The Relationship Between Hospital Construction and High-Risk Infant Auditory Function at NICU Discharge: A Retrospective Descriptive Cohort Study.

PURPOSE: To describe the difference in auditory function at neonatal intensive care unit (NICU) discharge between high-risk infant cases exposed to hospital construction during NICU stay and those not exposed. BACKGROUND: Noise produced by routine NICU caregiving exceeds recommended intensity. As California hospitals undergo construction to meet seismic safety regulations, vulnerable neonates are potentially exposed to even higher levels of noise. Ramifications are unknown. METHODS: Retrospective data-based descriptive cohort design was used to compare high-risk infant auditory function at N

Keeping our immigrants healthy: hygienist doctrine in the Hospedaria de Imigrantes da Ilha das Flores.

The Hospedaria de Imigrantes (Immigrant Lodgings) da Ilha das Flores was established in 1883 in accordance with the hygienist thinking of the time. Immigrants were isolated on the east coast of Guanabara Bay because of the epidemics of yellow fever which returned to the Imperial capital every summer since 1849-1850. Hygienists attributed the disease to the precarious health conditions in the city of Rio de Janeiro, which enabled germs to multiply and infect the atmosphere. As physicians reinterpreted the disease in light of Pasteurian theory, new procedures were adopted to receive immigrants,

Reimagining Construction and Renovation of Health Care Facilities During Emergence from a Pandemic.

The built environment has been integral to response to the global pandemic of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). In particular, engineering controls to mitigate risk of exposure to SARS-CoV-2 and other newly emergent respiratory pathogens in the future will be important. Anticipating emergence from this pandemic, or at least adaptation given increasing administration of effective vaccines, and the safety of patients, personnel, and others in health care facilities remain the core goals. This article summarizes known risks and highlights prevention strategies for dail

Conversion of positive-pressure cardiac catheterization and electrophysiology laboratories to a novel 2-zone negative-pressure system during COVID-19 pandemic.

During coronavirus disease-2019 (COVID-19) pandemic, there continues to be a need to utilize cardiac catheterization and electrophysiology laboratories for emergent and urgent procedures. Per infection prevention guidelines and hospital codes, catheterization and electrophysiology laboratories are usually built as positive-pressure ventilation rooms to minimize the infection risk. However, patients with highly transmissible airborne diseases such as COVID-19 are best caredfor in negative ventilation rooms to minimize the risk of transmission. From a mechanical and engineering perspective, posi

Radiological assessment of the structural shielding adequacy of the radiotherapy facility at Korle-Bu Teaching Hospital, Accra, Ghana.

A (60)Co radiotherapy source with an initial activity of 185 TBq has exhausted its useful half-life and has been replaced with a 222 TBq (60)Co source at the Korle-Bu Teaching Hospital; a radiological assessment was performed to ascertain the shielding integrity of the facility. Dose rate at selected critical positions were calculated for the old and new sources. Dose rate measurements were also performed at these critical locations for the new source. The dose rates at all the critical locations of the public and staff access areas were within the recommended dose rate limit of 0.5 and 7.5 µS

[From hospice to high-rise hospital--the development of western hospitals].

Departing from the first infirmaries in the Roman army installed as valetudinaria exclusively for sick and wounded soldiers the development of western hospital care began based on the idea of love for fellowman. The importance of the order of Monte Cassino founded by Benedict of Nursia in the 6th century is emphasized. The role of these hospices important as asylums and hospitals with spiritual assistance is exposed as well as the endeavours of lords, kings and citizens to found hospitals in the modern sense from the 17th century onwards. The institution of the general hospital became importan

Restructuring isolation: hospital architecture, medicine, and disease prevention.

This paper examines changing strategies of isolation in the New York Hospital between 1771 and 1930 by correlating the facilities available for isolation with changing reactions to internal disease incidence, changing medical rules and regulations, and shifting ward categories. To prevent internal "epidemics" of telltale diseases such as erysipelas, pyemia, and "hospital gangrene," what (or who) was isolated from what, and how that isolation was achieved, altered drastically. Traditional strategies of increasing the air space and flow around each patient gave way to Florence Nightingale's sani

[Nosocomial myiasis in Latin America and the Caribbean: an overlooked reality?].

Nosocomial myiasis is an infestation by fly larvae that occurs while a patient is hospitalized. To analyze the available information on nosocomial myiasis in Latin America and the Caribbean, a search was done for cases published in the last 52 years. Nine clinical cases were found for Brazil, Costa Rica, French Guiana, Honduras, and Jamaica. Two other publications mention 139 cases in El Salvador and some 32 in Colombia, respectively. The patients and environments described presented conditions that predispose to this type of infestation. Compulsory notification is not usually required for nos

A pseudoepidemic of Sporothrix cyanescens pneumonia occurring during renovation of a bronchoscopy suite.

Sporothrix cyanescens, an uncommonly isolated mold, was cultured from biopsy tissue, bronchial washings and sputum obtained at bronchoscopy from four patients during a one-month period. These patients had undergone fiberoptic bronchoscopy to aid in the diagnosis of pneumonic infiltrates. During this period of time the bronchoscopy suite was being extensively renovated. Cultures of bronchoscopes, biopsy forceps and effluents from the bronchoscopy injection ports were negative for fungi, as were all environmental samples taken from the bronchoscopy suite. Known pulmonary pathogens were eventuall

Facility issues of the 1990s.

In the 1980s there was overbuilding of facilities of all kinds, including facilities with acute psychiatric beds. A subsequent decrease in patient days in most of these hospitals caused significant excess capacity. With all the problems now confronting mental healthcare facilities, it may be difficult to focus attention on hospital buildings; however, intelligent planning for the use of these large investments may be part of an overall solution. An awareness of problems facing our facility managers should enhance the planning process and minimize unwanted surprises.

[Effect of parting surfaces on the transmission of airborne organisms at junctions between areas of different hygienic standards].

Parting surfaces between areas having different standards of hygienic requirements, represent naturally the weak points in the hygienic conception of operating theatres. These boundary areas between the operation room on the one hand and the adjoining entrance lock for the staff on the other are of specific interest in this connection. While opening the connecting doors, dust particles may be whirled up due to turbulence, thermic, constructional and other effects, which are difficult to be identified and may then settle down directly on the operating area after reaching there from the entrance

Spaces for smoking in a psychiatric hospital: social capital, resistance to control, and significance for 'therapeutic landscapes'.

This paper reports on research framed by theories of therapeutic landscapes and the ways that the social, physical and symbolic dimensions of landscapes relate to wellbeing and healing. We focus especially on the question of how attributes of therapeutic landscapes are constructed in different ways according to the variable perspectives of individuals and groups. Through an ethnographic case study in a psychiatric hospital in the North of England we explore the perceived significance for wellbeing of 'smoking spaces' (where tobacco smoking is practiced in ways that may, or may not be officiall

The Knights Hospitaller of Rhodes and the Black Death of 1498: a poetic description of the plague.

The study is a presentation of the sole written testimony of the great plague epidemic that struck the island of Rhodes in 1498, at the time when the Order of the Knights of St. John was settled there. The Greek poem "The Thanatikon (i.e., plague) of Rhodes", which was written by Emmanuel Georgillas Limenitis in the late 15th century and recounts the terrible events of the epidemic, was used as a source of information. Among the 644 verses of the poem, elements like the place, time, duration and how the epidemic spread can be identified. Within the historical context of the era, evaluation and

The "Suicide Guard Rail": a minimal structural intervention in hospitals reduces suicide jumps.

BACKGROUND: Jumping from heights is a readily available and lethal method of suicide. This study examined the effectiveness of a minimal structural intervention in preventing suicide jumps at a Swiss general teaching hospital. Following a series of suicide jumps out of the hospital's windows, a metal guard rail was installed at each window of the high-rise building. RESULTS: In the 114 months prior to the installation of the metal guard rail, 10 suicides by jumping out of the hospital's windows occurred among 119,269 inpatients. This figure was significantly reduced to 2 fatal incidents among
