A qualitative analysis of emotion and emotion regulation in hoarding disorder.

OBJECTIVE: The role of emotion regulation (ER) has been receiving increased attention in relation to various forms of psychopathology including hoarding disorder (HD). However, questionnaire designs are limited to finding associations of ER with symptoms or symptom groups, without finding out how such constructs might be involved in the disorder. METHODS: This study was a qualitative investigation of ER in a clinical HD sample (N = 11). RESULTS: Prominent themes provided support for ER difficulties in hoarding. In particular, difficulties with identifying and describing feelings, unhelpful a

Hoarding in obsessive compulsive disorder: results from a case-control study.

Hoarding occurs relatively frequently in obsessive-compulsive disorder (OCD), and there is evidence that patients with hoarding symptoms have more severe OCD and are less responsive to treatment. In the present study, we investigated hoarding symptoms in 126 subjects with OCD. Nearly 30% of the subjects had hoarding symptoms; hoarding was twice as prevalent in males than females. Compared to the 90 non-hoarding subjects, the 36 hoarding individuals had an earlier age at onset of, and more severe, obsessive-compulsive symptoms. Hoarders had greater prevalences of symmetry obsessions, counting c

Differential patterns of brain activation between hoarding disorder and obsessive-compulsive disorder during executive performance.

BACKGROUND: Preliminary evidence suggests that hoarding disorder (HD) and obsessive-compulsive disorder (OCD) may show distinct patterns of brain activation during executive performance, although results have been inconclusive regarding the specific neural correlates of their differential executive dysfunction. In the current study, we aim to evaluate differences in brain activation between patients with HD, OCD and healthy controls (HCs) during response inhibition, response switching and error processing. METHODS: We assessed 17 patients with HD, 18 patients with OCD and 19 HCs. Executive pr

To hoard or not to hoard purchased items - does it matter? A clinical cross-sectional study on compulsive buying-shopping disorder.

BACKGROUND: Research with individuals with compulsive buying-shopping disorder (CBSD) indicated that comorbid hoarding disorder (HD), as determined by questionnaire cutoffs, is associated with more severe CBSD. This study investigated a clinical sample with CBSD and determined the prevalence of HD by clinical interview, explored whether hoarding refers to consumer goods and/or non-purchased, free items, and compared patients with (HD+) and without (HD-) comorbid HD regarding materialism, impulsiveness, general psychopathology, CBSD and HD symptoms. Additionally, the relationship between CBSD s

Do they really care? Specificity of social support issues in hoarding disorder and obsessive-compulsive disorder.

OBJECTIVES: Unmet interpersonal needs may play a role in excessive emotional attachments to objects for people with hoarding disorder (HD). Previous research indicates that social support (but not attachment difficulties) may be specific to HD. The study aimed to evaluate social networks and support in HD relative to clinical controls with obsessive-compulsive disorder (OCD) and healthy controls (HC). The secondary aim was to explore the extent of loneliness and thwarted belongingness. Potential mechanisms for deficits in social support were also considered. DESIGN: A cross-sectional between-

A unique increase in prefrontal gray matter volume in hoarding disorder compared to obsessive-compulsive disorder.

BACKGROUND: Hoarding disorder (HD) is a disease concept newly presented in DSM-5. As far as we know, no studies have examined the structural changes relevant to hoarding by applying the diagnostic criteria of HD in DSM-5. In the present study, we aimed to find abnormalities in gray matter (GM) structures of patients with HD. METHODS: Seventeen patients who met the DSM-5 criteria for HD, 17 obsessive-compulsive disorder (OCD) patients, and 17 healthy controls (HCs) participated in this study. All participants underwent MRI scanning of the brain by a 3.0-Tesla MRI scanner. In a voxel-based morp

Identifying psychiatric and neurological comorbidities associated with hoarding disorder through network analysis.

BACKGROUND: The relationships between hoarding disorder (HD) and other neurological and psychiatric disorders remain largely unknown. Although psychiatric burden in those with HD is high, less is known about neurological disorders. Furthermore, which disorders are primarily associated with HD vs which can be better explained via a relationship with another disorder has not been determined. To address these questions, we examined comorbidity patterns of psychiatric and neurological disorders in a large online registry of adults using network analyses. METHODS: We first examined psychiatric com

Group cognitive-behavioural therapy for hoarding disorder: Systematic review and meta-analysis.

Previous meta-analysis has reported the rate of reliable and clinically significant changes in hoarding disorder (HD) after cognitive-behavioural therapy (CBT) based on the classical CBT model of HD, as between 42% and 25%. However, in this analysis, different types of therapy (group vs individual, G-CBT and I-CBT, respectively), different providers (psychologist vs nonpsychologist), and different diagnosis (HD vs hoarding behaviour) were analysed together. Hence, it remains unclear if reported rate of changes was due to limits of the CBT model of HD or due to the fact that different applicati

Clinical features of obsessive-compulsive disorder with hoarding symptoms: a multicenter study.

BACKGROUND: Factor analyses indicate that hoarding symptoms constitute a distinctive dimension of obsessive-compulsive disorder (OCD), usually associated with higher severity and limited insight. The aim was to compare demographic and clinical features of OCD patients with and without hoarding symptoms. METHOD: A cross sectional study was conducted with 1001 DSM-IV OCD patients from the Brazilian Research Consortium of Obsessive-Compulsive Spectrum Disorders (CTOC), using several instruments. The presence and severity of hoarding symptoms were determined using the Dimensional Yale-Brown Obses

Spontaneous activity and functional connectivity in patients with hoarding disorder comorbid with attention-deficit/hyperactive disorder.

Despite recent studies suggesting an important association of hoarder disorder (HD) and attention-deficit/hyperactive disorder (ADHD), no neuroimaging study has investigated the differences between patients with HD comorbid with ADHD and those without ADHD. This study investigated the regional spontaneous activity and functional connectivity in HD, focusing on the comorbidity with ADHD. Resting-state functional magnetic resonance imaging (MRI) data were obtained from 24 patients with HD and 31 healthy individuals. We investigated the group differences using the fractional amplitude of low-freq

Differences in symptom severity in hoarding disorder between those with and without attention deficit-hyperactivity disorder.

Hoarding disorder (HD) is associated with symptoms of inattention. Previous studies relied on self-report assessments, which pose limitations in diagnosis; however no study has investigated the severity of hoarding in patients with and without comorbid attention-deficit/hyperactivity disorder (ADHD) using structured diagnostic interviews. A total of 68 patients met the diagnostic criteria for HD, including 20 (29 %) with comorbid ADHD (20/68). This study aimed to compare the severity of hoarding symptoms in HD patients diagnosed with and without ADHD using a structured interview. The participa

Group cognitive behavioural therapy for hoarding disorder: outcomes from ten closed groups.

BACKGROUND: A group cognitive behavioural therapy (GCBT) protocol was developed for people with hoarding disorder (HD) as part of routine clinical care in a tertiary mental health Obsessive Compulsive service in Southwest London in 2014; to date 10 groups have been facilitated, and 71 people have been treated. AIM: The aim of the report was to describe the GCBT programme and ascertain if it is effective in treating symptoms of HD. METHOD: Each group cycle consisted of 11 or 12 × 2-hour group sessions, run on a closed group basis. A home-based assessment and a follow-up home visit was conduct

Hoarding Disorder and Diogenes Syndrome: Two Case Reports and a Narrative Review.

Hoarding disorder (HD) and Diogenes syndrome (DS) are challenging and multifaceted conditions that significantly impact patients and society. These disorders are characterized by excessive hoarding behaviors, often accompanied by multiple physical and mental comorbidities, diminished quality of life, and substantial public health challenges. Through the presentation of two case reports and a narrative review, this article revisits HD and DS, highlighting the need for clinical awareness, targeted management strategies, and further research in this field. The first case report describes a 35-yea

The Impact of Hoarding Disorder on Family Members, Especially the Significant Other.

Hoarding disorder, also known as compulsive hoarding, comes under the umbrella term of obsessive-compulsive disorder and related disorders. The constant building of clutter in the house of hoarders makes it impossible for family members to live a healthy life. It can have appalling effects on their mental health and can lead to severe depression and suicidal ideation. The shame and humiliation attached to hoarding does not allow the hoarders to seek help, causing them distress and hence continuing the vicious cycle of hoarding. By presenting the case of a patient with a spouse with a hoarding

A Case of Clutter and Chaos: Attention Deficit Hyperactivity Disorder Confounding Hoarding.

Hoarding disorder is a chronic disorder defined as the persistent difficulty in parting with possessions and the need to save items, regardless of their actual value. Severe hoarding has largely been a hidden clinical problem, and awareness has mostly been limited to voyeuristic depictions of the plight of hoarders in popular media. Approximately 28% to 32% of individuals with attention deficit hyperactivity disorder (ADHD), especially the inattentive subtype, have been reported to have clinically significant hoarding. It has been hypothesized that patients with ADHD initially acquire objects

Excessive Object Attachment in Hoarding Disorder: Examining the Role of Interpersonal Functioning.

Excessive attachment towards possessions can be maladaptive because it can lead individuals to excessively acquire and save objects. Little is known about how attachment to objects develops and changes over time; however, interpersonal factors have been theorized to play a role. The current study examined whether interpersonal factors, specifically interpersonal attachment style and empathy, moderate changes in object attachment over time. A total of 145 participants with excessive acquiring and discarding difficulties rated their level of attachment to a novel object just after receiving it,

Object attachment in hoarding disorder and its role in a compensatory process.

Hoarding disorder (HD) is characterised by compulsive acquisition and extreme difficulty discarding possessions, resulting in clutter that substantially impacts upon functioning. Heightened object attachment is a central feature of HD according to prominent theoretical models. We review current research on the nature and function of object attachment in HD. In particular, we describe growing evidence that object attachment in HD is a compensatory response to unmet relatedness needs, and recent research underscoring the insecure nature of object attachment in hoarding. Critical gaps include the

Hoarding Disorder and a Systematic Review of Treatment with Cognitive Behavioral Therapy.

Until recently, compulsive hoarding has been treated as a type of obsessive-compulsive disorder with mixed results. Little research exists on the efficacy of behavioral interventions specifically designed to treat hoarding disorder, and most existing research is limited with regard to the numbers of participants, their ethnic and cultural diversity, and study replication; therefore, the generalizability of findings is limited. This article reviews the prevalence of hoarding disorder, cognitive behavioral therapy treatment approaches, and measurement of symptoms. A systematic review compares th
