Risk factors associated with patient assaults of home healthcare workers.

This study used surveys from 677 home healthcare aides and nurses to explore factors associated with assaults by patients. Among respondents, 4.6% reported one or more patient assaults (being hit, kicked, pinched, shoved, or bitten) during the past year. Logistic regression analysis examined associations between several potential risk factors and assaults. Three factors were significant, including having one or more patients with dementia (OR = 4.31, 95% CI 1.47-12.67), routinely handling patients (OR = 8.48, 95% CI 1.89-37.94), and perceiving threats of violence by others in and around patien

Attitude to work and health problems among home care workers in settings with different degrees of institutionalization.

The relationship between different degrees of institutionalization in the care of the elderly (service house, home care, and a combination of both) and work-load, as well as staff feelings regarding psychosocial variables and health complaints, was investigated. Seventy-one staff members similar in background factors answered questionnaires semi-annually during a period of 3 years. The results revealed consistent and stable differences between the different settings. The staff in service houses had the highest documented and subjective work-load and were doubly at risk for neck pain, fatigue a

Family caregivers in rural Uganda: the hidden reality.

We conducted 16 in-depth interviews with family caregivers of AIDS patients in three rural districts in western Uganda. They were selected from a client visitation list of the home-based care program for AIDS patients, based on volunteer participation. Family caregivers reported huge problems associated with providing the necessary psychological, social, and economic care. They also said that the physical and emotional demands of caregiving are overwhelming daily challenges. Most support to AIDS patients provided by family, friends, and the churches. The study highlights the great burden of ca

Return on Returns: Building Scientific Capacity in AIDS Endemic Countries.

We examine public funding for health research through the United States National Institutes of Health Fogarty International Center's AIDS International Training and Research Program, which sought to strengthen scientific capacity in AIDS-endemic countries by providing African researchers with training opportunities in the U.S. We use a difference-in-differences framework with information on scientists who participated in the program and the research activities of African scientists working at their home institutions in adjacent topics of neglected tropical diseases. Relative to comparable scie

Behind the screens: care staff observations on delivery of oral health care in nursing homes.

OBJECTIVE: To identify qualitatively carer staff attitudes, practices and critical comments related to oral health care of functionally dependent nursing home clients. DESIGN: Open-ended questions included in a longer quantitative questionnaire. SETTING: 22 randomly selected nursing homes in the Bristol area. SUBJECTS: 416 carers employed in these homes. RESULTS: The majority of carers thought that clients had a right to good oral health, accepted the carers' role in helping clients with oral and denture hygiene, but recognised that oral health care provision was deficient. However, some c

Association between the home-to-healthcare center distance and hearing aid abandonment among older adults.

BACKGROUND: Access to audiology services for older adults residing in sparsely populated regions is often limited compared to those in central urban areas. The geographic accessibility to follow-up care, particularly the influence of distance, may contribute to an increased risk of hearing aid abandonment. OBJECTIVE: To assess the association between the home-to-healthcare-calibration-center distance and hearing aid abandonment among older adults fitted in the Chilean public health system. METHODS: 455 patients who received hearing aids from two public hospitals in two regions were considere

AI-facilitated home monitoring for cystic fibrosis exacerbations across pediatric and adult populations.

BACKGROUND: AI-aided home stethoscopes offer the opportunity of continuous remote monitoring of cystic fibrosis (CF) patients, reducing the need for clinic visits. AIM: This study aimed to analyze the possibility of detecting CF pulmonary exacerbations (PEx) at home using an AI-aided stethoscope (AIS). MATERIALS AND METHODS: In a six-month study, 129 CF patients (85 children, 44 adults) used AIS for at least weekly self-examinations, recording various parameters: wheezes, rhonchi, crackles intensity, respiratory and heart rate, and inspiration-to-expiration ratio. Health state surveys were a

Teaching Safety, Saving Skin: Preventing Childhood Hot Water Burns Through Caregiver Education in Soweto, South Africa.

BACKGROUND: Though preventable, burns are the leading cause of trauma-related death in children under 4 years in South Africa and account for almost half of all pediatric trauma cases at Chris Hani Baragwanath Academic Hospital. We investigated the impact of an education-based pediatric hot water burns prevention program on the knowledge and behavior of caregivers in Soweto. METHODS: We conducted a quasi-experimental nonequivalent controlled before-and-after intervention study with a 7-month follow-up. Households were randomly selected within purposively identified high-burden burns clusters.

[Preventing dependency in elderly persons: randomized study in the community].

The number of elderly people aged > 85 years will double by the year 2020. Helping the elderly to live independently at home is a major challenge for society. Physical exercise and protein energy intake might both be effective in preventing dependency. This paper describes the organisation of an ongoing study aimed at evaluating the preventive effect of such measures, involving 102 elderly persons randomised into two groups, one receiving preventive measures but not the other. The study organisation was based on representatives and selected workers from associations involved in home assistance

Recombinant human erythropoietin and health-related quality of life of AIDS patients with anemia.

To evaluate the effect of recombinant human erythropoietin on anemia and health-related quality of life in patients with acquired immunodeficiency syndrome (AIDS), we initiated an observational study with an open-label multicenter treatment protocol that involved multiple academic and community physicians in the United States. Our subjects comprised 251 anemic (i.e., hematocrit < 30%) patients with a clinical diagnosis of AIDS using 1987 CDC criteria, age > or = 12 years, and serum erythropoietin level < or = 500 IU/L. The initial dosage of recombinant human erythropoietin was 4,000 units subc

The authoritative metaphor and social change: Surgeon General C. Everett Koop's Direct Mailer, "Understanding AIDS".

In 1988, Surgeon General C. Everett Koop published "Understanding AIDS," the nation's first and only direct mailing sent to every private home in the country. His appeals therein were driven by what we label authoritative metaphors. Communicated by and/or attributed to persons of authority, authoritative metaphors capitalize on the symbolic force of sanctioned power by appealing to the ethos of office. In "Understanding AIDS," we find that Koop drew from his positions as a surgeon and a general, respectively, to equate AIDS with an unprecedented plague and an unprecedented war. He created new

Hearing difficulties, uptake, and outcomes of hearing aids in people 85 years of age.

OBJECTIVE: The aim of this study was to investigate self-reported hearing difficulties, uptake, and hearing-aid outcomes and their relationships to demographic, cognitive, psychosocial, and health variables in 85 year olds. DESIGN AND STUDY SAMPLE: Three hundred and forty-six elderly adults participated in a survey that included questionnaires and home visits. Fifty-five percent of participants admitted to having hearing difficulties, and 59% of these owned hearing aids. The participants' most frequently cited reason for not acquiring hearing aids was that they did not think their hearing pro

In-home work environment for home care workers in Northern Sweden before and during the Covid-19 pandemic.

BACKGROUND: The in-home work environment is the main work environment for home care workers, but it has only been sparsely studied. Our aim was to investigate the in-home work environment for home care workers by exploring challenges that arise regardless of a pandemic and by investigating Covid-19-specific challenges. METHODS: Two cross-sectional studies were conducted, one before (2017) and one during the pandemic (2021/2022) in three Swedish regions (Jämtland/Härjedalen, Västerbotten and Västernorrland), in which 1,154 (58%) out of 2,000 and 629 (33%) of 1,900 invited home care workers par

Clients' and workers' perceptions on clients' functional ability and need for help: home care in municipalities.

The aim of the study was to compare clients' and named home care (HC) workers' perceptions of clients' functional ability (FA) and need for help and to analyse which client- and municipality-related factors are associated with perceptions of client's FA. The total of 686 Finnish HC clients was interviewed in 2001. Further, the questionnaire was sent to 686 HC workers. FA was assessed by activities of daily living (ADL), which included both basic/physical (PADL) and instrumental (IADL) activities. The association between client's FA and municipality-related variables was analysed by using hiera

Innovative administration of long-acting injectables for HIV treatment enhancement at home (INVITE-HOME): implementation science study protocol.

INTRODUCTION: There is high interest in long-acting injectable antiretroviral therapy (LAI-ART) among people with HIV (PWH), with many conveniences for uptake and persistence. However, both patients and clinicians have expressed important barriers to effective implementation, including concerns about frequent clinic visits and strain on clinic resources. Administration of LAI-ART by a trained layperson injector (such as family, friend or partner of the patient) can help mitigate some of these patient-identified and clinician-identified barriers. Alternative LAI-ART delivery methods have the po

A prospective pilot trial of the health data monitoring system without educational content for patients with hypertension.

In Japan, a hypertension treatment aid application (CureApp Co., Ltd.) with educational content on hypertension was the first in the world to show significant blood pressure (BP) reduction in hypertensive patients in 2021. Omron Healthcare Corporation and I developed the Health data Monitoring System (HMS) without educational content, which allows patients to check their home BP values periodically and allows physicians to check their home BP data before the hospital visit. As a pilot study, nineteen patients with hypertension used HMS for six months. The percentage of patients achieving their

Physical and sexual abuse in the lives of HIV-positive women enrolled in a primary medicine health maintenance organization.

Over the past several years, the proportion of all cases of AIDS in the United States among adult and adolescent women has more than tripled from 7% in 1985 to 23% in 1998. Information obtained in the present study suggests that care providers need to be aware of the unique life circumstances of women with AIDS, which may predispose them to a number of negative health and mental health outcomes. Charts of the first 100 women enrolled in an intensive home-based primary medical HMO for people with advanced HIV/AIDS were examined retrospectively for evidence of trauma. Results from a chart review

A large-scale operational study of home-based therapy with ready-to-use therapeutic food in childhood malnutrition in Malawi.

Home-based therapy with ready-to-use therapeutic food (RUTF) for the treatment of malnutrition has better outcomes in the research setting than standard therapy. This study examined outcomes of malnourished children aged 6-60 months enrolled in operational home-based therapy with RUTF. Children enrolled in 12 rural centres in southern Malawi were diagnosed with moderate or severe malnutrition according to the World Health Organization guidelines. They were treated with 733 kJ kg(-1) day(-1) of RUTF and followed fortnightly for up to 8 weeks. Staff at each centre followed one of three models: m
