INTRODUCTION/BACKGROUND: Cerebral air embolism (CAE) is a rare but potentially fatal complication, characterized by the entry of air bubbles into the arterial circulation, subsequently obstructing cerebral blood flow and causing ischemic injury. While CAE is a recognized risk in cardiovascular surgery, barotrauma, trauma of the chest or head, it remains an extremely rare complication of pleural procedures, which is even more exceptional when occurring during chest tube placement. To our knowledge, we report the first case of CAE induced by insertion of a small-bore (12 Fr) chest tube via a saf

INTRODUCTION AND IMPORTANCE: The rupture of the pulmonary hydatid cyst is a serious clinical problem because it causes significant local and systemic symptoms. Also, cyst rupture is not rare, but it is one of the most frightening and severe complications of hydatid cysts in the lung.
CASE PRESENTATION: We report a 16-year-old male with a 15-x-15-cm cyst in the lower lobe of his left lung that had ruptured into the pleural space and bronchus, resulting in a fistula. The germinal layer was discovered within the fluid of the effusion inside the pleural cavity. The cyst was surgically excised and

INTRODUCTION: We report a case of Lemierre's syndrome.
CLINICAL PICTURE: A previously healthy 36-year-old woman presented with a 2- to 3-month history of fever, cough, dyspnoea and sore throat, which had worsened in the week prior to presentation. Computed tomography of the thorax showed multiple bilateral cavitating lesions and a right-sided hydropneumothorax with mediastinal shift. Blood cultures grew Fusobacterium and Bacteroides species.
TREATMENT: Broad-spectrum antibiotics were commenced, a chest drain was inserted, and the patient was transferred to the intensive care unit due to wors

Currently, lung ultrasound (LUS) is widely used worldwide for the diagnosing conditions such as neonatal pneumothorax (PTX) and pleural effusion, which are both common conditions during the neonatal period. However, the coexistence or simultaneous occurrence of both conditions, namely, hydropneumothorax, is relatively rare in clinical practice. Ultrasound enables a reliable diagnosis and can be used to guide and monitor appropriate treatment. In this case report, we describe a new ultrasound sign, the air-fluid point, which we propose as a specific sign for the diagnosis of neonatal hydropneum

Candida auris is an emerging multidrug-resistant fungal pathogen that has become a significant global health concern, particularly in critically ill patients within hospital settings. It is known for its high mortality rates, diagnostic challenges, and frequent misidentification, which delays appropriate treatment. We present a case of a 72-year-old male with diabetes and hypertension who initially presented with a persistent cough, hemoptysis, and fever and was initially suspected of having pulmonary tuberculosis. Despite tests negative for tuberculosis, empirical anti-tubercular treatment an

Malignant pleural mesothelioma (MPM) is an aggressive pleural malignancy typically associated with asbestos exposure and characterized by a prolonged latency period, often resulting in delayed diagnosis. Although pleural effusion and pleural thickening are common presentations, recurrent hydropneumothorax is an uncommon initial manifestation. Given the broad differential diagnosis of hydropneumothorax, recurrent cases require a thorough clinical and occupational history and appropriate diagnostic evaluation to avoid overlooking an underlying MPM. This case report describes an uncommon initial

The British Thoracic Society guidelines recommend a surgical opinion in cases of prolonged air leak or failure of lung re-expansion after 3-5 days. The management of prolonged air leak in non-surgical candidates often proves to be more challenging, with no expert consensus guidelines on treatment options. There is a paucity of data for the treatment of patients with a prolonged air leak in the setting of a hydropneumothorax, who are not suitable surgical candidates. We present the case of a novel treatment approach for an 89-year-old male with a symptomatic, large malignant pleural effusion tr

Hydropneumothorax is an uncommon but serious complication of pulmonary tuberculosis (PTB), often resulting from the rupture of a tuberculous cavity into the pleural space with bronchopleural fistula formation. Early diagnosis and optimal management are crucial to improving outcomes. A descriptive study was conducted over 18 months in 50 microbiologically confirmed PTB patients presenting with pneumothorax or hydropneumothorax at a tertiary care hospital in Punjab, India. Demographic, clinical, radiological, and microbiological data were recorded. Lung expansion was quantified using Light's Ind

Secondary bacterial infection and septic pulmonary embolism should be considered in patients recovering from dengue fever who develop focal soft-tissue infection, respiratory symptoms, or markedly elevated inflammatory markers. Characteristic radiological findings may establish the diagnosis even when blood cultures are negative and infective endocarditis is absent.

OBJECTIVE: This study was designed to explore the diagnostic value of multi-slice spiral CT (MSCT) scanning in lung compression ratio of patients with pulmonary contusion complicated by pneumothorax or hydropneumothorax.
METHODS: Seventy-eight patients with pulmonary contusions complicated by pneumothorax or hydropneumothorax treated in the Department of Emergency Surgery of our hospital were examined by CT and X-ray, and the diagnostic value of these two methods was observed. The correlation of lung compression ratio measured by multiplanar reconstruction (MPR) and volume rendering (VR) with

Non-expandable lungs are usually diagnosed after a pleural intervention. It can be challenging to differentiate between an iatrogenic pneumothorax and a new diagnosis of non-expandable lungs following a pleural intervention. The correct assessment can save the patient from undergoing the insertion of an unnecessary intercostal chest drain, which often leads to catastrophe. Suspicion and early evaluation remain the keys, particularly in patients with chronic effusion. Often the diagnosis is reached through a combination of history, pleural fluid analysis, and radiological features such as the a

BACKGROUND: Methicillin-susceptible Staphylococcus aureus (MSSA) can cause rapidly destructive pneumonia complicated by lung abscess, bacteremia, empyema, and hydropneumothorax. Extracorporeal membrane oxygenation (ECMO) may be considered for refractory hypoxemia but is not always feasible. We report a young adult with severe MSSA pneumonia who recovered with optimized conventional support without ECMO.
CASE PRESENTATION: A 36-year-old man presented with fever, melena, hemoptysis, and progressive dyspnea. He developed severe hypoxemic respiratory failure with a PaO2/FiO2 ratio of 75 mmHg, sep

INTRODUCTION: Late-onset congenital diaphragmatic hernia is a rare anomaly with misleading symptoms and signs.
CASE PRESENTATION: We describe the case of a 7-year-old Middle Eastern girl who presented with acute nonspecific abdominal symptoms and respiratory distress of 2 days' duration after sustaining a blunt trauma on her left chest wall on a background of chronic ill-defined left chest pain of 2 weeks' duration. Her initial chest radiograph showed an air-fluid level, which was thought to be a hydropneumothorax, so a chest tube was inserted and was shown to be positioned between the chest

INTRODUCTION: Diaphragmatic hernia (DH) is characterized by the protrusion of abdominal organs into the thoracic cavity through a diaphragmatic defect. Acquired DH typically results from trauma, prior surgery, or increased intra-abdominal pressure. Diagnosing DH complicated by hydropneumothorax is challenging due to atypical initial symptoms, often leading to misdiagnosis as primary pulmonary conditions.
CASE DESCRIPTION: A 60-year-old male presented to the emergency department with persistent respiratory distress, dyspnea, and an occasional cough. He had a surgical history of left hepatic lo

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is an opportunistic infection that predominantly affects immunocompromised individuals, most commonly HIV-infected patients with significantly reduced CD4+lymphocyte counts, and is associated with high clinical mortality. Currently, there are few reports of pneumothorax secondary to PJP, and most cases occur in HIV-infected populations. However, PJP complicated by hydropneumothorax in cancer patients receiving immunotherapy is exceedingly rare, with limited reports in the literature. To our knowledge, this article reports a rare clinical case

Recognizing pneumothorax and hydropneumothorax on chest x-rays of supine ICU patients requires attention to areas other than the lung apex. Pleural air in a supine patient collects in the anterior costophrenic sulcus, producing hyperlucency over the upper abdominal quadrants, and the deep costophrenic sulcus sign. Hydropneumothorax may be recognized when a sharp pleural line is bordered by increased opacity within the pleural space. The presence of pneumothorax and hydropneumothorax can be confirmed by decubitus or upright chest x-rays.

COVID-19 pneumonia causes several complications that include pneumothorax, hydropneumothorax, empyema, and rarely leads to bronchopleural fistula (BPF). BPF is a communication between the pleural space and the bronchial tree. We report a case of 24 years man with pneumothorax, hydropneumothorax, and BPF that appeared after COVID-19 infection.

UNLABELLED: Although the current medical literature is limited, hydropneumothorax was described as far back as the 5th century BC. It is characterized by the presence of air and fluid in the pleural cavity and is an infrequent finding. Causes include trauma, iatrogenesis following thoracentesis, the presence of gas-forming organisms, tuberculosis and malignancy. Diagnosis is based on clinical and radiological features. We report a case of hydropneumothorax and present radiological images showing the distinctive features of this entity.
LEARNING POINTS: Hydropneumothorax is defined as the pres

INTRODUCTION: Pneumothorax is a common complication of medical procedures that often occurs after thoracentesis despite the use of ultrasound guidance. Point-of-care lung ultrasound (POCUS) is a rapid and efficient diagnostic tool for pneumothorax.
CASE PRESENTATION: A 78-year-old man with severe ischemic cardiomyopathy was admitted with acute pulmonary edema and hypotension. He was managed with diuretics and inotropes. Due to severe left pleural effusion, a diagnostic left thoracentesis was performed, and the pleural fluid analysis revealed it was transudative. The follow-up chest X-ray reve
