One Clinician, Every Conversation: Palliative Care Delivery by Solo APRN Hospitalists in Critical Access Hospitals.

Advanced practice registered nurses (APRN) who practice as solo hospitalists in critical access hospitals are often underrecognized in their role in providing palliative and end-of-life care. Palliative and hospice care specialty services are often not readily available for consultations in a timely manner, especially in highly remote areas of the country. These hospitalists routinely work without an option for palliative care consultations, ethics committees, or structured advance care planning processes. These hospitalists must simultaneously manage admissions, discharges, inpatient deterior

Utility of advance care planning billing codes: a cross-sectional analysis of U.S. hospitalists' documentation.

OBJECTIVE: To establish the validity of Advance Care Planning (ACP) billing codes 99497 and 99498 to measure ACP occurrence in the hospital. RESULTS: As part of a clinical trial testing the effect of a physician-directed intervention on ACP for hospitalized older adults, we conducted a retrospective chart review of physician notes to assess validity of ACP billing codes on Medicare claims. Among 434 inpatient stays managed by 39 hospitalists at 35 hospitals, 85 (19.6%) had chart documentation of ACP, and 349 (80.4%) did not. Of the charts with documented ACP, 49 had a bill for ACP (57.6% sens

Prevalence of Workplace Violence and Effects of De-escalation Training Among Hospitalists: A Pilot Study.

INTRODUCTION: Workplace violence (WPV) is increasing in healthcare and negatively impacts healthcare worker outcomes. De-escalation training for healthcare workers is recommended to reduce WPV from patients and visitors. Hospitalists may be at high risk for WPV, but the magnitude of WPV and the impact of de-escalation training among hospitalists is not known. METHODS: We investigated the baseline prevalence of WPV experienced by 37 hospitalists at a single center. After an in-person de-escalation training, we measured hospitalists' self-reported "Confidence in Coping with Patient Aggression"

Adverse Events and Patient Outcomes Among Hospitalized Children Cared for by General Pediatricians vs Hospitalists.

IMPORTANCE: Pediatric hospital medicine is a relatively new and growing specialty. However, research remains inconclusive on outcomes for inpatients cared for by pediatric hospitalists compared with those cared for by general pediatricians. OBJECTIVE: To analyze outcomes, adverse events (AEs), and types of AEs associated with care provided for pediatric patients by hospitalists vs general pediatricians. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used data from the medical records of a US urban academic children's hospital comprising 1423 hospitalizations between January 1,

A Hospitalist-Run Procedure Service Safely Streamlines Inpatient Intrathecal Chemotherapy.

Background In patients with hematologic malignancies, intrathecal chemotherapy by means of a lumbar puncture (LP) has found a critical role in delivering vital medications to patients with central nervous system (CNS) spread. However, many oncology patients who require this procedure have refractory thrombocytopenia that can lead to delays in intrathecal chemotherapy administration, as well as potentially the excess use of platelet transfusions, which are a limited resource. At our institution, intrathecal chemotherapy is delivered by radiology with a strict minimum number of platelets or a ho

Impact of a hospitalist system on length of stay and cost for children with common conditions.

OBJECTIVE: This study examined mechanisms of efficiency in a managed care hospitalist system on length of stay and total costs for common pediatric conditions. PATIENTS AND METHODS: We conducted a retrospective cohort study (October 1993 to July 1998) of patients in a not-for-profit staff model (HMO 1) and a non-staff-model (HMO 2) managed care organization at a freestanding children's hospital. HMO 1 introduced a hospitalist system for patients in October 1996. Patients were included if they had 1 of 3 common diagnoses: asthma, dehydration, or viral illness. Linear regression models examinin

Health and hospitalistions after discharge in extremely low birth weight infants.

Apart from higher rates of mortality and adverse neurosensory outcome, extremely low birth weight (ELBW, birth weight 500-999 g) children have more hospital readmissions and other health problems in the early years after discharge than do normal birth weight (NBW, birth weight >2499 g) children. Respiratory illnesses, including lower respiratory infections, are the dominant cause for hospital readmission. ELBW survivors with bronchopulmonary dysplasia (BPD) have even more ill-health and hospital readmissions than do ELBW survivors without BPD. ELBW survivors require closer surveillance after d

Implementing a Hospital Internal Medicine Interest Group - Innovative and Personalized Structured Support for Future Hospitalists.

Hospital medicine offers diverse career opportunities, yet formal guidance for residents pursuing hospitalist careers is often lacking. To address this gap, the Hospital Internal Medicine Interest Group (HIMIG) was launched at an academic institution in the Southeastern United States to provide structured mentorship and professional development. Residents expressing interest completed an entry survey to identify career goals and areas of focus, informing a longitudinal curriculum that included education, leadership, research, quality improvement, and networking. Seventeen residents (≈25% of th

Comprehensive Framework of Heart Failure with Preserved Ejection Fraction Management for Hospitalists: A Scoping Review.

Heart failure with preserved ejection fraction (HFpEF) accounts for nearly half of all heart failure cases and remains a major source of morbidity and hospitalization, particularly among older adults with multiple comorbidities. HFpEF is characterized by diastolic dysfunction, ventricular stiffness, and impaired filling, driven by systemic inflammation, endothelial dysfunction, and myocardial fibrosis. Management is complicated by common comorbidities such as hypertension, type 2 diabetes, atrial fibrillation, chronic kidney disease, obesity, and coronary artery disease. Recent advances have e

Role of hospitalists in Japan for heart failure in the elderly: single center retrospective cohort study.

OBJECTIVE: In Japan, the benefits of hospitalist physician-led care after heart failure have not been sufficiently demonstrated. We evaluated quality of care by the general internal medicine hospitalist (GIM-H) system for patients after acute heart failure and compared it with care by cardiologists. METHODS: This retrospective cohort study enrolled adult patients from within a two-year period who were admitted to our institution for heart failure. Primary outcome measures were medico-economic indicators: length of hospital stay and medical costs. Secondary outcomes included readmission within

Surgical Hospitalist Assisted REcovery (SHARe): Preventing Post-Operative Emergency Transfers.

BACKGROUND AND OBJECTIVES: We observed an increase in emergency transfers (ET) among post-operative patients at our health system's largest free-standing children's hospital. We aimed to reduce ET rates by 50%-from 0.21 to 0.11 events per 10 000 post-operative days-on our 72-bed inpatient surgical unit within 1 year. METHODS: An interdisciplinary team applied quality improvement methods to design and implement a hospitalist surgical co-management model for children with medical complexity, Surgical Hospitalist Assisted REcovery (SHARe), along with several unit-level safety interventions. We a

Hospitalist-driven penicillin allergy delabeling at a quaternary care academic medical center.

A penicillin allergy label is not benign and significantly impacts patient care. Penicillin allergy delabeling can be conducted in an efficient, cost-effective, and safe manner by proceeding directly to oral challenge. Through a quality improvement initiative, we developed and implemented a protocol for hospitalist clinicians to delabel penicillin allergies among low-risk hospitalized patients. Risk stratification was determined by PEN-FAST, a validated screening tool. The primary outcome was the total number of patients that underwent delabeling within 1 year. We exceeded our goal of delabeli

Association of Hospitalist Co-management and Patient Outcomes with Patients Hospitalized for Hip Fracture.

INTRODUCTION: Patients admitted for hip fracture surgery may receive care from a team led either by the operating surgeon or a hospitalist. OBJECTIVE: To describe the prevalence of the hospitalist care model for hip fracture admissions and its association with patient outcomes. DESIGN: We conducted a retrospective cohort study of patients admitted in 2018-2019 for fracture of the head and neck of the femur (ICD10 S72.0x-2x). We compared outcomes at hospitals with low versus high use of the hospitalist care model. Our exposure was the hospital-level adoption of hospitalist care, categorized i

The implementation of an innovative just-in-time educational huddle for faculty development of academic hospitalists.

Academic hospitalists supervising residents on inpatient wards face the dual challenge of managing changing patient complexity and learner needs. Resident experience is highly dependent on supervising faculty; however, traditional faculty development often fails to respond to real-time challenges of clinical teaching. To provide just-in-time faculty development, we implemented a weekly educational huddle for hospitalists rotating on resident ward teams using a Community of Practice model. The huddle emphasized peer coaching and discussion of real-time challenges. In this perspective article, w

The geriatric trauma hospitalist service: An analysis of a management strategy for injured older adults.

BACKGROUND: Management of geriatric trauma patients requires balancing chronic comorbidities with acute injuries. We developed a care model in which patients are managed by hospitalists with trauma-centered education and hypothesized that clinical outcomes would be similar to outcomes in patients primarily managed by trauma surgeons. METHODS: This was a retrospective study of trauma patients aged ≥65 from January 2020 to December 2021. Groups were defined by admitting service: trauma surgery service (TSS) or geriatric trauma hospitalist service (GTHS). The primary outcome was in-hospital mort

[Use of pocket-sized ultrasound in internal medicine (hospitalist) practice: Feedback and perspectives].

INTRODUCTION: Point of care ultrasound (POCUS) is routinely used by intensivists and emergency physicians for many years. Its interest is not arguable any more for these specialists, despite the large variety of diseases they care. Hospitalists and internists also should find some interest in POCUS, which convenience and wide range of indications responds well to the variety of their practice. However, it is still not widely used in internal medicine departments. METHODS: We here report our experience of using a pocket-sized ultrasound device in a French internal medicine department.
