Optimization of Methods Verifying Volunteers' Ability to Provide Hospice Care.

The subject of the presented work was an attempt at optimization of the methods used for verification of the candidates for medical voluntary workers in a hospice and decreasing the danger of a negative influence of an incompetent volunteer on a person in a terminal stage of a disease and his or her relatives. The study was carried out in St. Lazarus Hospice in Krakow, Poland, and included 154 adult participants in four consecutive editions of "A course for volunteers - a guardian of the sick" organized by the hospice. In order to improve the recruitment of these workers, the hitherto methods

Antimicrobial use in patients with advanced cancer receiving hospice care.

Patients with advanced cancer receiving hospice and palliative care are highly susceptible to infections. The decision whether to treat an active or suspected infection in end-of-life care may be difficult. In order to develop guidelines for the use of antimicrobials (antibiotics and antifungals) in palliative care, we discussed antimicrobial options with 255 patients with advanced cancer at the time they entered a community-based hospice and palliative care program. We subsequently documented the use and effectiveness of the antimicrobials employed during the palliative care period. Most pati

Pediatric home-based hospice and palliative care: a scoping review.

Pediatric palliative and/or hospice care is provided across a broad spectrum of settings, ranging from inpatient to outpatient to a child's home. Pediatric home-based hospice and/or palliative care teams offer a specialized, interdisciplinary approach to care, allowing children to stay in the home while offering comprehensive support. This scoping review seeks to summarize what is known about pediatric home-based hospice and/or palliative care and to identify gaps in the current research. Following Arskey and O'Malley's scoping review framework, we searched four databases for studies focused o

[Home hospice care, working support and rehabilitation for MS/NMO].

There are a huge amount of physical, mental and financial handicaps to patients with multiple sclerosis (MS) or neuromyelitis optica (NMO) and their family members, although treatment strategies have remarkably progressed recently. Among many important issues, to support them, here we make issues of home hospice care, working support and rehabilitation. We should consider social immaturity and ignorance to resolve the issues, and the key point to contribute useful support is consistent cooperation among medical teams and appropriate understanding of MS/NMO by all the people in our society as w

Can hospice care be provided to people who live alone?

A growing number of people live alone; some in need of hospice care. Current approaches to providing care include the "patchwork" approach, institutional contracts, hospice day care, a demonstration program for people living alone, and supported housing. A brief discussion of each is presented, with a closing challenge to nursing to provide leadership and bold thinking to meet the changing need of these patients.

Palliative and Hospice Care Mobile Applications: A Comprehensive Review and Recommendations.

Palliative and hospice care, traditionally provided in hospitals, are expanding into homes and communities with an increase in the elderly population and number of patients with chronic diseases. Despite the development of various mobile applications, the characteristics and relevance of these technologies in community settings have been insufficiently analyzed, resulting in a lack of clarity regarding their effectiveness. This systematic scoping review aimed to analyze and categorize the characteristic features of mobile applications for palliative and hospice care, and propose key attributes

A Rapid Review on the Management of Constipation for Hospice and Palliative Care Patients.

Constipation is a distressing symptom that has a high prevalence in patients receiving hospice and palliative care services, particularly in cases of opioid use. A thorough assessment, root cause analysis, monitoring, and prophylactic approach are essential for symptom management and quality of life. This rapid review assessed studies published between 2018 and 2023 to identify strategies implemented by health care professionals to prevent and/or mitigate this distressing symptom. We identified 12 articles that addressed constipation in palliative and end-of-life settings and reported on the n

Home Milrinone in Pediatric Hospice Care of Children with Heart Failure.

CONTEXT: The symptom profile of children dying from cardiac disease, especially heart failure, differs from those with cancer and other non-cardiac conditions. Treatment with vasoactive infusions at home may be a superior therapy for symptom control for these patients, rather than traditional pain and anxiety management with morphine and benzodiazepines. OBJECTIVES: We report our experience using outpatient milrinone in children receiving hospice care for end-stage heart failure. METHODS: Retrospective review of a contemporary cohort of all patients at Lucile Packard Children's Hospital, Sta

Recommendations for Palliative and Hospice Care in NCCN Guidelines for Treatment of Cancer.

BackgroundIntegration of specialist palliative care into routine oncologic care improves patients' quality of life and survival. National Comprehensive Cancer Network (NCCN) cancer treatment guidelines are instrumental in standardizing cancer care, yet it is unclear how palliative and hospice care are integrated in these guidelines. In this study, we examined the frequency of occurrence of "palliative care" and "hospice care" in NCCN guidelines and compared between solid tumor and hematologic malignancy guidelines.Materials and methodsWe reviewed all 53 updated NCCN Guidelines for Treatment of

Prevalence and Clinical Intentions of Antithrombotic Therapy on Discharge to Hospice Care.

BACKGROUND: There are no guidelines for antithrombotic therapy on admission to hospice care. Antithrombotic therapy may offer some benefit in these patients, but is also associated with well-described risks. OBJECTIVE: We quantified the frequency and characteristics of patients prescribed antithrombotic therapy on discharge from acute care to hospice care. DESIGN: Retrospective cohort study. Settings/Subjects: Adult (age> = 21 years) patients discharged from acute care to hospice care between January 1, 2010 and June 30, 2014. MEASURES: Our primary outcome of interest was receiving an outpa

Hospice care in a cohort of elders with dementia and mild cognitive impairment.

The objectives of this study were to identify the characteristics associated with hospice enrollment, to determine whether enrollment in hospice was associated with less pain and psychiatric symptoms, and to assess caregiver satisfaction with care near death in a sample of patients with dementia and mild cognitive impairment (MCI). Participants included decedents with dementia or MCI and other medical illnesses whose deaths were expected (n = 81) during a 3-year prospective, longitudinal, community-based cohort study. A total of 29.6% (n = 24) of participants received hospice care prior to dea

Perinatal hospice. Comprehensive care for the family of the fetus with a lethal condition.

OBJECTIVE: To describe our experience in providing a program of structured interdisciplinary care for the families of fetuses prenatally diagnosed with a lethal congenital anomaly. STUDY DESIGN: We developed a comprehensive "perinatal hospice" program for the supportive care of families with fetuses known to have a lethal condition. Upon prenatal diagnosis of a lethal fetal condition, parents were presented with the option of elective pregnancy termination versus a multi-disciplinary program of ongoing supportive care until the time of spontaneous labor or until delivery was required for obst

Pharmaceutical care applied to the hospice setting: a cancer pain model.

A review of the concept of pharmaceutical care applied to the hospice setting with emphasis on the management of cancer pain. Traditionally pharmacists involved in hospice care have acted in a distributive function and as an information resource. The advantages of having a hospice pharmacist practicing pharmaceutical care are many and include enhanced patient quality of care through monitoring patient outcomes, establishing drug therapy protocols, enhanced patient compliance through patient education, better pain and symptom control, and staff education. Monitoring therapeutic outcomes starts

Closing the hospice gap: the value of home-based medical care.

INTRODUCTION: Home-based medical care (HBMC), longitudinal medical care provided in a patient's home led by physicians or advanced practice providers, addresses complex medical and social needs of seriously ill older adults, particularly those with dementia. We examined whether HBMC use is associated with hospice enrollment and length of stay and whether this varies by dementia status. METHODS: We analyzed data from 100% fee-for-service Medicare decedents in 2019, who were eligible for HBMC prior to death. Models used inverse probability weighting, adjusting for clinical, demographic, and reg

Evaluation of Reporting of Race and Ethnicity in Hospice and Palliative Care Research.

The disproportionate impact of the COVID-19 pandemic on marginalized communities due to structural racism has led to an increased focus on diversity, equity, and inclusion in medical literature. To assess pre-pandemic racial and ethnic representation among subjects in palliative care research, we examined the reporting of race and ethnicity in research presented at the American Academy of Hospice and Palliative Medicine and Hospice and Palliative Nurses Association State of the Science (SOS) plenary (2004-2019), as these articles frequently influence patient care and clinical practice. Prevalence Rate of Depression in Palliative and Hospice Care: A Narrative Review.

Depression is a common but not universal experience among individuals receiving hospice and palliative care. Though much research exists on depression in this population, there is little consensus on the actual prevalence rate, with estimates varying drastically. This is due, in part, to non-standardized definitions of what constitutes depression, variance based on assessment methods and tools, and the presence of somatic symptoms that may be due to physical illness and not depression, thus distorting the prevalence rate in this population. Depression can cause significant negative consequence
