Volume Alone Does Not Predict Quality Outcomes in Hospitals Performing Pediatric Cardiac Surgery.

BACKGROUND: Lower institutional volume has been associated with inferior pediatric cardiac surgery outcomes. This study explored the variation in mortality rates among low-, mid-, and high-volume hospitals performing pediatric cardiac surgery in the United States. METHODS: The Kids' Inpatient Database was explored for the years 2016 and 2019. Hospitals performing only off-bypass coarctation and ventricular septal defect repair were omitted. The hospitals were divided into 3 groups by their annual case volume. Multivariable logistic regression models were fit to obtain risk-adjusted in-hospita

Learning curve for open surgical repair of acute type A aortic dissection.

There is scarce evidence about the surgeon learning curve of acute type A aortic dissection surgery and whether the optimal procedure number exists when training a cardiovascular surgeon. A total of 704 patients with acute type A aortic dissection surgery performed by 17 junior surgeons who can identify their first career surgery from January 1, 2005, to December 31, 2018, are included. The surgeon experience volume is defined as the cumulative number of acute type A aortic dissection surgery of the surgeon since January 1, 2005. The primary outcome was in-hospital mortality. The possibility o

Outcomes Associated With Esophageal Perforation Management: Results From a French Nationwide Population-based Cohort Study.

OBJECTIVE: To evaluate outcomes associated with esophageal perforation (EP) management at a national level and determine predictive factors of 90-day mortality (90dM), failure-to-rescue (FTR), and major morbidity (MM, Clavien-Dindo 3-4). BACKGROUND: EP remains a challenging clinical emergency. Previous population-based studies showed rates of 90dM up to 38.8% but were outdated or small-sized. METHODS: Data from patients admitted to hospitals with EP were extracted from the French medico-administrative database (2012-2021). Etiology, management strategies, and short and long-term outcomes wer

Short-term outcome of isolated lateral malleolar fracture treatment is independent of hospital trauma volume or teaching status: a nationwide retrospective cohort study.

INTRODUCTION: In light of current discussions about centralisation and teaching in medicine, we wanted to investigate the differences in in-hospital outcomes after surgical treatment of isolated ankle fractures, taking into account high-volume centres (HVCs) and low-volume centres (LVCs) and teaching procedures. METHODS: A retrospective analysis of malleolar fractures recorded in a National Quality Assurance Database (AQC) from the period 01-01-1998 to 31-12-2018 was carried out. Inclusion criteria were isolated, and operatively treated lateral malleolar fractures (ICD-10 Code S82.6 and corre

Impact of surgical volume and specialist availability on thyroidectomy outcomes in Brazil: an ecological study with a nationwide retrospective analysis of 230,345 cases (2008-2023).

INTRODUCTION: Thyroid disorders affect a significant proportion of the population and often necessitate surgical intervention, particularly in cases of malignancy. In this study, we analyzed thyroidectomy procedures performed within the public health system (SUS) of Brazil from 2008 to 2023, with a focus on factors influencing in-hospital mortality. METHODS: In the retrospective analysis, data on 230,345 thyroidectomies were extracted from SUS records and stratified by hospital volume and state-level distribution of head and neck specialists. Mortality rates were evaluated using non-parametri

Volume-outcome relationship in septal myectomy for hypertrophic obstructive cardiomyopathy.

BACKGROUND: Septal myectomy is the gold standard treatment for refractory hypertrophic obstructive cardiomyopathy. The present study characterized the association of septal myectomy volume and cardiac surgery volume with outcomes after septal myectomy. METHODS: Adults undergoing septal myectomy for hypertrophic obstructive cardiomyopathy were identified in the 2016 to 2019 Nationwide Readmissions Database. Centers were grouped into low-, medium-, and high-volume hospitals by tertiles based on institutional septal myectomy caseload. Overall cardiac surgery volume was similarly assessed. Genera

Balloon Pulmonary Angioplasty for Chronic Thromboembolic Pulmonary Hypertension: A Nationwide Prospective Multicenter Registry in Japan (J-BPA).

BACKGROUND: Over the past decade, balloon pulmonary angioplasty (BPA) for chronic thromboembolic pulmonary hypertension has shown improved outcomes with procedural refinement in expert hospitals with high procedural volume. Whether the outcomes of BPA are reproducible in hospitals with limited procedural volumes remains unknown. The Japan BPA registry was designed to assess the outcomes of contemporary BPA from a nationwide perspective, including hospitals with low treatment volume. METHODS: This prospective multicenter registry enrolled 1202 consecutive patients with chronic thromboembolic p

Association Between Hospital Volume and 30-Day Readmissions Following Transcatheter Aortic Valve Replacement.

IMPORTANCE: With the approval of transcatheter aortic valve replacement (TAVR) for patients with severe symptomatic aortic stenosis at intermediate surgical risk, TAVR volume is projected to increase exponentially in the United States. The 30-day readmission rate for TAVR was recently reported at 17.9%. The association between institutional TAVR volume and the 30-day readmission metric has not been examined. OBJECTIVE: To assess the association between hospital TAVR volume and 30-day readmission. DESIGN, SETTING, AND PARTICIPANTS: In this observational study, we used the 2014 Nationwide Read

Thoracic Surgery Volume-Quality Relationship.

This article evaluates the volume-outcome relationship in thoracic surgery. While higher hospital and surgeon volumes have been associated with improved patient outcomes, it remains unclear whether higher volumes lead to better outcomes. Factors such as case mix, surgical judgment and expertise, institutional resources, and referral patterns may explain some, or all, of the observed associations. Mandatory surgeon and hospital volume thresholds overlook the multitude of factors that drive quality, and may have unintended consequences for patients, providers, and the health care system. Impact of interhospital transfer on clinical outcomes and costs of extracorporeal life support.

BACKGROUND: The impact of interhospital transfers for extracorporeal life support have not been studied in large datasets. The present study sought to determine the impact of such patient transfers on survival, complications, and hospitalization costs. METHODS: The 2010 to 2016 database of the National Inpatient Sample was used to identify all adults who underwent extracorporeal life support. Patients were categorized based on whether or not they were transferred to another facility. Trend analysis and multivariable models were used to characterize the impact of inter hospital transfer on in-

Is there a relationship between hospital volume and patient outcomes in gastroschisis repair?

PURPOSE: Given the well-established relationship between surgical volume and outcomes for many surgical procedures, we examined whether the same relationship exists for gastroschisis closure. METHODS: We conducted a retrospective analysis of infants who underwent gastroschisis closure between 1999 and 2007 using a California birth-linked cohort. Hospitals were divided into terciles based on the number of gastroschisis closures performed annually. Using regression techniques, we examined the effects of hospital volume on patient mortality and length of stay while controlling for patient and ho

From routine to rescue: Higher elective aortic surgery volume is associated with improved outcomes after acute type A aortic dissection repair.

BACKGROUND: The association between case volume and outcomes in acute type A aortic dissection (ATAAD) repair is well established; however, the impact of elective proximal aortic surgery volume on emergent ATAAD repair outcomes remains unclear. METHODS: Using the 2017-2020 Nationwide Readmission Database, we identified 22,700 elective proximal aortic repairs and classified hospitals into high-volume (median, 58 cases/year), medium-volume (median, 22 cases/year), and low-volume (median, 5 cases/year) groups. We analyzed 10,793 emergent ATAAD repairs performed at these hospitals. The primary ou

Low-Volume vs High-Volume Centers and Management of Fournier's Gangrene in Washington State.

BACKGROUND: Fournier's gangrene (FG) is a life-threatening infection affecting the perineum and genitals. Complex patient management often necessitates transfer to tertiary centers. We aimed to characterize hospital transfer patterns and assess morbidity among patients with FG in Washington State. STUDY DESIGN: The Washington State Comprehensive Hospital Abstract Reporting System includes claims from all hospital discharges in Washington. We identified patients with FG between 2007 and 2013, based on diagnosis and treatment codes. Analyses were stratified by center volume (low-volume centers

Clinical determinants of hospital mortality in liver failure: a comprehensive analysis of 62,717 patients.

BACKGROUND: Liver failure (LF) is characterised by a loss of the synthetic and metabolic liver function and is associated with a high mortality. Large-scale data on recent developments and hospital mortality of LF in Germany are missing. A systematic analysis and careful interpretation of these datasets could help to optimise outcomes of LF. METHODS: We used standardised hospital discharge data of the Federal Statistical Office to evaluate current trends, hospital mortality and factors associated with an unfavourable course of LF in Germany between 2010 and 2019. RESULTS: A total of 62,717 h
