Meleis's theory of nursing transitions and relatives' experiences of nursing home entry.

AIM: This paper explores the extent to which Meleis's mid-range theory of nursing transitions is supported by the findings of a study exploring relatives' experiences of the move to a nursing home. BACKGROUND: Mid-range nursing theories are useful tools in helping to understand the scope of nursing practice in a range of contexts and situations. However, as yet, many formal mid-range theories have not been adequately tested. METHODS: Findings from a constructivist study of relatives' experiences of nursing home entry were re-analysed in relation to the extent to which they reflected the doma

Chronic pain in elderly nursing home residents: the need for nursing leadership.

The incidence of chronic pain amongst elderly people in nursing homes is very high, making pain in this population a serious problem for aged care facilities. Research studies reveal a pattern of poor pain management in this setting despite the high incidence of pain suggesting that the management of pain in nursing homes is limited in scope and only partially effective. What is not fully appreciated by health professionals is the impact pain has on the lives of elderly people who live in nursing homes. In the study reported here a phenomenological method was used involving several in depth in

Predicting nursing home placement among home- and community-based services program participants.

BACKGROUND: Several states offer publicly funded-care management programs to prevent long-term care placement of high-risk Medicaid beneficiaries. Understanding participant risk factors and services that may prevent long-term care placement can facilitate efficient allocation of program resources. OBJECTIVES: To develop a practical prediction model to identify participants in a home- and community-based services program who are at highest risk for long-term nursing home placement, and to examine participant-level and program-level predictors of nursing home placement. STUDY DESIGN: In a retr

Sexual Abuse of Older Residents in Nursing Homes: A Focus Group Interview of Nursing Home Staff.

The objective of this study was to increase knowledge of sexual abuse against older residents in nursing homes. A qualitative approach was used. Through a focus group interview with staff in nursing homes, the aim was to reveal employees' thoughts, experiences, and attitudes. Findings from the focus group interview show that sexual abuse of older residents is a taboo topic among health professionals. Acts of sexual abuse are difficult to imagine; it is hard to believe that it occurs. The fact that staff are not aware that it could happen, or have a hard time believing that it actually happens,

Symptoms associated with urinary tract infection in nursing home residents: a study among nursing home staff across eight European countries.

Urinary tract infections (UTIs) are common among nursing home (NH) residents and a leading cause of antibiotic use. NH staff members are key in recognizing suspected UTIs based on observed symptoms. This study surveyed 286 staff across eight European countries to identify which symptoms prompt UTI suspicion. While UTI-specific symptoms like painful and frequent urination were commonly cited, non-specific signs such as foul-smelling urine, confusion, and behavioural changes were also frequently reported. These findings reveal substantial cross-country variation and highlight the diagnostic chal

Licensed Nurse and Nursing Assistant Recognition of Delirium in Nursing Home Residents With Dementia.

Many nursing home residents experience delirium. Nursing home personnel, especially nursing assistants, have the opportunity to become familiar with residents' normal cognitive function and to recognize changes in a resident's cognitive function over time. The purpose of this study was to determine the accuracy of delirium recognition by licensed nurses and nursing assistants from eight nursing homes over a 12-month period. Participants were asked to complete five case vignette assessments at three different time points (in 6-month intervals) to test their ability to identify different subtype

Prevalence of Sarcoptes scabiei in the homes and nursing homes of scabietic patients.

This study determined the prevalence of Sarcoptes scabiei in the home environment of 37 confirmed cases of scabies and in five nursing homes with scabietic patients. Dust samples from 44% of infested patients' homes contained scabies mites. Live mites at the time the dust samples were analyzed were present in 64% of these homes. Live mites were recovered most often from bedroom floors or overstuffed chairs and couches. The density of live and dead mites ranged from 1 to 5 mites/0.1 gm of dust or 1 to 9 mites/m2 of surface area sampled. The presence of mites in fomites coupled with survival and

Factors explaining the referral of the elderly to home nursing or home help.

Factors explaining the referral of the elderly to home nursing or home help or both were studied in a population (n = 1358) aged 60 years or over and living in the commune of Ahtäri, a semi-industrialized community in middle-western Finland, using social and health care registers and questionnaires in collecting data. The clients of home nursing and of combined home nursing and home help were predominantly women, whereas men were in the majority among those referred to home help. A stepwise regression analysis showed old age, living alone, low self-perceived health, low education and few visit

Disability among home-nursed patients and home-help clients.

218 chronically ill or elderly (65 years or over) home-nursed patients and home-help clients were studied to obtain information about the degree of disability, factors associated with disability, and predictors of disability. About 9% of the patients and clients needed help to a moderate extent in daily and self-care activities, but none was totally dependent. Poorly self-perceived health status, dementia and neurological disease were associated with serious disability. Male sex and living alone were correlated with high functional capacities. Only one-tenth of the patients and clients alive a

Readmission Risk Coming Home After Rehabilitation in a Skilled Nursing Home With Dementia.

BACKGROUND: Community dwelling people living with dementia (PLWD) are more likely to visit a skilled nursing home (SNH) after an acute hospital stay and have a high risk of readmission to the hospital. Staff in SNHs have familiarity with caring for PLWD that may mitigate readmissions, but dementia's role in readmissions for this high risk population remains unclear. METHODS: Using Health and Retirement Study data linked to Medicare claims between 2000 and 2019, this study aimed to investigate the role of dementia in hospital readmissions after a short rehabilitative stay (< 100 days) in the n

Texture modified diet in German nursing homes: availability, best practices and association with nursing home characteristics.

BACKGROUND: For nursing home (NH) residents with swallowing or chewing problems, appealing texture-modified-diets (TMD) need to be available in order to support adequate nutrition. The aim of this study was to describe the availability of TMD and best practices for TMD in German NHs and to identify related NH characteristics. METHODS: Information on NH characteristics, available texture-modified (TM)-levels (soft, "minced & moist", pureed) and implemented best practices for TMD (derived from menu plan, separately visible components, re-shaped components, considering individual capabilities of

Delirium in Nursing Home Residents: A Narrative Review.

Delirium is an important component of the geriatric syndromes and has been recognized to negatively influence the prognosis of older people in hospital and in a post-acute setting. About 2-5% of older people world-wide live in nursing homes and are characterized by functional impairment, cognitive decline, dementia, comorbidities, and polypharmacotherapy, all factors which influence the development of delirium. However, in this setting, delirium remains often understudied. Therefore, in this narrative review, we aimed to describe the latest evidence regarding delirium screening tools, epidemio
