Diuretic therapy (DT) plays a major role in disease management. However, one issue of concern in nutritionally vulnerable elders is that diuretic therapy also increases thiamin excretion, and little attention has been paid to the linkage between DT use and dietary intake of thiamin in this older population. The purpose of this study was to assess the relationship between DT use and dietary intake of thiamin in a randomly recruited sample of 342 homebound older adults. Baseline data, including three 24-hour recalls, were used to identify thiamin intake from food (15% < EAR and 33% < RDA), DT us

With growing evidence that sarcopenia, disability and frailty can be prevented with physical activity, people living with pain, stiffness and weakness due to a musculoskeletal condition should be offered physical activity as a preventive strategy. By changing beliefs and attitudes towards ageing and raising the value and importance of physical activity, disability can be avoided or delayed and the costly burden of social care lessened. The effects of ageing and the effects of muscle strength loss are often confused. Older people can increase their strength and decrease their fitness to that of

AIM: Improving the availability of dental care is essential to maintain older adults' general health and wellbeing. Domiciliary dental care is a feasible alternative. The present study aimed to investigate factors affecting the use of domiciliary dental care among home-dwelling dependent older adults.
METHODS: A retrospective nested case-control study was carried out. We identified long-term care recipients who used home care services between April 2012 and March 2014 using Japan's nationwide long-term care service claim database. One-to-one case-control matching was carried out between those

BACKGROUND: Engaging in thought suppression as a coping mechanism has been associated with higher rates of anxiety and depressive disorders in younger adults. Homebound older adults are a population of elders experiencing poor health and high levels of depression and anxiety. It is unclear the extent to which psychological factors, such as thought suppression, are associated with distress, given that their health and disability status may be more salient. The aim of this study was to investigate thought suppression in relation to anxiety and depressive symptoms in homebound older adults.

BACKGROUND: Micronutrient status can affect cognitive function in the elderly; however, there is much to learn about the precise effects. Understanding mediating factors by which micronutrient status affects cognitive function would contribute to elders' quality of life and their ability to remain in the home.
OBJECTIVES: The Nutrition, Aging, and Memory in Elders (NAME) Study is designed to advance the current level of knowledge by investigating potential mediating factors by which micronutrient status contributes to cognitive impairment and central nervous system abnormalities in the elderl

BACKGROUND AND OBJECTIVES: The purpose of this study was to examine the trajectories of homebound status in older adults and to investigate the risk factors in shaping the pattern of these trajectories.
RESEARCH DESIGN AND METHODS: The study sample was a nationally representative sample of Medicare beneficiaries aged 65 and older (N = 7,607) from the National Health and Aging Trends Study (Round 1-Round 7). Homebound state was defined as never or rarely went out the home in the last month. Homebound trajectories were identified using an enhanced group-based trajectory modeling that accounted

Interviews during home visits were used to assess social services and caregiving status for the bedridden and senile elderly living at home in urban Tokyo. In Chuo ward 197 bedridden and senile elderly were identified from 3,409 mailed surveys (response rate 5.8%). This study analyses data from 110 subjects who were followed up with interviews during home visits. As indicated by the Karasawa senile scale, 70% of the respondents have communication problems with the elderly person in their care. Incontinence problems are reported in 56.7% of the frail elderly. There is no significant association

Fathers report feeling sidelined and being treated as secondary parents in healthcare settings. Despite their vital caregiving contributions, fathers remain underrepresented in pediatric palliative care research, particularly outside oncological contexts. This qualitative study, using a phenomenological hermeneutical design, explores the lived experiences of Norwegian fathers raising children with progressive life-limiting conditions. Thirteen fathers of children aged 1-18 years participated in in-depth, semi-structured interviews conducted online or in person. All children had received a diag

OBJECTIVE: People with previous incarceration and a mental disorder, a substance use disorder, or both are less likely to obtain employment than are those without these characteristics. Distance learning and education (DLE) systems are increasingly being integrated into clinical care and provide a pathway to disseminate vocational services. DLE systems can help reach people with barriers to accessing services, including people without transportation, experiencing homelessness, living in rural areas, or who are homebound. This study evaluated the effectiveness of one DLE system, the COMPASS pro

Almost 50 million Americans--about one in five--live with a mental or physical disability, many in homes that are inconvenient, restrictive, or dangerous for them. Modifying the home using universal (barrier-free) design, architectural accessibility, and other such concepts can reduce the risk of falls and improve overall function. Nurses can be involved by assessing patients' living quarters and recommending specific modifications.

OBJECTIVES: To examine the (1) short-term effectiveness of behavioral therapies in homebound older adults and (2) characteristics of responders and nonresponders to the therapies.
DESIGN: Prospective, controlled clinical trial with cross-over design.
SETTING: Adults aged 60 and older with urinary incontinence and who met Health Care Financing Administration criteria for being homebound were referred to the study by homecare nurses from two large Medicare-approved home health agencies in a large metropolitan county in southwestern Pennsylvania.
MEASURES: Structured continence and medical his

INTRODUCTION: The Barthel index has designed to assess functional capacity to perform basic activities of daily living. This study analyzes the association between each of these activities and community living, the use of mobility aids, and various socioeconomic factors, as well as survival associated with each activity at 5 years.
METHODS: Study conducted between 2020 and 2025 in the functionally dependent population (Barthel ≤60) of the Orcasitas neighborhood in Madrid (Spain). A total of 127 people participated, with a mean age of 86 ± 6.3 years. Barthel index activities were classified in

The demand for house calls is increasing because of the aging U.S. population, an increase in patients who are homebound, and the acknowledgment of the value of house calls by the public and health care industry. Literature from current U.S. home-based primary care programs describes health care cost savings and improved patient outcomes for older adults and other vulnerable populations. Common indications for house calls are management of acute or chronic illnesses, coordination of a post-hospitalization transition of care, health assessments, and end-of-life care. House calls may also includ

OBJECTIVES: Although homebound older adults are at increased risk for poor nutritional health and adverse nutrition-related outcomes, little attention has focused on the tasks involved in meal preparation and consumption and the influence of those tasks on dietary intake.
METHODS: We examined the self-reported dietary intake from 3, 24-h dietary recalls and physical limitations in meal preparation and consumption (LMPC) activities from a randomly recruited sample of 345 homebound older men and women. Ordered logistic regression was used to examine the correlation of demographic characteristic

The present study was designed to investigate the relationship between self-reported oral handicap as measured by socio-dental indicators and both dental state and felt need for dental treatment in a group of frail and functionally dependent older adults. A total of 263 housebound adults over the age of 60 years were investigated with regard to their self-reported dental handicap using the socio-dental indices derived by Locker (Community Dent Health 1992;9:109-24). Of these, 117 were in long-stay residential facilities, 67 in sheltered housing and 79 in private housing. The majority were fema
