Acute spinal subdural hematoma: MR and CT findings with pathologic correlates.

PURPOSE: To determine the MR and CT findings that characterize acute spinal subdural hematoma (ASSH). METHODS: The MR, CT, and clinical findings in three patients with surgically proved ASSH were reviewed and also correlated with the postmortem MR, CT, and cryomicrotome findings in three other patients, two with ASSH and one with an acute spinal epidural hematoma. RESULTS: Imaging findings in ASSH included: (a) hyperdense lesions on plain CT within the dural sac, distinct from the adjacent low-density epidural fat and silhouetted against the lower-density spinal cord and cauda equina, which

Spontaneous spinal subarachnoid hemorrhage and subdural hematoma. Report of two cases.

Two patients with spontaneous spinal subarachnoid hemorrhage are presented to emphasize the clinical and radiological features of this uncommon illness. Both had severe back pain at the onset. One patient had a subdural hematoma that compressed the conus medullaris and cauda equina, and was drained percutaneously; the other had clots in the subarachnoid space. The cerebrospinal fluid showed a polymorphonuclear pleocytosis that simulated septic meningitis. Complete spinal angiography failed to reveal a cause for the hemorrhages.

Ruptured spinal dural arteriovenous fistula with subdural hematoma: A case report.

BACKGROUND: Spinal dural arteriovenous fistulas (SDAVFs) are the most common spinal vascular malformations and can lead to significant neurological deficits if left untreated. Although typically associated with chronic myelopathy, rupture of SDAVFs is a rare yet severe complication that can result in acute spinal cord injury secondary to intradural hematoma expansion. CASE DESCRIPTION: A 53-year-old male presented with fluctuating neck and back pain, unsteady gait, right leg numbness, and urinary retention, eventually attributed to a ruptured thoracic SDAVF resulting in a T3-T4 subdural hemat

Recurrent spinal subdural hematoma following percutaneous kyphoplasty: A unique case report.

Spinal intradural (subdural and subarachnoid) hematoma following percutaneous kyphoplasty is an extremely rare complication. In this report, we described a case of 2 episodes of subarachnoid hemorrhage with delayed paralysis after kyphoplasty. A 73-year-old man underwent percutaneous kyphoplasty in our hospital an osteoporotic vertebral fracture at the T12 level. On the 55 h after kyphoplasty for T12 osteoporotic vertebral fracture, he developed paralysis of the lower limbs. An emergency posterior decompression from T8 to L2 was performed. And the subarachnoid hematomas were removed. Postopera

Subarachnoid Hemorrhage and Spinal Subdural Hematoma Due to Acute CSF Hypotension.

BACKGROUND: Intracranial subarachnoid hemorrhage (SAH) and spinal subdural hematoma (SDH) are rare complications of spine surgery, thought to be precipitated by cerebrospinal fluid (CSF) hypotension in the setting of an intraoperative durotomy or postoperative CSF leak. Considerable clinical variability has been reported, requiring a high level of clinical suspicion in patients with a new, unexplained neurologic deficit after spine surgery. METHODS: Case report. RESULTS: An 84-year-old man developed symptomatic spinal stenosis with bilateral lower extremity pseudoclaudication. He underwent L

Intracranial subdural hematoma: a rare complication following spinal anesthesia: case report.

BACKGROUND AND OBJECTIVES: Intracranial subdural hematoma is a rare complication following spinal anesthesia. The diagnosis is usually difficult because initial symptoms are the same of post-dural puncture headache. The objective was to report a case of early diagnosed subdural hematoma after spinal anesthesia performed with a fine-gauge needle and single puncture. CASE REPORT: 48-year old female patient, ASA I, undergoing spinal anesthesia for surgery to correct urinary incontinence. The spinal anesthesia was performed with 27G Quincke needle and single puncture. There was no unexpected even

Intracranial subdural hematoma after resection of a thoracic spinal cord tumor.

Intracranial subdural hematoma associated with intracranial hypotension is a rare complication of dural puncture and other procedures on the dura. This report describes the case of a 25-year-old man who developed intracranial subdural hematoma after resection of a thoracic spinal cord tumor. Intracranial subdural hematoma could result in severe and permanent neurologic deficit if it remains unrecognized. Severe and prolonged postdural puncture headache should be regarded as a warning sign of an intracranial subdural hematoma, and in those patients early investigation is recommended.

Spinal subdural hematoma with cauda equina syndrome: A complication of combined spinal epidural anesthesia.

Combined spinal-epidural anesthesia (CSE) is considered safe in lower limb surgeries. We report a case of sudden neurological deterioration in a stable postoperative patient who was given CSE for total knee replacement and low molecular weight heparin in postoperative period. On the 4(th) postoperative day, she developed sudden onset weakness in left lower limb along with bladder incontinence. Magnetic resonance imaging spine revealed a subdural hematoma at L2-L3 level. Immediate laminectomy along with cord decompression was done and patient recovered well except for a persistent foot drop on

Rivaroxaban-Induced Nontraumatic Spinal Subdural Hematoma: An Uncommon Yet Life-Threatening Complication.

In the last decade, the desire for safer oral anticoagulants (OACs) led to the emergence of newer drugs. Available clinical trials demonstrated a lower risk of OACs-associated life-threatening bleeding events, including intracranial hemorrhage, compared to warfarin. Nontraumatic spinal hematoma is an uncommon yet life-threatening neurosurgical emergency that can be associated with the use of these agents. Rivaroxaban, one of the newly approved OACs, is a direct factor Xa inhibitor. To the best of our knowledge, to date, only two published cases report the incidence of rivaroxaban-induced nontr

Remote Spinal Subdural Hematoma Following Lumbar Biportal Endoscopic Surgery : Two Case Reports.

Remote spinal subdural hematoma (SSH) following unilateral biportal endoscopic (UBE) spine surgery is rare, even without intraoperative dural injury. We report two such cases. A 76-year-old woman underwent anterior lumbar interbody fusion and UBE decompression for lumbar spinal stenosis. Intraoperatively, dense adhesions were noted, but no cerebrospinal fluid (CSF) leakage occurred. Postoperatively, she developed left leg monoplegia. Magnetic resonance imaging (MRI) revealed a subdural hematoma at L2-3, remote from the surgical site. She recovered completely with steroid therapy and conservati

Acute spinal subdural hematoma: A case report of spontaneous recovery from paraplegia.

RATIONALE: Spontaneous spinal subdural hematoma (SSDH) is a rare disease that can cause severe permanent neurological dysfunction. Here we present a case of spontaneous SSDH, in which a series of magnetic resonance images (MRIs) taken through the course of the disease facilitated understanding of the resolution process of the hematoma and the diagnosis of SSDH. PATIENT CONCERNS: A 59-year-old male presented with sudden severe back pain and rapid onset of paraplegia. This symptom had continued developing while he was transferred to the emergency department. Initial physical examination showed

Intracranial Acute Subdural Hematoma Following Spinal Anesthesia: Our Experience with Six Patients.

OBJECTIVE: To describe our experience with the occurrence of intracranial acute subdural hematoma (ASDH) following spinal anesthesia. PATIENTS AND METHODS: We reviewed our records from 2010 to 2017 to detect cases of nontraumatic ASDH following spinal anesthesia. All cases were analyzed for the etiological factors, time lag between the procedure and hematoma diagnosis, treatment, and outcome. RESULTS: Of 329 cases of nontraumatic ASDH, we identified 6 patients whose spontaneous ASDH developed following spinal anesthesia. All our patients were obstetrical and received spinal anesthesia for de

Spinal subdural hematoma following lumbar decompressive surgery: a report of two cases.

Spinal subdural hematoma (SSDH) following spine surgery is an extremely rare condition, with only three cases being reported in the literature. Unintended durotomy has been associated with SSDH due to alterations of pressures in the dural compartments. The objective of the present report was to report two rare cases of acute SSDH developed after lumbar decompressive surgery. In one of the patients, the diagnosis of SSDH was followed by urgent hematoma evacuation via durotomy due to the patient's worsening neurological symptoms. In the second patient, the SSDH was treated conservatively due to

Traumatic or Nontraumatic: A Case of Spinal Subdural Hematoma Caused by Multiple Factors.

Spinal subdural hematoma (SSDH) is a rare but potentially life-threatening complication following spinal decompression surgery. Vertebral compression fractures in elderly patients are commonly associated with osteoporosis. This case illustrates how the interplay of bleeding diatheses, coagulopathy, and medication interactions may collectively contribute to severe clinical outcomes.

A Case of Acute Massive Spinal Subdural Hematoma With a Spot Sign on CT.

Acute massive spinal subdural hematoma (ASSDH) is an extremely rare condition. We report a case of ASSDH in a 74-year-old woman who suddenly developed paraplegia after receiving oral anticoagulant therapy for severe back pain. Reported cases of ASSDH show various clinical presentations, ranging from low back pain to varying degrees of paralysis. In this case, the patient developed acute complete paraplegia. Despite undergoing emergency decompression surgery as soon as possible, the symptoms did not improve. Preoperative angiography revealed a "spot sign" originating from the radiculomedullary

A calvarial acute subdural hematoma migrating into the spinal canal in a young male.

It is not common for an acute subdural hematoma (SDH) in the supratentorial region to show rapid resolution or migration during the clinical course. In this report, we present a rare case where the SDH in the supratentorial region was observed to rapidly migrate into the lumbar spinal canal, leading to severe radiculopathy. A 20-year-old male patient was admitted to the emergency department with severe headache after head trauma. The patient's overall condition was good, whereas his Glasgow Coma Scale score was 15 and blood pressure was normal. He had vomited 3 times after the onset of pain. N

[Intracranial and spinal subdural hematoma: a rare complication of epidural and subarachnoid anesthesia].

Subdural and epidural hematomas complicating central blocks are rare but serious complications that can lead to permanent neurological deficits. This review discusses cranial and spinal subdural hematomas, including the history of this complication in the literature. Theories to explain the mechanisms by which hematomas are formed are presented and risk factors are analyzed. The associations between such hematomas and the design of the various needles used for lumbar puncture are evaluated and the most common cranial and spinal locations are discussed. The anatomy of tissues that envelop and c
