Portopulmonary hypertension.

Portopulmonary hypertension (PPHT) is a respiratory complication of portal hypertension, defined as an increase in mean pulmonary artery pressure (PAP) of > 25 mmHg with an increase in pulmonary vascular resistance of > 240 dyn.s/cm(-5) and a normal pulmonary capillary wedge pressure ( < 15 mmHg), which often occurs in subjects with liver cirrhosis. Histopathological features of PPHT are endothelial and smooth-muscle cell proliferation and fibrosis leading to luminal obstruction in the resistance arteries. The pathogenesis of PPHT may result from an imbalance between vasoconstrictor and vasodi

Mineralocorticoid hypertension.

Hypertension affects about 10 - 25% of the population and is an important risk factor for cardiovascular and renal disease. The renin-angiotensin system is frequently implicated in the pathophysiology of hypertension, be it primary or secondary. The prevalence of primary aldosteronism increases with the severity of hypertension, from 2% in patients with grade 1 hypertension to 20% among resistant hypertensives. Mineralcorticoid hypertension includes a spectrum of disorders ranging from renin-producing pathologies (renin-secreting tumors, malignant hypertension, coarctation of aorta), aldostero

Neonatal hypertension.

Blood pressure (BP) in neonates depends on a variety of factors, including gestational age, postnatal age and birth weight. The incidence of hypertension in neonates ranges from 0.2 to 3%, and may be higher in premature infants and those who have undergone umbilical arterial catheterization, or who have chronic lung disease. A careful diagnostic evaluation should lead to determination of the underlying cause of hypertension in most infants. Treatment decisions should be tailored to the severity of the hypertension, and may include intravenous and/or oral therapy. Hypertension will resolve in m

Hypertension crisis.

Hypertensive crises (76% urgencies, 24% emergencies) represented more than one fourth of all medical urgencies/emergencies. Hypertensive urgencies frequently present with headache (22%), epistaxis (17%), faintness, and psychomotor agitation (10%) and hypertensive emergencies frequently present with chest pain (27%), dyspnea (22%) and neurological deficit (21%). Types of end-organ damage associated with hypertensive emergencies include cerebral infarction (24%), acute pulmonary edema (23%) and hypertensive encephalopathy (16%), as well as cerebral hemorrhage (4.5%). Portal hypertension.

PURPOSE OF REVIEW: Portal hypertension is the most common complication of cirrhosis accounting for significant morbidity and mortality mainly because of variceal hemorrhage, ascites, bacterial infections, hepatic encephalopathy, and hepatorenal syndrome. Advances in the diagnosis and management of portal hypertension over the last year are reviewed. RECENT FINDINGS: The measurement of the hepatic venous pressure gradient provides important prognostic information in patients with portal hypertension. Noninvasive testing with transient elastography, capsule endoscopy, and computed tomography sc

[Resistant hypertension].

Resistant hypertension, defined as a persistent blood pressure over 140/90 mmHg despite the use of three antihypertensive drugs including a diuretic, is unusual. The diagnosis requires ruling out initially pseudoresistance and a lack of compliance with treatment. Ambulatory blood pressure recording allow the recognition of white coat hypertension. When there is a clinical or laboratory suspicion, secondary causes of hypertension should be discarded. Excessive salt intake, the presence of concomitant diseases such as diabetes mellitus, chronic renal disease, obesity, and psychiatric conditions

PURPOSE OF REVIEW: Report on significant advances in the pathophysiology, diagnosis, and management of the complications of portal hypertension that have occurred in the last year. RECENT FINDINGS: The specific areas reviewed refer to experimental studies aimed at modifying the factors that lead to portal hypertension (increased intrahepatic vascular resistance and splanchnic vasodilatation) and recent advances in the diagnosis and management of the complications of portal hypertension. The specific complications reviewed in this paper are varices and variceal bleeding (primary prophylaxis, t

Resistant hypertension.

The incidence of resistant hypertension remains to be clarified. In this article, three categories of resistance are distinguished: resistant patient, resistant clinician and refractory hypertension. Inadequate compliance, which in case of antihypertensive treatment means taking fewer doses of medications than prescribed, remains a significant cause of poor blood pressure control. Among most frequent physician-related causes of resistant hypertension, there are measurement errors, pseudohypertension, white coat hypertension and therapy errors. Careful elimination of patient- and physician-rela

Pulmonary hypertension.

The modern era in cardiopulmonary medicine began in the 1940s, when Cournand and Richards pioneered right-heart catheterization. Until that time, no direct measurement of central vascular pressure had been performed in humans. Right-heart catheterization ignited an explosion of insights into function and dysfunction of the pulmonary circulation, cardiac performance, ventilation-perfusion relationships, lung-heart interactions, valvular function, and congenital heart disease. It marked the beginnings of angiocardiography with its diagnostic implications for diseases of the left heart and periph

Adrenocortical hypertension.

Primary aldosteronism, congenital adrenal hyperplasia, Cushing's syndrome, glucocorticoid-remediable aldosteronism, and corticotropin-dependent forms of adrenal pathology can cause hypertension by excessive production of adrenocortical hormones. Although traditional biochemical assays continue to be used, genetic testing has simplified the diagnosis of glucocorticoid-remediable aldosteronism. Also, new interventional radiologic approaches for the diagnosis and treatment of corticotropin-dependent forms of Cushing's syndrome are available. Medical and surgical approaches, however, still remain

[Monogenic hypertension].

The results of many studies performed on animals and humans strongly suggest that genetic factors lead to the development of hypertension (HT). Syndromes in which mutations in single genes are sufficient to result in large changes of blood pressure are rare. Nevertheless, it is anticipated that their understanding will lead to new insights into forms of hypertension occurring more often, including essential hypertension. At least 9 monogenic forms of HT including Liddle syndrome, type I familial hyperaldosteronism (GRA) and type II familial hyperladosteronism, Gordon syndrome, apparent mineral

Portal hypertension, the main complication of cirrhosis, is responsible for its most common complications: variceal hemorrhage, ascites, and portosystemic encephalopathy. Portal hypertension is the result of increased intrahepatic resistance and increased portal venous inflow. Vasodilatation (splanchnic and systemic) and the hyperdynamic circulation are hemodynamic abnormalities typical of cirrhosis and portal hypertension. Gastroesophageal varices result almost solely from portal hypertension, although the hyperdynamic circulation contributes to variceal growth and hemorrhage. Ascites results

Pulmonary hypertension.

Pulmonary arterial hypertension (PAH) must be classified into primary pulmonary hypertension and PAH related to other diseases such as collagen vascular diseases, HIV infection or portal hypertension. PAH must also be differentiated from other entities, in particular pulmonary hypertension secondary to thromboembolic diseases, requiring specific approaches. All PAH results in similar histological remodelling of pulmonary arteries, with thickening of the intima, proliferation of the media and plexogenic lesions. Today the physiopathology of these lesions is much better understood and has result

Renovascular hypertension.

This article discusses the pathophysiologic features, diagnosis, treatment, and management of patients with renovascular hypertension. Timely diagnosis is important as the disease can carry a more serious prognosis than essential hypertension and is less amenable to drug treatment. Renovascular hypertension also carries a greater risk of progression to accelerated or malignant hypertension and may result in irreversible ischemic failure of the affected kidney. Various treatments are available including medical management, angioplasty and/or stenting, and revascularization. Regardless of treatm
