Comparative impact of bacteriophage disinfection on hospital microbiota and multidrug-resistant organisms in critical care units.

BACKGROUNDS: Currently used disinfection methods for multidrug-resistant organisms (MDROs) in intensive care units (ICUs) have limitations. This study evaluated the efficacy of bacteriophage disinfection compared to traditional methods. METHODS: Initially, 30 ICU rooms housing patients with cultured MDROs were included between 2020-2022 (yielding 630 initial swabs). After applying exclusion criteria for unmatched environmental strains, 373 relevant ICU surface samples from 29 rooms were analyzed. Disinfection methods were categorized into three groups: sodium hypochlorite (bleach), bleach wit

Utilizing the biosimulator to analyze the environmental microbiome within the intensive care units of a hospital.

Hospital-acquired infections (HAIs), also known as nosocomial infections, are illnesses contracted during treatment at a healthcare facility and can result in severe or life-threatening complications. HAIs are caused by microorganisms that exhibit resistance to standard antibiotics. HAIs can lead to severe complications, longer stays, and increased mortality, particularly in vulnerable patients. In our previous study, we demonstrated the ability of an engraved Petri dish, referred to as a "biosimulator," to induce adhesion of non-adherent cells and the microbiome. This paper explores the use o

Comparison of antimicrobial resistance in neonatal and adult intensive care units in a tertiary teaching hospital.

Intrahospital variations in antimicrobial profiles may be related to many factors. This study compared causative agents of nosocomial bloodstream infections between a neonatal intensive care unit (NICU) that adopted a ward-tailored antibiotic policy and adult intensive care units (ICUs). Data on organisms from blood cultures obtained from the respective wards between 2005 and 2009 were analyzed. Compared with the adult ICUs, the NICU had a higher frequency of Enterobacteriacae and lower frequencies of typical hospital-acquired pathogens (eg, Klebsiella pneumoniae, 17.4% vs 10.0% [P < .001]; Ac

Predictors and outcome of discharge against medical advice from the psychiatric units of a general hospital.

OBJECTIVE: The study examined predictors of discharge against medical advice (AMA) and outcomes of psychiatric patients with AMA discharges, as measured by poorer symptom ratings at discharge and higher rates of rehospitalization. METHODS: A total of 195 patients discharged AMA from general hospital psychiatric units were compared retrospectively with 2,230 regularly discharged patients. AMA status was defined as signing out against medical advice, being absent without leave, or being administratively discharged. All patients received standardized assessments within 24 hours of admission and

[Adenoviruses from stool samples in hospital units. Comparison with main pathogens in gastroenteritis (rotavirus, Campylobacter, Salmonella)].

During a six-years period (1988-1993), a total of 14,644 stool samples from in-patients of Limoges University Hospital were examined for the presence of principal enteric pathogens, such as adenovirus, rotavirus, Campylobacter, Salmonella, Shigella and others. Stools were processed for identification of bacteria by standard methods and viruses were detected in fecal specimens using antigen detection methods: ELISA (Enzyme Linked Immunosorbent Assay) and latex agglutination test. The decreasing rates of presence of enteric agents were respectively 6% for rotavirus, 3.2% for Salmonella, 2% for a

An analysis of community and hospital-acquired bacteraemia in a large teaching hospital in the United Kingdom.

A total of 875 episodes of bacteraemia and fungaemia were analysed in patients admitted to University Hospital, Nottingham, and three smaller hospitals over a four-year period. In 814 episodes (93 per cent) a single organism was isolated, most commonly Escherichia coli (27.4 per cent), other enterobacteria (14.4 per cent), Staphylococcus aureus (11.2 per cent), Streptococcus pneumoniae (9.0 per cent), or Haemophilus influenzae (6.4 per cent). In 61 cases (7.0 per cent) bacteraemia was due to two or more species. In almost 60 per cent of cases, bacteraemia was considered to be community-acquire

Changing Hospital Care For Older Adults: The Case for Geriatric Hospitals in the United States.

Hospital care of frail older adults is far from optimal. Although some geriatric models of care have been shown to improve outcomes, the effect size is small and models are difficult to fully implement, sustain and replicate. The two root causes for these shortcomings are competing interests (high revenue generating diseases, procedures and surgeries) and current hospital cultures (for example a culture of safety that emphasizes bed alarms and immobility rather than frequent ambulation). Geriatric hospitals would be hospitals completely dedicated to the care of frail older patients, a group wh

Incidence, features, in-hospital outcomes and predictors of in-hospital mortality associated with toxic megacolon hospitalizations in the United States.

Toxic megacolon (TM) is a potentially fatal condition characterized by non-obstructive colonic dilatation and systemic toxicity. It is most commonly caused by inflammatory bowel disease (IBD). Limited data for TM are available demonstrating incidence, in-hospital outcomes and predictors of mortality. We sought to investigate incidence, characteristics, mortality and predictors of mortality associated with it. Data were obtained from the Healthcare Cost and Utilization Project (HCUP)'s Nationwide Inpatient Sample (NIS) database from January 2010 through December 2014. An analysis was performed

Antipsychotic Prescribing Practice Patterns for Management of Delirium in a Tertiary Care Children's Hospital Intensive Care Units.

OBJECTIVE: Delirium in the setting of critical illness occurs in 15%-60% of pediatric patients and is associated with increased duration of hospital admission. Strategies for symptom management include minimizing exposure to deliriogenic medications, optimizing day-night cycles, and in some cases prescribing antipsychotic medications. Symptom management occurs while simultaneously identifying and treating the underlying etiology. Over a 5-year period, our pediatric intensive care units implemented several unit-based initiatives to prevent and minimize the severity of delirium in critically ill
