A 60-year-old man without comorbidity underwent a totally extraperitoneal repair of bilateral inguinal hernias under general anesthesia. Forty minutes after the procedure he developed a slow, shallow respiratory pattern with a respiratory rate of 5/min and a self-limiting grand mal seizure lasting 30 seconds. Arterial blood gas analysis indicated significant hypercarbia and acidosis. The total dose of morphine administered was 20 mg intravenously. Naloxone was administered and the respiratory rate improved. The patient was discharged after 24 hours after making a good recovery and has had no f

The effects of pneumoperitoneum on peak venous flow velocity in the common femoral vein and the vena cava have already been studied. The results suggested that venous stasis occurs during surgical pneumoperitoneum. This study determines the effects of pneumoperitoneum on the overall venous outflow resistance of the lower limbs. Venous outflow resistance was measured during surgical procedures by impedance plethysmography in 12 patients undergoing laparoscopic cholecystectomy, 4 patients undergoing laparoscopic herniorrhaphy, 4 patients undergoing conventional cholecystectomy, and 2 patients un

In this report, we present two cases of small bowel obstruction after laparoscopic herniorrhaphy (LH). One case involved incarceration of the small intestine into the port site, resulting in obstruction in an 80-year-old man on the third day after LH. The other case involved a 78-year-old man with bilateral inguinal hernia and a recurrent type on one side. In this case, a small bowel obstruction occurred due to intestinal herniation through the repaired peritoneum in the pelvic floor. In both cases, the location of the obstruction was diagnosed by means of a computed tomography scan. Subsequen

To improve the safety and speed of the peritoneal dissection, we developed a saline ballooning method in which a dull needle is inserted lateral to the inferior epigastric vessels under direct laparoscopic visualization to a tissue plane just superficial to the peritoneum. Then, saline solution is injected. As a result, the peritoneum is easily separated from the spermatic cord and inferior epigastric vessels. Furthermore, bleeding is minimized during dissection of the peritoneum.

Tolerance to inguinal hernia repair under local anesthesia was prospectively evaluated in a consecutive series of 41 patients who underwent hernia repair with local infiltration of 0.75 per cent lidocaine. Intraoperative discomfort was measured by an analog visual scale. All complications were also recorded. Mean intraoperative pain was mild (1.6 +/- 1.4). There were two complications. Wound infection and wound hematoma occurred in one patient each. There was no operative mortality. Normal diet was tolerated the day of surgery in 39 patients, and 30 were discharged from the hospital on the sam

This technical note describes a surgical technique for repairing a right dorsal crural diaphragmatic hernia and reports postoperative outcomes. Two client-owned dogs were referred for a diaphragmatic hernia diagnosed via thoracic radiographs, suspected to be chronic and secondary to trauma. During surgery, a hernia was identified in the right dorsal crus of the diaphragm, and herniated contents were reduced. In Dog 1, the lateral aspect of the defect was closed using mattress sutures; however, insufficient local tissue precluded complete primary closure. To address this, the soft tissues later

This case report describes the use of an autologous vaginal tunic as a reinforcement material in the repair of an inguinal hernia in a 10.5-year-old intact male mixed-breed dog, presented to the Veterinary Hospital of the Federal University of Western Bahia with a 10-month history of swelling in the right lateral region of the penis. The patient was diagnosed with prostatic hyperplasia associated with an inguinal hernia. Surgical treatment consisted of inguinal herniorrhaphy; given the muscle weakness observed intraoperatively, the inguinal wall was reinforced using a double-layered autologous

BACKGROUND: As surgical techniques progress, laparoscopic herniorrhaphy is now performed more often in premature babies. The aim of this study was to analyze the outcomes of newborns and infants who underwent single-incision laparoscopic herniorrhaphy (SILH) at our center.
METHODS: We retrospectively reviewed patients younger than 12 months old who received SILH at our department from 2016 to 2020. SILH involved a 5 mm 30-degree scope and 3 mm instruments with a 3-0 Silk purse-string intracorporeal suture for closure of the internal ring. At the time of surgery, Group 1 newborns, whose correc

OBJECTIVE: To evaluate the effectiveness of a herniorrhaphy technique, using an autogenous fascia lata graft (FLG) for perineal hernia (PH) repair in dogs.
STUDY DESIGN: Prospective clinical study.
ANIMALS: Twelve dogs with PH.
METHOD: PHs were repaired with FLG harvested from the dog's ipsilateral thigh and sutured directly into the perineal defect. Correction of associated conditions, and castration were performed. Surgical time, pain, inflammation, pattern of defecation, lameness, hospitalization time, postoperative complications, and owner satisfaction were recorded. Histopathologic exa

Effective postoperative analgesia is important, especially in pediatric surgery. The efficacy of intraoperative surgical wound irrigation with bupivacaine for postoperative analgesia was investigated in 90 children undergoing elective inguinoscrotal surgery. We found that this method effectively reduced postoperative pain and narcotic drug requirement. Bupivacaine irrigation was simple and complication-free. We believe that the irrigation of surgical wounds with bupivacaine should be a routine procedure in elective inguinoscrotal surgery in children.

Thanatophoric dysplasia (TD) is a rare and severe type of skeletal dysplasia. Typical clinical findings include macrocephaly, shortening of the four limbs, underdeveloped lungs, and thoracic hypoplasia. Neonates with TD develop severe respiratory problems due to thoracic hypoplasia and require respiratory management for survival. Despite the resolution of respiratory problems, long-term survival cases are rare. Previous studies have reported that surgical procedures in patients with TD are limited to those necessary for survival, including tracheostomy, laminectomy, and ventricular shunt. A 1-

Incisional herniorrhaphy remains a formidable challenge to the general surgeon. Recurrence rates after primary repair are reported between 31-54 per cent while tension-free repairs with prosthetic mesh have lowered this rate to 10 per cent. Repairs with composite mesh (polypropylene/ePTFE) have been gaining in popularity due to the ease of mesh placement in the intraperitoneal location. This paper reviews our experience with composite repairs at a teaching community hospital. A retrospective chart review was performed which evaluated all patients undergoing abdominal incisional hernia repairs

BACKGROUND: Recently there has been interest in performing laparoscopic herniorrhaphies without the use of staples or tacks to fix the mesh. Although mesh fixation has been linked to an increased incidence of nerve injury and involves increased operative costs, many surgeons feel that fixation is necessary to reduce the risk of hernia recurrence. This study evaluates the outcomes of laparoscopic herniorrhapies performed with and without mesh fixation at our institution.
METHODS: We retrospectively evaluated our last 172 laparoscopic herniorrhaphies, which span a period of conversion from stap

In laparoscopic herniorrhaphy, an alternative to mesh fixation with a stapler is now offered by the helical fastener. The helical fastener was used in 46 patients (39 men and 7 women) with 30 unilateral and 15 bilateral inguinal hernias and 1 femoral hernia. We also used a 5-mm optical system, with which we were able to reduce the size of the trocar incisions. Two 5-mm incisions and one 7-mm incision were used. The fixation elements could easily penetrate bone; therefore, the mesh was easily fixed in the public bone. Laterally, the mesh was fixed in the iliopubic tract. The peritoneum was clos

BACKGROUND: Nonsteroidal antiinflammatory drugs (NSAIDs) have become a popular component of analgesia regimens, particularly in combination with narcotics. We questioned whether there might also be a place for their use in conjunction with regional anesthesia and whether there was a preferable route for NSAID administration.
METHODS: Ilioinguinal and field blocks were performed preoperatively on seventy patients undergoing outpatient inguinal hernia repair. Patients were divided into a control group who received no ketorolac and four study groups who received a preoperative dose of 30 mg keto

OBJECTIVE: The application of opiates after day case surgery in childhood is discussed controversially because of possible complications. Therefore many anaesthetists try to avoid these substances and prefer peripheral analgesics for postoperative pain control.
METHODS: By distributing a questionnaire concerning the arguments connected with postoperative pain to 267 parents (a total of 78.3% of the questionnaires was returned) of children that had undergone inguinal herniorrhaphy or orchidopexy we hoped to gain insight into the judgement of peripheral analgesics-in this case acetaminophen-fol
