Hypotonic and isotonic fluid overload as a complication of hysteroscopic procedures: two case reports.

Hysteroscopy is used extensively for both the diagnosis and treatment of intrauterine pathology. Although considered a safe procedure, complications such as cervical laceration, uterine perforation, absorption of irrigation solutions and, rarely, gas or air embolism may occur. The authors present two cases with successful outcomes, whereby complications following hysteroscopy occurred due to excessive fluid absorption. Hypotonic as well as isotonic distention media can cause serious complications. The authors focus on factors that may increase the risk of fluid overload, which should be known

Swelling- and cAMP-activated Cl- currents in isolated rat carotid body type I cells.

1. In the whole-cell configuration of the patch clamp technique, isolated rat carotid body type I cells exhibited reversible activation of Cl- currents during cell swelling effected by hypotonic extracellular solutions. 2. Hypotonic solutions evoked outwardly rectifying, non-inactivating currents which showed time-independent activation. The reversal potential (E(rev)) for the hypotonically evoked current was 1.6 +/- 0.6 mV (n = 26). Reduction of extracellular Cl- from 133 to 65.5 mM caused a shift in E(rev) of +14.7 +/- 0.4 mV (n = 5). 3. The swelling-activated Cl- current could not activate

A case of hyperglycemic hyperosmolar non-ketotic coma during anesthesia: a possible cause of failed re-awakening.

The case of a non diabetic 6-year-old boy affected by Down's syndrome, who developed hyperosmolar hyperglycemic non-ketotic coma following the infusion of hypertonic dextrose solution during general anesthesia for a surgical procedure for cryptorchidism is reported. Following surgery, the patient remained deeply comatose and generalized seizures occurred. Hyperosmolarity due to hyperglycemia and acidosis were reduced by administration of insulin at low rate, hypotonic saline and sodium-bicarbonate solutions. The patient's clinical conditions promptly improved following normalization of blood g

Inhibitory motor neurons of the esophageal myenteric plexus are mechanosensitive.

Mechanosensitivity of enteric neurons has been reported in the small intestine and colon, but not in the esophagus. Our earlier in vivo studies show that mechanical stretch of the esophagus in the axial direction induces neurally mediated relaxation of the lower esophageal sphincter, possibly through mechanosensitive motor neurons. However, this novel notion that the motor neurons are mechanosensitive has not been examined in isolated esophageal myenteric motor neurons. The goal of our present study was to examine the mechanosensitivity of esophageal motor neurons in primary culture and elucid

TRPM7 Is Involved in Volume Regulation in Salivary Glands.

Under hypotonic conditions, the regulatory volume decrease (RVD) is essential to maintain physiological homeostasis and functions in diverse biological systems. Intracellular Ca2+ has been reported as an important mediator of this response, but the underlying Ca2+ mechanism responsible for RVD is still controversial. Here we investigate the role of Ca2+ in the RVD response using live-cell imaging, microspectrofluorimetry, and a patch-clamp technique. A typical RVD was observed in submandibular gland acinar cells after swelling in a hypotonic solution, whereas intracellular Ca2+ chelation compl

Naturally occurring interference in Luminex assays for HLA-specific antibodies: characteristics and resolution.

Substances occurring naturally in the sera of patients can interfere with Luminex antibody assays, causing increased background and changes in antibody specificity. We present data on the effectiveness of hypotonic dialysis (HD) or dithiothreitol (DTT) treatment in eliminating this interference. HD significantly increased reaction strength of positive control beads and reduced reaction strength of negative control beads. HD also improved specificity identification, determination of donor-specific antibody (DSA) strength, and crossmatch predictability compared with values in untreated serum. DT

The effect of pinealectomy on plasma vasopressin response to isotonic, hypertonic and hypovolemic treatments in rats supplemented with L-thyroxine.

The present study was conducted to determine the effects of basal, isotonic as well as hypertonic and hypovolemic treatments on fluid-electrolyte balance and plasma AVP levels in rats supplemented with L-thyroxine and pinealectomized L-thyroxine. The animals were initially separated into 4 groups: control (n = 24), L-thyroxine treated (n = 24); L-thyroxine + sham-pinealectomy (n = 24) treated and 4-L-thyroxine + pinealectomy (n = 24) treated. L-thyroxine was given for 4 weeks. At the end of the 4-week experimental period, the sub-groups were formed before decapitation, which were classified as

Is a low-osmolarity ORS solution more efficacious than standard WHO ORS solution?

The clinical efficacy of a diluted oral rehydration salts (ORS) solution was compared in a pilot study with that of intravenous (i.v.) therapy and of standard World Health Organization (WHO)/United Nations Childrens Fund (UNICEF) ORS solution in children with acute diarrhea. Sixty-one boys aged 3 to 24 months, admitted to hospital with acute diarrhea and signs of dehydration, were randomly assigned to groups receiving standard ORS solution, diluted ORS solution, or i.v. therapy. In children treated with standard ORS solution and small amounts of plain water, the total fluid intake was 25-39% g

Repetitive cell 'jumps' during hypotonic lysis of erythrocytes observed with a simple flow chamber.

This is the first report describing cell movements of a repetitive character during hypotonic lysis of erythrocytes (haemolysis). A new, simply constructed chamber is described for microscopy of freely suspended cells, for example, blood cells, during the inflow of a new medium. Hypotonic haemolysis of individual red blood cells was studied. During the first phase of haemolysis discontinuities were found: the cells made between zero and seven sudden movements or 'jumps', interpreted as caused by an ejection of cytoplasm due to excess intracellular hydraulic pressure and the formation of a hole

An improved DAPI-DNA cytofluorometry by hypotonic cytolysis. Its application to tumor cell heterogeneity.

The effect of cell cytolysis on DAPI-DNA cytofluorometry was studied in order to eliminate nonspecific cytoplasmic hindrances in DAPI staining. A great improvement in the reproducibility of DNA determination was attained when cells were treated with hypotonic KCl (0.075 M) solution. The DNA content per cell of the mgC5 clonal line (normal diploid cell containing 2C DNA units) after hypotonic treatment was 3.07 +/- 0.06, while that of mgC5 cells without hypotonic treatment was 3.50 +/- 0.26, indicating a remarkable decrease from 7.4% to 2.0% in the coefficient of variation. In detecting tumor c

Detergent induced lysis of erythrocytes in kwashiorkor.

The effect of the non-ionic detergent Nonidet P40 on lysis of erythrocytes in children suffering from kwashiorkor was studied. The concentration of the detergent causing 50% haemolysis was significantly reduced in these patients. Detergent haemolysis was more sensitive than osmotic fragility (which was reduced). The abnormality was only slight in marasmic children.

Hyponatremia after pediatric surgery: Randomized trial of fluid composition on antidiuretic hormone response.

BACKGROUND: We examined the underlying mechanisms of whether hyponatremia could be induced by hypotonic solution administration after the postoperative invasive phase (POIP). METHODS: We included patients who had undergone surgery with expected oral feeding resumption after postoperative day (POD) 3. In this open-label, randomized controlled trial, 100 patients were assigned to three groups by sodium concentrations ([Na]) used for maintenance infusions: 136 mEq/L (ISO) (n = 34), 68 mEq/L (HYPO) (n = 33), and 34 mEq/L (exHYPO) (n = 33). Fluid Management in Acute Brain Injury.

PURPOSE OF THE REVIEW: The aims of fluid management in acute brain injury are to preserve or restore physiology and guarantee appropriate tissue perfusion, avoiding potential iatrogenic effects. We reviewed the literature, focusing on the clinical implications of the selected papers. Our purposes were to summarize the principles regulating the distribution of water between the intracellular, interstitial, and plasma compartments in the normal and the injured brain, and to clarify how these principles could guide fluid administration, with special reference to intracranial pressure control. Intraoperative isotonic balanced versus hypotonic crystalloids on postoperative sodium homeostasis in small children undergoing major neurosurgery: a randomized controlled trial.

BACKGROUND: Whether intraoperative isotonic balanced maintenance fluid is associated with less variation in sodium homeostasis in small children undergoing major neurosurgery remains unknown. METHODS: Patients aged up to 6 years undergoing major neurosurgery were randomly assigned to receive either isotonic balanced solution (IB) or 0.2% hypotonic solution (H) as intraoperative maintenance fluid. Serum electrolyte levels were measured from preoperative baseline to 6 d following surgery. RESULTS: Eighty patients were included in the primary analysis. Serum sodium change was significantly less

Interaction between cholinergic and osmolar stimulation of the lateral hypothalamic area (LHA) on sodium and potassium excretion.

Hypotonic and isotonic solutions into the LHA of unrestrained rats caused no alteration in renal water and electrolyte excretion. Similar results were obtained after injecting hypo, iso and hypertonic glucose solutions. On the other hand, a hypertonic NaCl solution produced an increase in natriuresis and kaliuresis immediately after being injected into the LHA. The carbachol elicited an increase in natriuresis and kaliuresis as well as a decrease in urine output in the first urine samples collected after the injection being injected into the LHA. This response was not modified when the same do

Classifying Hyponatremias According to Tonicity Disorder: Hypotonic Hyponatremia, Hypertonic Hyponatremia, and Pseudohyponatremia as Distinct Entities.

Tonicity of a solution refers to its ability to cause cells in it to either swell or shrink through osmotic fluid flux across the cell membranes due to a difference in the total concentrations of osmoles in the solution and in the intracellular space. For serum, the sodium concentration ([Na]S) constitutes the major determinant of tonicity. Three categories of tonicity can be found in cases of hyponatremia: (a) hypotonic hyponatremia created by a relative excess of body water over the sum of exchangeable body sodium plus exchangeable body potassium and causing osmotic fluid entry into cells; (

Improving intravenous fluid therapy in children with gastroenteritis.

Gastroenteritis is one of the most common medical conditions seen by pediatricians. The standard approach to intravenous fluid therapy for these children has been to administer a 0.9% sodium chloride (NaCl) bolus followed by a hypotonic solution ranging from 0.2-0.45% NaCl to replace the remaining deficit plus maintenance. We have questioned the safety of this approach as there have been reports of death or permanent neurologic impairment from hyponatremic encephalopathy. Hanna and Saberi (Pediatr Nephrol. doi: 10.1007/s00467-009-1428-y ) found the incidence of hospital-acquired hyponatremia (

Transient Ca2+ mobilization caused by osmotic shock initiates salmonid fish sperm motility.

Salmonid fish sperm motility is known to be suppressed in millimolar concentrations of extracellular K(+), and dilution of K(+) upon spawning triggers cAMP-dependent signaling for motility initiation. In a previous study, however, we demonstrated that suspending sperm in a 10% glycerol solution and subsequent dilution into a low-osmotic solution induced motility independently of extracellular K(+) and cAMP. In the present study, we further investigated the glycerol-induced motility mechanism. We found that treatment with solutions consisting of organic or inorganic ions, as well as glycerol, i
