The study aimed to investigate the association of plasma aldosterone concentration (PAC) on admission before delivery with preeclampsia, admission blood pressure, and pregnancy outcomes. In this multicenter observational study (May 2023-December 2024), 886 pregnancies (125 preeclampsia, 761 controls) from six hospitals were analyzed. Associations were evaluated using multivariable logistic regression, Spearman's correlation, and restricted cubic spline models, with sensitivity analysis employing Winsorization. Piecewise regression was employed to identify breakpoint and examine threshold effec

Hyperkalemia is developed in a part of patients with aldosterone-producing adenoma (APA) after adrenalectomy, suspected to be due to the insufficiency of aldosterone secretion. The purpose of this study is to determine the frequency and characteristics of prolonged postoperative hypoaldosteronism (PPHA) using chemiluminescent enzyme immunoassay (CLEIA). We studied 58 patients with APA with long time after adrenalectomy and whose PAC was measured using a CLEIA kit. The PAC value measured using CLEIA was significantly lower than that of using RIA between two consecutive visits before and after t

A 13-year-old spayed female Schnauzer dog with chronic kidney disease (CKD; International Renal Interest Society stage 2, non-proteinuric, normotensive), diabetes mellitus, hypercortisolism and myxomatous mitral valve degeneration (American College of Veterinary Internal Medicine stage B2) presented with electrolyte imbalance that had progressed to hyperkalaemia and hyponatremia, with a sodium to potassium (Na:K) ratio of 19.6. Cortisol levels after the adrenocorticotropic hormone stimulation test were within the therapeutic range, but aldosterone levels were below the reference range; hence,

We report 15 years follow-up of a girl born with nails hypothrophy and primary hypoaldosteronism. The failure to thrive, troubles with feeding and vomiting were observed since the first weeks of life. The results of the laboratory examination confirmed isolated hypoaldosteronism. The growth velocity was diminished and short stature was present during first 4 years of life. Thyroid and pituitary disturbances were excluded. At the age of 4 the treatment with fludrocortisone was gradually discontinued without clinical manifestation of aldosteron deficiency. The child was on high sodium chloride d

Diabetes mellitus is a common cause of hyporeninaemic hypoaldosteronism that might result in significant hyperkalaemia. We describe a patient with diabetic nephropathy and proteinuria who developed a remarkable hyperkalaemia on treatment with an angiotensin-receptor blocker. The management of hyperkalaemia and the pathophysiological background of hyporeninaemic hypoaldosteronism are discussed.

Nephrogenic syndrome of inappropriate antidiuresis (NSIAD) is an X-linked disease caused by activating mutations in the arginine vasopressin (AVP) receptor-2 (AVPR2) gene. Affected patients excrete concentrated urine despite very low levels of AVP, and consequently develop euvolemic hyponatremia. Due to its low frequency, patients may be misdiagnosed and treated incorrectly. We report two related male infants with NSIAD that was initially confused with hyporeninemic hypoaldosteronism (HH). First, a 2-month-old male presented with hyponatremia, low plasma osmolality, relatively high urine osmol

An obese man suffered cardiac arrest twenty minutes after receiving epidural anesthesia for incision and debridement of wound over the right leg. The patient's condition stabilized after emergent cardiopulmonary resuscitation. It was found that the patient had been self-administering an herbal drug continuously for a year and a half, and that this drug contained ethoxybenzamide, which is a nonsteroidal anti-inflammatory drug (NSAID). Low plasma renin and aldosterone levels were noted from the blood sample taken at the time of the cardiac arrest. The cardiac arrest was believed to be related to

A 70-year-old woman was admitted to the intensive coronary care unit with suspected myocardial infarction. During the follow-up period, the patient twice developed left bundle branch block, which was shown to be related to high serum potassium levels secondary to hyporeninemic hypoaldosteronism and moderate renal failure.

SUMMARY: Primary aldosteronism is one of the most common (affecting up to 10%) yet treatable causes of hypertension in our community, notable due to an associated elevated risk of atrial fibrillation, stroke and myocardial infarction compared to essential hypertension. Guidelines have focussed on improving case detection due to significant underdiagnosis in the community. While our case experienced significant delay in diagnosis, we highlight a state of protracted, persistent post-operative hypoaldosteronism which manifested with severe hyponatraemia and hyperkalaemia, necessitating long-term

A 54-year-old male had monoclonal IgM-kappa light chains in the serum and free monoclonal kappa light chains in the urine. Renal biopsy revealed nodular glomerulosclerosis associated with the accumulation of kappa light chains. Isolated microscopic hematuria was present for over 1 year. He also showed a defect in urine concentration for which the light chains deposited along the basement membrane in the inner medullary tubules were judged to be responsible. When the glomerular filtration rate fell to 30 ml/min, the syndrome of mild renal failure, decreased potassium excretion, and hyporeninemi

Biochemical investigations in a boy with a salt losing syndrome revealed a very low secretion of aldosterone which did not rise during salt deprivation, in spite of a normal rise in plasma renin activity. Cortisol secretion was normal - but subsequently decreased, while the corticosterone secretion was high. The patient was studied at the age of 5 weeks, 3 months and also at the age of 8 months. He survived until the age of 18 months on treatment with sodium chloride and DOCA, but did not receive glucocorticoids. At autopsy the adrenal glands were absent, but in fat tissue from the upper renal

Persistent hypoaldosteronism post-adrenalectomy for unilateral primary aldosteronism is not uncommon and should be anticipated in patients with risk factors for development of such condition. Most cases of hypoaldosteronism postadrenalectomy are transient. However, persistent hypoaldosteronism may occur as a result of delayed recovery of contralateral zona glomerulosa suppression, requiring mineralocorticoid replacement for the prevention or treatment of life-threatening hyperkalemia.

offlated hypoaldosteronism with or without hyperkalemia in patients with diabetes mellitus has been shown to exist occasionally without hyporeninemia. To assess in detail the adrenal function in this disorder, the responses of plasma aldosterone (PA) and its precursor steroids to angiotensin II (AII) infusion and adrenocorticotropic hormone (ACTH) injection were studied in seven patients with asymptomatic normoreninemic hypoaldosteronism (ANH) and 11 age-matched normal subjects. The ANH diabetic patients had, by definition, a low PA level after furosemide (80 mg orally) plus upright posture (4

In order to find a biochemical marker to assist the physician in the difficult differential diagnosis between malignant and nonmalignant adrenal tumors, plasma levels of the mineralocorticosteroids (deoxycorticosterone, 18-hydroxydeoxycorticosterone, corticosterone, 18-hydroxycorticosterone, and aldosterone) were determined. The same method (RIA which is preceded by a crucial separation step) was used to measure all these steroids including aldosterone. The subjects included 15 adults presenting various clinical signs of adrenocortical tumors (histopathologically: 6 with adrenal carcinoma, 1 w

A 24-yr-old woman with hypertension, hypokalemic alkalosis, low plasma renin and hypoaldosteronism was studied. Plasma aldosterone, renin and potassium returned to normal and blood pressure fell after sodium restriction or the administration of triamterene. Thiazide therapy also normalized her blood pressure while dexamethasone, spironolactone and furosemide did not improve her symptoms. Plasma aldosterone levels were low and responded poorly to a short term ACTH injection, but responded well to the maximal adrenal stimulation by ACTH-Z. Plasma levels of cortisol, corticosterone and deoxycorti

Intravenous infusion of hypertonic glucose induced a transitory paradoxical serum potassium elevation in 3 nondiabetic patients with selective hypoaldosteronism. The abnormal serum potassium response developed in spite of a normal insulin release and was abolished by pharmacological doses of mineralocorticoids in two. It is apparent that in certain patients with aldosterone deficiency normal insulin production alone cannot reverse completely the hyperosmolality-induced transfer of potassium from the intracellular to the extracellular fluid.

The renin-aldosterone system and tubular responsiveness to exogenous mineralocorticoid administration was evaluated in 2 hyperkalemic patients with obstructive uropathy. Both patients manifested marked renal sodium wasting and a modest inability to lower urine pH despite systemic acidosis. None of the abnormalities was corrected with supraphysiologic doses of mineralocorticoid. In addition, plasma renin and aldosterone levels were inappropriately low. Thus, both hypoaldosteronism and renal tubular resistance to mineralocorticoids coexisted in these patients. It is likely that an association be
