Pitfalls in the management of newborn cloacas.

Clinicians caring for newborns with persistent cloaca face significant challenges in the newborn period. Avoiding key pitfalls during this time can have dramatic implications. We reviewed the medical records of 361 patients with cloaca operated on at our institution and analyzed sequelae that resulted from incorrect management in the newborn period. Of 361 patients, 282 underwent primary operations at our institution, and 79 patients were referred to us after a failed repair at other institutions. Pitfalls in management during the newborn period included the following: (1) Failure to recognize

Hydrometrocolpos in Kaufman syndrome.

A newborn infant presented with a large abdominal mass, urinary retention and postaxial polyaxial polydactyly. On the basis of intial information from the attending physician, this was at first thought to be a grossly distended bladder, whereas in fact it was a hydrocolpos. The association of hydrometrocolos and postaxial polydactyly is known as Kaufman syndrome.

Persistent urogenital sinus: prenatal diagnosis and pregnancy complications.

Prenatal diagnosis of fetal hydrocolpos caused by persistent urogenital sinus is described. The urethrovaginal communication allowed urine to empty from the bladder into the vagina, which subsequently dilated, causing bilateral hydronephrosis, distention of the fetal abdomen, decreased urinary output, oligohydramnios, and abdominal dystocia at delivery.

[Urinary retention caused by hematocolpos secondary to imperforate hymen].

We report on two adolescent girls with imperforate hymen that had presented clinically as urinary retention. In the second case described herein, hydrocolpos could have resulted from the imperforate hymen and might have caused the congenital hydronephrosis incorrectly diagnosed during the neonatal period and childhood as such.

Clean intermittent catheterization as an initial management strategy provides for adequate preservation of renal function in newborns with persistent cloaca.

INTRODUCTION: Persistent cloaca is a rare, congenital anomaly involving the genital, urinary, and rectal organ systems. While prompt bowel diversion is the standard of care, the optimal method of genitourinary decompression is unclear. Bladder outlet obstruction and hydrometrocolpos are common complications that can lead to obstructive uropathy, abdominal distention, infection, perforation, and acidosis. Proposed management strategies include early surgical diversion (vesicostomy, vaginostomy, ureterostomy, nephrostomy) or clean intermittent catheterization (CIC) of the common channel. We hypo

Prenatal ultrasonographic and magnetic resonance imaging findings of congenital cloacal anomalies associated with meconium peritonitis.

BACKGROUND/PURPOSE: Cloacal malformations are rare anomalies, occurring in females, and in which they demonstrate a single perineal orifice for urethra, vagina, and rectum. Prenatal ultrasonograms (US) of cloacal malformations sometimes show ascites, hydrocolpos, and hydronephrosis. We herein describe the characteristic prenatal US and magnetic resonance imaging (MRI) findings of the cloacal malformations associated with meconium peritonitis. METHODS: The pre- and postnatal records of 11 newborn patients with cloacal malformations, treated in our hospital from 1988 to 2004, were reviewed. All

Vesicovaginal reflux: a case report and literature overview.

Vesicovaginal reflux (VVR) is a functional voiding disorder characterized by the retrograde flow of urine into the vagina during micturition, often presenting with daytime incontinence or postvoid dribbling. It is commonly underrecognized and may be mistaken for anatomical abnormalities such as vesicovaginal fistula or obstructive hydrocolpos, which require surgical intervention. We report a case of an 8-year-old girl who presented with dysuria, urinary urgency, and fever. Transabdominal and transrectal ultrasounds revealed an anechoic area measuring 7.3 × 3.3 × 4.0 cm in the vagina while urin

[McKusick-Kaufman syndrome in a new born girl (author's transl)].

In a newborn, the presence of an abdominal tumor due to hydrocolpos resulting from vaginal atresia, with polydactyly and a congenital heart disease suggest the diagnosis of McKusick-Kaufman syndrome. This syndrome is different from the Ellis Van Creveld syndrome, which also includes polydactyly and congenital heart disease, associated with "chondrodysplasia' and "ectodermodysplasia'.

Vesicovaginal reflux presenting as transient urocolpos-A diagnostic dilemma.

Urocolpos refers to urinary distension of the vagina; it commonly results from vesicovaginal fistula or reflux. In this case report, we present the clinical and radiological aspects of an 18-year-old female presenting with no significant urinary complaints, but with imaging findings of hydrocolpos. This would disappear after voiding. Vesicovaginal reflux resulting in urocolpos is a rarely diagnosed condition, and the radiologist may be mystified by the intermittent nature of the findings. We emphasize the importance of recognizing the entity before proposing surgical treatment.

The obstructed bladder syndrome in the neonate.

Eleven neonates with bladder obstruction, causing retention of urine, and a large, palpable lower abdominal mass were treated at the Yale-New Haven Hospital during a five year period. Intrinsic and extrinsic lesions, including urethral atresia, posterior urethral valves, ectopic ureteroceles, hydrocolpos and a neuroblastoma of the pelvis, were diagnosed by cystography, vaginography, pyelography and barium enema examination or a combination of these reoentgenographic methods. The multiplicity of underlying lesions make the obstructed bladder syndrome in the neonate a unique condition.

Endoscopic ablation of longitudinal vaginal septa in prepubertal girls: a minimally invasive alternative to open resection.

INTRODUCTION: Vaginal septation is a congenital defect seen in patients with persistent cloaca, urogenital sinus and disorders of Müllerian duct aplasia. Rarely, these patients present with symptoms in infancy and childhood with the exception of hydrocolpos. Treatment traditionally consists of surgical excision of the vaginal septum. We present our experience with minimally invasive endoscopic ablation of vaginal septa. MATERIALS AND METHODS: We reviewed retrospectively the management of four patients with vaginal septae: Herlyn-Werner-Wunderlich syndrome (1), cloacal malformations (2), and u

OHVIRA syndrome: Early recognition prevents genitourinary complications.

INTRODUCTION: The obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome is characterized by the triad uterus didelphys, obstructed hemivagina, and ipsilateral renal dysplasia. To make a radiological diagnosis, knowledge of this syndrome is of paramount importance. Early recognition may prevent complications such as unnecessary surgical procedures, endometriosis, and infections, which could adversely affect fertility. CASE REPORT: A 1-day-old female newborn in whom a right-sided cystic kidney abnormality was seen on antenatal ultrasound was admitted with anuria and intralabial

13-year-old female with imperforate hymen.

Imperforate hymen is a rare occurrence in younger women resulting in hydrocolpos and hydrometrocolpos. Though imperforate hymen may be diagnosed in infancy, females ages 9-15 years most often present with the problem. Imperforate hymen usually is a congenital anomaly, but has been reported as a result of sexual abuse. This case report reviews the presentation and treatment of a 13-year-old female with imperforate hymen.

A Multidisciplinary Perspective on the Evaluation and Management of the Müllerian System with Attention to Urologic Considerations in Patients Born with Persistent Cloaca.

PURPOSE OF REVIEW: To perform a detailed review of the current literature on management of the müllerian system with an understanding of urologic considerations in patients with persistent cloaca. RECENT FINDINGS: Recent literature has emphasized the co-occurrence of müllerian and urologic anomalies in children with persistent cloaca emphasizing their shared embryologic origins. Review of imaging approaches has emphasized the difficulty in accurately delineating müllerian anatomy pre-pubertally and the importance of follow-up for management of potential sequelae arising pubertally. Important

Hydrometrocolpos--diagnostic and therapeutic dilemmas.

Congenital distal vaginal obstruction is a rare condition, usually asymptomatic until menarche, thus often diagnosed only during puberty. Hydrometrocolpos is caused by such an obstruction, only when reasonable amounts of fluid are secreted from the mucosal lining of the genital tract. This presentation may be symptomatic and already diagnosed early in life, even during the perinatal period. There are various causes of congenital vaginal obstruction, and according to their etiology, surgical procedures may be either simple or more complicated. Timing of the surgical procedure for opening the ob

Factors predicting the need for vaginal replacement at the time of primary reconstruction of a cloacal malformation.

BACKGROUND: A subset of patients with cloacal malformations requires vaginal replacement during their primary reconstruction, increasing the surgical complexity. Identifying factors which predict the need for vaginal replacement would facilitate operative planning. METHODS: We retrospectively reviewed patients who underwent primary cloacal reconstruction at our Center (2014-2018) and assessed the length of the common channel, urethra, and vagina. The presence of hydrocolpos at birth, Müllerian anomalies, sacral ratio, and tethered cord were also assessed between patients who did and did not r

Prenatal diagnosis of imperforate hymen with hydrometrocolpos.

Imperforate hymen is one of the most simple and most common anomalies in the female genital organs, occurring in 0.1% of girls born at term. Some are recognized because of mucocolpos at birth, but the diagnosis is usually not detected before puberty. Rarely, obstetric sonography may prenatally detect imperforate hymen due to presentation of hydrocolpos or hydrometrocolpos in the fetus if fetal cervical and vaginal secretions accumulate in response to circulating maternal estrogens. Nonetheless, reports of prenatal ultrasound diagnosis of hydrometrocolpos are still very rare. In this article we

Female reproductive tract obstruction and its imaging mimics - A pictorial review.

Obstruction of the female reproductive tract can occur in multiple locations. This can result in hydrocolpos at the level of the vagina, hydrometra at the level of the uterus, and hydrosalpinx at the level of the uterine tubes, each with their own underlying causes. Identification of relevant anatomy helps localize the obstruction, which facilitates diagnosis and appropriate clinical management. Cross-sectional techniques, especially MRI can be useful for anatomic localization in difficult cases. Here, we review the various causes of female reproductive tract obstruction, their pathophysiology
