Saturated Salt Solution Cadaver-Embalming Method Improves Orthopaedic Surgical Skills Training.

BACKGROUND: Human cadaver surgical skills training offers the highest-fidelity simulation of the operative environment, with the potential to enhance surgeon training and to reduce operative risks to patients. Embalming extends the duration that a cadaveric specimen may be used for surgical skills training and reduces the risk of disease transmission, but it can alter the properties of the cadaver tissue, reducing the simulation fidelity and training quality. The purpose of this controlled laboratory study was to evaluate 3 embalming methods, formaldehyde solution, alcohol-glycol solution, and

Critical design and validation considerations for the development of neonatal minimally invasive surgery simulators.

BACKGROUND/PURPOSE: Pediatric surgical trainees have limited exposure to advanced minimally invasive surgery (MIS) during their clinical training, particularly for cases such as esophageal atresia/tracheoesophageal fistula (EA/TEF). Simulation on validated neonatal models offers an alternative means of training that may augment traditional forms of training; but to be useful, they must fulfill certain criteria. METHODOLOGY: Review of the currently available MIS, thoracoscopic and laparoscopic, simulators for pediatric surgery, and identification of those factors that contribute to their fidel

[Bringing clinical simulation into an Anesthesia residency training program in a university hospital. Participants' acceptability assessment].

INTRODUCTION: Clinical simulation is currently an integral part of the curriculum of the Anesthesiology residency programs in other countries. We aimed to describe and evaluate the insertion of simulation in an anesthesia residency training program. METHODS: Activities feasible to be used for training in a simulated environment were classified into 2 modules: workshops for technical skills conducted with first year residents, and high fidelity simulation scenarios performed with second and third year residents. After each activity, and using an anonymous questionnaire, residents assessed thei

Ballooning to Success: A Novel and Reproducible Sengstaken-Blakemore Model for Educating Emergency Medicine Trainees.

Balloon tamponade placement for variceal hemorrhage is a critical but rarely performed emergency procedure. Current hands-on low-fidelity simulation task trainers are limited, leaving a gap in high-quality training. We developed an anatomically accurate, cost-effective task trainer constructed from commonly available Emergency Department items to teach balloon tamponade device insertion for controlling variceal bleeding. This descriptive report of a low-fidelity simulation model uses 5-inch pieces of flexible silicone tubing to simulate the esophagus, connected to a 1-liter bottle as the stoma

Evaluation and Implementation of a High-Fidelity Cleft Palate Simulator.

BACKGROUND: Cleft palate repair is a challenging procedure to learn because of the delicate tissue handling required and the small confines of the infant oral cavity. As a result, cleft palate simulators have previously been described to augment cleft palate repair training. Although valuable, they lack the fidelity for this complex procedure. METHODS: A high-fidelity cleft palate simulator was evaluated by staff and fellows in pediatric plastic surgery who provided feedback on its realism, anatomical accuracy, and effectiveness as a training tool. The simulator was implemented within a train

Development of cadaver perfusion models for surgical training: an experimental study.

PURPOSE: Perfusion techniques on cadavers are heterogeneous and imperfect. The objective of this study was to improve the existing circulation model for surgical simulation on cadavers. METHODS: We used a three-step experimental approach. The first part of the experiment tested two variables: the type of circuit and the use of a heater for perfusion. The second approach evaluated two parameters: the injection fluid and the type of body conditioning (embalmed or freshly dead prepared using different washing techniques). The third one was an improvement on the best circulation obtained, which f

Oral HIV Self-Implemented Testing: Performance Fidelity Among African American MSM.

Oral-Self Implemented HIV Testing (Oral-SIT) offers a low-cost way to extend the reach of HIV testing systems. It is unclear, however, if high risk populations are able to perform the test with high fidelity. Using a simulation-based research design, we administered desensitized Oral-SIT kits to African American MSM (AAMSM; 17-24 years, N = 178). Participants were HIV negative or never tested, and had never self-administered an Oral-SIT kit. We assessed performance fidelity, and hypothesized antecedents. High levels of social stigma were associated with lower levels of training knowledge (Rang

Learning Gain and Skill Retention Following Unstructured Bronchoscopy Simulation in a Low-fidelity Airway Model.

BACKGROUND: Simulation is invaluable for bronchoscopy training. Studies report improved procedure time, dexterity/technique, and trainee satisfaction supported by low-fidelity and high-fidelity simulators in structured-training programs. We sought to determine (1) Learning-gain in bronchoscopic dexterity after a single 45-minute unstructured exposure using a low-fidelity simulator. (2) Whether acquired skills are maintained 8 weeks later, during which trainees receive no interim exposure to simulation or clinical bronchoscopy. METHODS: Using a low-fidelity model, medical students were assesse

Comparative Effectiveness of Physical and Virtual Reality Simulators in Robotic Surgical Training.

Introduction: Robotic surgery training requires effective simulation methods to ensure proficiency. Virtual reality (VR) simulators and physical models offer different approaches. Methods: A study was conducted with 30 surgical residents, divided into two groups: one trained on a high-fidelity physical simulator and the other on a VR simulator. Both groups completed standardized exercises, followed by an assessment of their surgical performance using the da Vinci Surgical System simulator. Performance scores were analyzed using statistical methods, including t-tests and multiple linear regress

High-Fidelity Simulation for Assessing Catheterization-Based Skills Among Trainees.

BACKGROUND: Simulation-based training for coronary angiography has not been widely adopted, and an operator's skillset should be accurately reflected in a simulation-based task. The purpose of this study is to determine whether increasing levels of clinical experience were associated with improved performance and whether trainees' confidence improved with use of a high-fidelity simulator. METHODS: Physicians with different catheterization experience (internal medicine residents, first- through third-year cardiology fellows, interventional fellows, and attending interventional cardiologists) p

An Innovative Application of High-Fidelity Medical Simulators to Objectively Demonstrate the Impact of Sports on the Development of Fine Motor Skills-A Pilot Study.

Operative medicine needs fine manual skills; therefore, several educational training programs focus on skill development as well. Related to sports sciences, various sport types are also dependent on fine motor skills. We hypothesized that an adequate sport training program may contribute to the development of medical students' manual dexterity. We conducted objectively tests using high-fidelity medical simulators. Volunteer medical students were delegated to table tennis group (TG), where students participated in 2 h/week of table tennis training for 7 weeks, or to a Control group (CG) that i

Simulation-Based Up-Training in Response to the COVID-19 Pandemic.

The dramatic outbreak of COVID-19 placed unprecedented strain on the critical care workforce of New York City. The enhanced precautions required to safely care for COVID-19 patients impacted the performance of even routine critical care procedures. Meanwhile, staff were stretched to care for exponentially rising case volume as COVID intensive care units (ICUs) expanded. Simulation was used to bridge these gaps-first to familiarize personnel within the Division of Pulmonary and Critical Care with revised COVID-19 care procedures, then to orient noncritical care clinicians volunteering from othe

Incorporating Cleft Lip Simulation Into a "Bootcamp-Style" Curriculum.

INTRODUCTION: The traditions of surgical education have changed little over the years. However, the increasing focus on patient safety and duty hour restrictions mandates that residents start developing complex skill sets earlier to ensure they graduate with procedural competency. Surgical training is poised to exploit high-fidelity simulation technology to mitigate these pressures. METHODS: By revisiting principles of adult learning theory, the authors created a "bootcamp-style" cleft lip curriculum that sought to (1) maximize educational impact and (2) pilot a high-fidelity procedural train

Simulation of cotside silo application, reduction, and sutureless closure, for gastroschisis: Performance of the GABBY-T model.

AIMS: Simulation training enhances efficacy and safety of preformed silo deployment in gastroschisis. Limitations of existing simulation models include use of biological material, low fidelity, poor durability, and high cost. We developed a novel gastroschisis simulation model and report its validation and performance in a multicentre interventional study. METHOD: Paediatric surgeons collaborated with a medical-model specialist to develop a novel GAstroschisis BaBY (GABBY)-T model. This was validated against two existing simulation models by 15 paediatric surgeons using a Likert scale (1-10)

Toward the design of a tailored training course for birth assistance: an Ethiopian experience.

Simulation in healthcare has already demonstrated extraordinary potential in high-income countries. However, to date, few authors have explored the possibility of applying simulation-based training in African settings, highlighting the necessity of need-based training protocols capable of addressing economic, social, and cultural aspects. In this framework, this research investigates the main features of a simulation training course on umbilical cord care and placenta management should be considered effective and sustainable in an African healthcare environment. Local facilitators were identif
