous ventricular septal defects. Transcatheter techniques appeared to promise a possible alternative, obviating the need for cardiac surgery. However, significant technical and anatomi

Late complete heart block in an adult patient undergoing percutaneous ventricular septal defect closure.

With advances in transcatheter treatment options, percutaneous device closure of ventricular septal defects has become a safe and practical alternative to surgical repair. While outcomes have been excellent, late complete heart bock has been documented during follow up of pediatric patients. We report a case of late complete heart block complicating percutaneous device closure of a ventricular septal defect in a 37-year-old female requiring permanent pacemaker insertion. The patient underwent transcatheter closure of an atrial and ventricular septal defect in the context of treated pulmonary h

The protective effect of a leucocyte removal filter on the lung in open-heart surgery for ventricular septal defect.

We elucidated the protective effect of a leucocyte removal filter on cardiopulmonary bypass (CPB)-induced lung dysfunction during open-heart surgery for ventricular septal defect (VSD). Forty-six VSD patients were divided into two groups: (a) a control group of 22 patients in whom the banked blood was used to prime the CPB circuit, and (b) a leucocyte removal group of 24 patients in whom a leucocyte removal filter was used for priming and every supplement of banked blood during and after the operation. The respiratory index immediately after the CPB was significantly lower in the leucocyte rem

An easy and safe technique for ventricular septal defect closure in hearts with atrioventricular discordant connection and dextrocardia.

When ventricular septal defect exists in hearts with atrioventricular discordant connection and dextrocardia, closure of the defect can most easily be accomplished by exposure through the left atrium and retracted tricuspid valve leaflets. This approach provides good visualization of all aspects of the defect and keeps the suture line away from the conduction tissue, thus reducing the possibility of iatrogenic heart block.

Heart rate variability is markedly abnormal following surgical repair of atrial and ventricular septal defects in pediatric patients.

BACKGROUND: Abnormalities in heart rate variability (HRV) have been associated with increased morbidity and mortality. Adults with a history of successful correction of atrial septal defects (ASDs) and ventricular septal defects (VSDs) have clinically relevant abnormalities in HRV. The etiology of these abnormalities is unknown as is when they occur in the natural history of these lesions. We sought to determine whether the abnormalities described in adult patients are identifiable in pediatric patients. MATERIALS AND METHODS: Eighty-five ASD patients, 105 VSD patients, and 142 non-cardiac su

[Is minimal skin incision and partial sternotomy approach for congenital heart defects less invasive?; evaluation of SIRS on ventricular septal defect].

OBJECTIVES: Minimally invasive cardiac surgery (MICS) has been developed to offer patients the benefits of open heart operations with limited skin incision. But it is unclear whether this procedure is less invasive. We evaluate postoperative duration of systemic inflammatory response syndrome (SIRS) on ventricular septal defect (VSD). METHODS: From August 1997 to March 2000, 47 patients VSD underwent total repair by the minimal skin incision and lower partial median sternotomy (MICS group). We compared duration of SIRS between MICS and conventional method group (n = 14) and between early MICS

Acquired von Willebrand syndrome in young children with congenital heart defects: focus on patent ductus arteriosus and ventricular septal defect.

BACKGROUND: Acquired von Willebrand syndrome (AVWS) is a rare bleeding disorder due to a deficiency of von Willebrand factor (VWF). High shear stress causes stretching and rupture of VWF multimers, leading to functional loss and increased proteolysis. This occurs in cardiovascular diseases, reducing high-molecular-weight multimers (HMWMs). Patent ductus arteriosus (PDA) and ventricular septal defect (VSD) cause blood shunting between systemic and pulmonary circulation, increasing shear stress, and may contribute to AVWS. OBJECTIVES: To investigate whether children with PDA and VSD experience
