INTRODUCTION: Acute painful scrotum in children may be associated with torsion of the testis, hematocele, epididymitis and direct testicular injury with hematoma formation. More frequently, however, acute scrotum occurs without a precipitating factor. Testicular trauma is not uncommon. Apart from accidental trauma it may deliberately be inflicted during fight by striking blows in the scrotal area. The most common posttraumatic complications are hematoma, hydrocele and hematocele, orchiepididymitis and occasionally malignant degeneration of the injured testis. Haematocele is an unusual scrotal disorder in children, and is generally associated with direct trauma to the inguinoscrotal region. A 6-year-old boy with acute communicating haematocele who had a history of blunt abdominal trauma one week earlier is presented. Interestingly, no solid organ injury was detected on his previous admission for abdominal trauma, nor there was a history of hydrocele.

According to our examination of 266 patients, ultrasonography of scrotal contents, specifically of the testes, proves to be a highly reliable, non-invasive diagnostic method. The major advantage of this technique is the fact that it enables adequate evaluation of the testes and exclusion of tumours, even when examination by palpation seems difficult or impossible due to extratesticular alterations (e.g. hydrocele, epididymitis). By means of this method also small, non-palpable lesions of the testes may be clearly demonstrated.

A review of 100 boys less than 15 years old who were hospitalized for an acute scrotum revealed that the most common causes of the disorder were testicular torsion, including torsion of the spermatic cord (42 per cent) and torsion of the appendages (32 per cent). Fifty-three patients with testicular injury were investigated.
