Feasibility of Continuous Infusions of Acyclovir.

Current guidelines for severe herpes simplex virus infection recommend 21 days of intravenous therapy. The thrice-daily administration of intravenous acyclovir makes it challenging to deliver as outpatient therapy. We describe 2 cases with confirmed or presumed neonatal herpes simplex virus encephalitis treated with acyclovir administered as a continuous-infusion at home and review the pharmacologic and clinical evidence for continuous infusions of acyclovir.

Home continuous positive inotropic infusion as a bridge to cardiac transplantation in patients with end-stage heart failure.

BACKGROUND: The clinical use of positive inotropic therapy at home in patients awaiting cardiac transplantation has not been reported since United Network for Organ Sharing (UNOS) regulations were changed to allow home infusions in Status 1B patients. METHODS: We observed 21 consecutive patients with UNOS 1B status during positive inotropic therapy at home. We used hemodynamic monitoring at the initiation of therapy to optimize dosing. We selected for home therapy patients with stable clinical status and improved functional capacity during inotropic treatment. Implantable cardioverter defibri

[Outpatient parenteral antibiotic therapy (OPAT)].

According to American College of Rheumatology fibromyalgia syndrome (FMS) is a common health problem characterized by widespread pain and tenderness. The pain and tenderness, although chronic, present a tendency to fluctuate both in intensity and location around the body. Patients with FMS experience fatigue and often have sleep disorders. It is estimated that FMS affects two to four percent of the general population. It is most common in women, though it can also occur in men. FMS most often first occur in the middle adulthood, but it can start as early as in the teen years or in the old age.

Practical considerations of outpatient infusion therapy in the HIV arena.

In conclusion, OPAT is a cost-effective, quality-controlled alternative setting for treating patients with HIV. The program provides a desirable situation for the patient, physician, and nurse. Continuity of care provided by the health care team in the physician's office is a unique situation that can meet the treatment modalities necessary to care for the HIV patient with dignity and pride. In short, OPAT offers an attractive alternative to long-term hospitalization for a variety of HIV-related infections. Such therapy is rapidly becoming a standard of treatment that provides both cost saving

Caregiving for Children With Acute Lymphoblastic Leukemia Receiving Home-Based Blinatumomab: Caregiver Experiences and Recommendations.

BACKGROUND: It is expected that blinatumomab will be increasingly incorporated into the care of children with B-lineage acute lymphoblastic leukemia (B-ALL) based on improved survival for subsets of patients and a favorable safety profile. Most of the 28-day continuous infusion can be administered in outpatient or home settings. Limited studies have described low rates of complications at home when appropriate safety plans are in place. However, home administration shifts the burden of monitoring and care to caregivers. PROCEDURE: Caregivers of children with B-ALL within 1 month of completing

Health care costs of home care enzyme replacement therapy for patients with lysosomal storage diseases in Germany.

BACKGROUND: Lysosomal storage diseases (LSDs) can be treated with intravenous enzyme replacement therapy (ERT). ERT is being administered either in specialized clinics or in the home care setting. Studies indicate that home-based ERT can be considered safe and positively effects patient reported outcomes. Due to the higher level of individual care required by nurses when administering ERT at home, questions arise whether this will result in additional costs for health care systems. Since cost data to the German health care system are currently unavailable, this study aims to assess home care E

Is home-based therapy in Fabry disease the answer to compelling patients' needs during the COVID-19 pandemic? Survey results from the Polish FD Collaborative Group.

BACKGROUND: Fabry disease (FD) is an X-linked disorder related to a deficiency of the lysosomal enzyme alpha-galactosidase A. In Poland, enzyme replacement therapy (ERT) for FD is offered by the National Health Fund only at selected hospital infusion centers. Patients with FB are considered at a high risk of developing complications from COVID-19. Some patients omitted infusions due to fear of infection or outbreaks in hospitals. Lack of alternative infusion sites hampered the situation. OBJECTIVES: To analyze the impact of the SARS-CoV-2 pandemic on FD patients, especially their fears and ex

Home care versus hospital care in patients with multiple myeloma treated with pamidronate.

Long-term bisphosphonate therapy has been shown to offer clinical benefit in the management of multiple myeloma. This study sought to explore the feasibility and potential advantages of monthly home-based intravenous infusions of pamidronate in patients with multiple myeloma. In a prospective crossover, multicentre trial, 37 patients were randomly allocated to receive 3 months of treatment with pamidronate given in the home followed by 3 months of treatment with pamidronate given in hospital or vice versa. Results from a patient preference questionnaire indicated most patients preferred treatm

Feasibility and resource utilization of nurse-administered subcutaneous immunoglobulin therapy in antibody deficiency: A cross-sectional study.

Primary and secondary antibody deficiencies (PAD and SAD) are amongst the most prevalent immunodeficiency syndromes, often necessitating long-term immune globulin replacement therapy (IRT). Both intravenous immunoglobulin (IVIG) and subcutaneous immunoglobulin (SCIG) have demonstrated efficacy in antibody deficiency. Comparative analyses of these two routes of administration are limited to nurse-administered IVIG and home therapy with self-administered SCIG. A third programmatic approach, SCIG administered by nurses in hospital-based outpatient infusion clinics, combines certain advantages of

Characterizing the Objective Time Spent on Health Care Activities in Clinic and at Home for Patients With Cancer.

BACKGROUND: While the time burden of cancer care is increasingly recognized in oncology, existing research lacks the granularity needed to identify intervention targets. This study characterized time spent on cancer care at home and in-clinic, including travel, waiting, and clinical tasks, assessing variations by treatment modality. METHODS: We conducted a secondary analysis of two longitudinal cohorts. Cohort 1 included patients with metastatic breast or advanced ovarian cancer (n = 55) and Cohort 2 included patients with any cancer (n = 55) receiving systemic anti-cancer therapy. Participan

Safety and Tolerability of Home Infusions in Down Syndrome Regression Disorder.

PURPOSE: Down syndrome regression disorder (DSRD) is a rare neuropsychiatric condition affecting otherwise healthy individuals with Down syndrome. Multiple studies on DSRD have revealed that immunotherapy with intravenous immunoglobulin (IVIg) is both safe and effective, although site of infusion has never been studied. This study sought to evaluate the safety and tolerability of IVIg in individuals with DSRD receiving home-based infusions. METHODS: A single-center, retrospective chart review evaluating infusion reactions was performed for individuals meeting criteria for DSRD and having rece

Continuous infusion chemotherapy in the ambulatory setting: the nurse's role in patient selection and education.

Continuous infusion therapy (CIT) is an alternative to traditional intravenous bolus or brief infusion chemotherapy. The benefits of CIT are improved therapeutic index, reduced toxicity, and modification of drug resistance. CIT administered in the ambulatory setting fosters cost effectiveness and improved patient satisfaction. Nurses use their clinical skill and judgement to assess potential CIT patients prior to treatment for self-care ability and to prepare them for treatment. Topics to include in patient education are care of the vascular access device, introduction to CIT, toxicities and s

Prophylactic treatment of severe factor X deficiency with prothrombin complex concentrate.

Factor X (FX) deficiency is an autosomal recessive trait that occurs in fewer than 1 in 500 000 people. Not surprisingly, reports of prophylactic treatment for FX deficiency are exceedingly rare. We now report our experience of the use of prophylactic therapy in a FX-deficient patient. This 18-year-old African-American male presented at the age of 4(1/2) years with an FX level < 1%. Treatment was on demand with prothrombin complex concentrates (PCCs) given at two times the dose per kilogram of body weight for factor IX. He experienced frequent epistaxis, soft tissue bleeding and joint bleeding

Infusion therapy patient outcomes are similar at reduced costs in alternative sites of care compared with hospital outpatient departments: A matched cohort analysis of infusion therapy across multiple chronic conditions.

BACKGROUND: Infusion therapy, used to administer medications for multiple chronic diseases, can be performed in hospital outpatient departments (HOPDs), patients' homes, ambulatory infusion centers, and physicians' offices. The site for administering infusion therapy can impact the cost of care. However, there is limited evidence on quality associated with the site of care (SOC). OBJECTIVE: To assess how patient utilization, cost, and adherence outcomes differ between infusions administered in HOPDs compared with alternative SOCs (ambulatory infusion centers, patients' homes, and physicians'
