Massive hemoptysis in cystic fibrosis.

Massive hemoptysis occurs in 5% to 7% of patients with cystic fibrosis. Approximately 11% of these patients will die within 48 hours of manifestation because of uncontrolled hemoptysis and asphyxiation. If conservative medical treatment fails to control hemoptysis, fiberoptic or rigid bronchoscopy is the least risky and most accurate method of localizing the source of hemoptysis. Acute control of hemoptysis can be obtained in up to 100% of patients with endobronchial Fogarty balloon tamponade and in up to 88% of patients with bronchial artery embolization. Emergency thoracotomy can be performe

An Infrequently Encountered Cause of Hemoptysis.

Mitral stenosis (MS) is not a common entity in modern-day medicine, especially in developed countries, as the most common etiology is still rheumatic fever. MS can present mainly with a wide range of cardiac symptoms. However, infrequently, MS can cause extra-cardiac symptoms as well. We present a case report of a patient with severe bioprosthetic mitral valve stenosis with intermittent hemoptysis and cardiogenic shock. We aim to report this case to remind clinicians about this uncommon but significant cause of hemoptysis. This case report also emphasizes the importance of utilizing a team app

Evaluation and Treatment of Massive Hemoptysis.

Massive hemoptysis is appropriately defined as life-threatening hemoptysis that causes airway obstruction, respiratory failure, and/or hypotension. Patients with this condition die from asphyxiation, not hemorrhagic shock. Any patient who presents with life-threatening hemoptysis requires immediate treatment to secure the airway and stabilize hemodynamics. Early activation and coordinated response from a multidisciplinary team is critical. Once the airway is secure and appropriate resuscitation is initiated, priorities are to localize the source of the bleeding and gain hemorrhage control. Non

An Unusual Case of Chronic Hemoptysis.

A 62-year-old African American man was admitted to the hospital with hemoptysis. He had a complicated medical history significant for active tobacco use (>50 pack-year history), coronary artery disease, and heart failure with reduced ejection fraction. He reported intermittent episodes of coughing up streaks of blood in the sputum for the past 3 years. For the past few days before this presentation, he had multiple episodes of coughing up over a tablespoon of only blood. He was not on any anticoagulant agents. There were no risk factors for TB, nor was there a history of fevers, chills, shortn

Massive hemoptysis in a patient with sarcoidosis.

A 39-year-old African American man presented with massive hemoptysis. He was known to have a history of type III sarcoidosis and a chest x-ray consistent with aspergilloma. Bronchoscopy was performed within 48 hours from the onset of the patient's symptoms revealing generalized heme in both the left and right main bronchi. Massive hemoptysis is a potentially life-threatening complication from a variety of diseases. This case serves to illustrate the diagnostic considerations and potential therapeutic interventions in patients with advanced sarcoidosis.

Hemoptysis in a patient with tracheopathia osteochondroplasia.

Tracheopathia osteochondroplasia is a rare slowly progressive benign disease of unknown etiology characterized by submucosal nodules protruding in the airway. Most patients are asymptomatic but with progression of disease, symptoms of cough, hemoptysis, dyspnea, wheezing, hoarseness, and rarely airway compromise have been reported. Management is symptomatic with control of cough, prophylaxis with antibiotics, and rarely surgery.

Etiology and management of pediatric hemoptysis.

OBJECTIVE: To review the diagnostic and treatment strategies of hemoptysis in children. DESIGN: Retrospective analysis of patients evaluated between January 1, 1995, and August 31, 1999. SETTING: Tertiary pediatric referral center. PATIENTS: Nineteen consecutive children presenting with hemoptysis to the otolaryngology service. RESULTS: Chest radiography and bronchoscopy established the correct etiology in 15 patients. Infection and tracheostomy-related complications were the most common underlying problems. Other causes included congenital heart disease, pulmonary hemosiderosis, inflammat

Just another hemoptysis or a fluke?

Hemopytsis is commonly encountered in the daily practice of the pulmonary physician. Younger patients with normal chest x-rays frequently have acute or chronic bronchial disease accounting for their complaint. Occasionally parasitic disease is described as an unusual cause for a patient presenting with hemoptysis. Although pulmonary paragonimiasis is unusual in this country, because of the rapid growth in travel as well as immigration, physicians will need to be aware of this disorder.

Role of bronchoscopy in massive hemoptysis.

Airway hemorrhage is a potentially rapidly fatal condition. Death may occur within minutes from asphyxiation before control can be achieved. The primary prognostic factors are the rate of bleeding and the underlying cardiopulmonary status of the patient. Bronchoscopy is central in management, but the goals differ, depending on circumstances. In stable patients who have minimal hemoptysis, bronchoscopy can diagnose the cause specifically and be used as the primary treatment modality. In the setting of massive or life-threatening bleeding, bronchoscopy primarily is performed to maintain ventilat

[Interventional radiology in the treatment of hemoptysis].

Massive haemoptysis, usually related to lesions of bronchial arteries, is a life-threatening complication of chest lesions. Embolization of bronchial arteries demonstrates immediate favourable results in at least 90% of cases. The main complications of embolization of bronchial arteries are medullary strokes. Consequently it is mandatory to visualize the anterior spinal artery in order to avoid its embolization. Massive haemoptysis of pulmonary arterial origin is far less common. It is also feasible to perform the embolization of the abnormal pulmonary artery in order to control the haemoptysi

Imaging evaluation of hemoptysis in children.

Hemoptysis is an uncommon but distressing symptom in children. It poses a diagnostic challenge as it is difficult to elicit a clear history and perform thorough physical examination in a child. The cause of hemoptysis in children can vary with the child's age. It can range from infection, milk protein allergy and congenital heart disease in early childhood, to vasculitis, bronchial tumor and bronchiectasis in older children. Acute lower respiratory tract infections are the most common cause of pediatric hemoptysis. The objective of imaging is to identify the source of bleeding, underlying prim

Hemoptysis in a referral hospital for pulmonology.

OBJECTIVE: To determine the main causes of hemoptysis and to classify this symptom, in terms of the amount of blood expectorated, in patients hospitalized at a referral hospital for pulmonology. METHODS: The study included 50 patients with hemoptysis admitted to the pulmonology ward of a general hospital in the city of Recife, Brazil, between July of 2005 and February of 2006. The data of interest were analyzed and compared with those in the literature. RESULTS: The most common cause of hemoptysis was infection--in 39 patients (78%)--mostly related to tuberculosis sequelae or active tubercul

[An unusual cause of hemoptysis: a leech].

We report an exceptional case of hemoptysis observed in a 38-year-old woman with an uneventful past history. Bronchial fibroscopy demonstrated the presence of a leech attached to the laryngeal orifice. This condition is common in endemic areas but is underdiagnosed. The association of hemoptysis, voice alterations, and difficult respiration are suggestive signs. Extraction should be achieved rapidly to avoid death due to acute respiratory failure.
