A computed tomography-based classification of Hoffa fracture: Surgical treatment considerations and prognostic outcome with assessment of reproducibility.

INTRODUCTION: Isolated fracture of femoral condyle in the coronal plane (Hoffa fracture) is rare and surgically challenging to treat. The purpose of this study is to introduce a new classification for this fracture, evaluating its utility in treatment guidelines and its reproducibility. MATERIALS AND METHODS: 103 patients with Hoffa fracture were operated in our tertiary care centre from 2004 to 2014. Computed tomography (CT) was done in all cases preoperatively. Many fractures had variable patterns which could not be defined and categorized as per Letennuer's classification. A pilot study wi

Long Term Follow-up of an Open Bicondylar Hoffa Fracture with a Disrupted Extensor Mechanism: A Case Report.

This is the first report of a long-term follow-up of an open bicondylar Hoffa with patella fracture. It is interesting to note the radiological changes of osteoarthritis 15 years after global intra-articular injury of the distal femur. The good clinical outcome is possibly due to the integrity of the knee ligaments and reconstruction of the extensor mechanism in addition to stable anatomical reduction and fixation.

Arthroscopy-Assisted Fixation of an Intra-Articular Medial Femoral Condyle (Hoffa) Fracture of the Knee: A Case Report.

CASE: A thirty-one-year-old woman was diagnosed with a minimally displaced medial femoral condyle (Hoffa) fracture. Reduction and retrograde percutaneous screw fixation were performed under arthroscopic visualization. At three months postoperatively, she was able to walk without support, had no pain, and had full range of movement of the knee. CONCLUSION: In Hoffa fractures, arthroscopy-assisted fixation is an option for treatment. It is advantageous because it allows the ability to evaluate other intra-articular structures, decreases blood loss, shortens surgical time, decreases soft-tissue

Single-Stage Fixation of Bilateral Open Medial Hoffa Fractures Associated with Bilateral Patella Fracture and Bilateral Distal Radius Fractures: A Case Report.

INTRODUCTION: Posterior cruciate ligament (PCL) avulsion fractures, lateral collateral ligament (LCL)-biceps femoris complex avulsion injuries are relatively uncommon injuries, and associated diaphyseal tibia fracture further is the rarest combination. It is challenging to diagnose such complex injuries, and missed injuries, particularly at the knee can lead to significant instability, increasing patients' morbidity. CASE REPORT: A 25-year-old male patient was diagnosed with PCL avulsion fracture, LCL - biceps femoris complex injury at the fibular side with Type I open mid-shaft both-bone leg

Different internal fixation methods for Hoffa-like fractures of the tibial plateau: a finite element analysis.

Due to the low incidence of posteromedial tibial plateau fractures and limited clinical data available, the optimal treatment for this type of fracture remains to be established. This type of fracture, also known as Hoffa-like fracture of the tibial plateau, shares a similar mechanism of injury with the Hoffa fracture of the femoral condyle. In the field of orthopedics, finite element analysis is considered a valuable method to guide clinical decision-making. In this study, four methods used for internal fixation of Hoffa-like fractures of the tibial plateau were compared using computer simula

Hoffa's fracture in a five-year-old child diagnosed and treated with the assistance of arthroscopy: A case report.

BACKGROUND: Hoffa's fracture is a coronal-oriented fracture of the femoral condyle. It is rarely observed in pediatric patients that isolated coronal fracture of the medial femoral condyle accompanies an intact lateral femoral condyle. Only a few cases involving Hoffa's fracture of the medial femoral condyle have been reported in patients with undeveloped skeletons. Such a fracture cannot be observed by routine imaging examinations, thus resulting in possible misdiagnosis and further treatment challenges. CASE SUMMARY: A 5-year-old boy with Hoffa's fracture of the medial femoral condyle suffe

Medial Hoffa Fracture: Description of a Novel Classification System and Rationale for Treatment Based on Fragment-Specific Fixation Strategy.

PURPOSE: This study aims to present a novel classification system and a rationale for treatment of medial Hoffa fractures. METHODS: We developed a simple comprehensive classification system for medial Hoffa fractures based on the fragment size and presence of fracture comminution. Furthermore, we propose a treatment algorithm based on two pillars: our case series of nine patients presenting medial Hoffa fractures and the best evidence-based pertinent literature. Fracture healing, range of motion, function, and complications were evaluated after a minimum of 6 months follow-up. RESULTS: All f

Surgical treatment and rehabilitation of medial Hoffa fracture fixed by locking plate and additional screws: A retrospective cohort study.

PURPOSE: Hoffa fracture fixed by only using a single plate or lag screws might be not strong enough to achieve direct stability. The goal of this study is to determine the functional outcome of the surgical treatment and rehabilitation of medial Hoffa fracture by a locking plate combined with cannulated or lag screws. METHODS: A total of 13 patients suffering isolated medical Hoffa fractures were identified during the study period (2005.February-2013.February) and retrospectively analyzed. All the fractures were treated by open reduction via the medial approach, and internal fixation by a loc

Nonunion of an Isolated Medial Condyle of a Femur Hoffa Fracture in a 21-Year-Old Male: A Case Report.

Isolated Hoffa fractures of the femur are often missed on initial radiographic evaluations. Routine CT scans for intraarticular fractures in suburban populations are not routinely done. Nonunion of medial condyle Hoffa fracture of the distal femur is a rare finding and presents late with pain in weight-bearing and painful flexion. This is a case report of a 21-year-old male who sustained trauma as a result of a motorcycle-car collision and was managed elsewhere conservatively on an above-knee slab. He presented after five months to our side with a limited range of movement at the knee and pain

Algorithmic treatment of Busch-Hoffa distal femur fractures: A technical note based on a modified Letenneur classification.

Treatment of distal femur fractures in the coronal plane can be challenging. Depending on fracture line orientation, topography and associated comminution, decision-making regarding approach and fixation is not straightforward and can result in complications. Therefore, treatment of coronal plane distal femur fractures (Busch-Hoffa fractures) should be approached in a systematic manner, leading to efficient planning and operative execution. Here, we offer a proposed treatment algorithm, guiding treatment, approach and fixation based on the modified Letenneur classification of coronal plane dis

Surgical treatment of femoral medial condyle impaction fracture complicated with Hoffa fragment by contralateral medial distal tibia plate: a case report.

BACKGROUND: Medial femoral condylar fractures are uncommon and present diagnostic and management challenges. Medial buttress plating should be utilized for treatment. However, there are currently no available anatomical plates that fit the femoral medial condyle, posing management challenges and necessitating the use of alternative implants. CASE PRESENTATION: We present a case of a 65-year-old male with poliomyelitis who experienced a ground-level fall resulting in a comminuted medial condylar impaction fracture with a posterior Hoffa fragment. Reduction was achieved through a medial approac

Pediatric lateral Busch-Hoffa fracture associated with lateral meniscus tear and incarcerated popliteus tendon: systematic review of literature and case report.

INTRODUCTION: Busch-Hoffa fractures are rare, particularly in the pediatric population, with limited available literature. As it is an inherently unstable fracture and as the posterior femoral condyle is prone to non-union, correct and adequate treatment of the fracture is critical but challenging. OBJECTIVE: This study aims to (1) systematically review the literature on pediatric Busch-Hoffa fractures, and (2) present a rare case of a lateral Busch-Hoffa fracture with a bucket-handle lateral meniscus tear and popliteus tendon incarceration in a pediatric patient. METHODS: A systematic revie
