Impact of peritoneal dialysis strategy on technique and patient survival.

BACKGROUND: The aim of this study was to evaluate the impact of peritoneal dialysis (PD) strategy on technique and patient survival. METHODS: This was a retrospective, single-center study conducted on consecutive patients with chronic kidney disease who underwent PD between January 2009 and December 2019. The study sample was stratified into four different groups according to PD technique [automated (APD) or manual (CAPD)] and icodextrin use (yes versus no). The primary endpoints were survival of both technique and patient. RESULTS: A total of 531 patients were included in the analysis. Mean

Factors influencing skin autofluorescence of patients with peritoneal dialysis.

Skin autofluorescence (SAF) measurement is a simple, noninvasive method to assess tissue advanced glycation end products (AGE). In patients with end-stage renal disease and in those on hemodialysis AGE production is increased. Less is known about those treated with peritoneal dialysis (PD). In this study we tested if SAF is influenced by clinical and treatment characteristics in PD patients. This cross-sectional study included 198 PD patients (of those, 128 were on traditional glucose-based solutions and 70 patients were partially switched to icodextrin-based PD). SAF measurements were done wit

Eosinophilia in a peritoneal dialysis patient: Answers.

Icodextrin is a starch-derived glucose polymer used in peritoneal dialysis dialysate to treat volume overload by increasing ultrafiltration in patients with end-stage renal disease. Reported adverse reactions to icodextrin are mild and rare and mainly consist of skin rash that resolves spontaneously after discontinuation of treatment. We describe a young patient with extreme eosinophilia that appeared with the use of icodextrin, disappeared after its discontinuation, and reappeared after a rechallenge with the drug. The eosinophilia was not associated with peritonitis, was asymptomatic, and fu

Impact of peritoneal dialysis fluid composition and infusion timing on 18F-fluoro-2-deoxy-2-d-glucose positron emission tomography-computed tomography interpretability in peritoneal dialysis patients.

BackgroundGlucose and insulin blood levels are well known altering factors for 18F-fluoro-2-deoxy-2-d-glucose positron emission tomography-computed tomography ([18F]-FDG PET-CT) interpretability in peritoneal dialysis (PD) patients. A potential influence of glucose-based solutions and Icodextrin in PD patients has not been studied. The aim of this study was to assess PET-CT interpretability in PD patients according to the osmotic agent used in the dialysis fluid (Icodextrin vs. glucose) and the timing of dialysate infusion before imaging.MethodsWe searched for all PET-CT performed on individua

Differential effects of biocompatible peritoneal dialysis fluids on human mesothelial and endothelial cells in 2D and 3D phenotypes.

INTRODUCTION: Peritoneal dialysis (PD) is a life maintaining treatment in patients with end-stage renal disease. Its chronic application leads to peritoneal mesothelial layer denudation and fibrotic transformation along with vascular activation of inflammatory pathways. The impact of different PD fluids (PDF) on mesothelial and endothelial cell function and repair mechanisms are not comprehensively described. MATERIALS AND METHODS: Mesothelial (MeT-5A) and endothelial cells (EA.hy926) were cultured in 1:1 ratio with cell medium and different PDF (icodextrin-based, amino acid-based, and glucos

A case of peritoneal dialysis-associated peritonitis caused by Rhodococcus pyridinivorans.

BACKGROUND: Rhodococcus infections are exceedingly rare and largely reported in immunocompromised hosts, with no consensus on optimal therapy due to limited evidence. CASE PRESENTATION: This article reports for the first time a case of peritoneal dialysis-associated peritonitis caused by Rhodococcus pyridinivorans in a patient with diabetic nephropathy. The dialysate leukocyte count declined substantially with combination antimicrobial therapy, but complete normalization occurred only after replacing icodextrin with a glucose-based dialysate. CONCLUSIONS: These findings support the use of co

Morphological characteristics in peritoneum in patients with neutral peritoneal dialysis solution.

Peritoneal dialysis solution (PDS) plays a role in functional and morphological damage to the peritoneum. This study aimed to clarify the effect of neutral PDS in preventing morphological changes by assessing peritoneal damage and comparing morphological alterations between PD patients treated with neutral PDS and acidic PDS. Sixty-one patients participated from seven hospitals. All patients were treated with neutral PDS excluding icodextrin, during their entire PD treatment, and experienced no episode of peritonitis. The thickness of submesothelial compact (SMC) zone and the presence of vascu

Vascular endothelial growth factor receptor-3 is a novel target to improve net ultrafiltration in methylglyoxal-induced peritoneal injury.

Appropriate fluid balance is important for good clinical outcomes and survival in patients on peritoneal dialysis. We recently reported that lymphangiogenesis associated with fibrosis developed in the peritoneal cavity via the transforming growth factor-β1-vascular endothelial growth factor-C (VEGF-C) pathway. We investigated whether VEGF receptor-3 (VEGFR-3), the receptor for VEGF-C and -D, might be a new target to improve net ultrafiltration by using adenovirus-expressing soluble VEGFR-3 (Adeno-sVEGFR-3) in rodent models of peritoneal injury induced by methylglyoxal (MGO). We demonstrated th

Pregnancy with peritoneal dialysis- a challenge revisited.

BackgroundPregnancy with kidney failure is challenging, even more so with peritoneal dialysis (PD). The pregnancy rate is 50% lower while on PD than on hemodialysis, and usually, patients transition to hemodialysis to support better outcomes. There are only a few reported cases of pregnancy carried beyond 32 weeks' gestation on PD alone.MethodsWe present a patient treated with PD for over two years at the time of natural conception. Higher clearance needs arose during pregnancy, but the patient did not transition to hemodialysis due to fear of hematological infections and need for new dialysis

The mesothelium under the siege of dialysis solutions: old glucose, new glucose, and glucose-free osmotic agents.

Use of one bag of glucose peritoneal dialysis fluid (PDF) results in the development of a dose-related senescent mesothelial cell phenotype not linked to acidic pH or the presence of lactate buffer. This complication derives mostly from oxidative stress induced both by glucose itself and by glucose degradation products (GDPs). New glucose formulations are offered in dual- or three-chambered bags, keeping the glucose at a low pH. The result is a reduced presence but not complete elimination of GDPs. These formulations have the potential to slow injury to the peritoneal membrane. Icodextrin and

Peritoneal Dialysis Therapy Discontinuation Due to Noninfectious Complications: Insights from a Retrospective Study in End Stage Renal Disease Patients from Saudi Arabia.

Noninfectious complications (NCs) disrupt the continuity of peritoneal dialysis (PD) and may result in catheter loss. This retrospective study evaluated the incidence, risk factors, and impact of NCs on PD catheter outcomes in patients aged ≥14 years. Data from 126 PD catheters were analyzed for NCs, including flow dysfunction, catheter leaks, hernias, bloody or chylous effluent, hydrocele, hydrothorax, and external catheter damage. Among 126 catheters, 37 (29.4%) experienced 46 NCs, with outflow obstruction (32.6%), inflow/outflow issues (21.7%), leaks (21.7%), hernia (10.8%), bloody effluent

Osmotic agents hamper mesothelial repopulation as seen in the doughnut in vivo model.

BACKGROUND: The problem of mesothelial cell injury derived from the use of peritoneal dialysis solutions has been explored deeply. Conversely, the eventual detrimental effects upon mesothelial cell regeneration have awaked less investigative efforts than those focused on injury. OBJECTIVE: To evaluate in the in vivo and in situ rat "doughnut" model of mesothelial repopulation, the eventual effect of peritoneal lavage with Hank's Balanced Salt Solution (HBSS) as well as that of 4.25% glucose and 7.5% icodextrin dialysis solutions. EXPERIMENTAL ANIMALS: 100 Sprague-Dawley albino rats were incl

Biocompatible dialysis fluids for peritoneal dialysis.

BACKGROUND: Biocompatible peritoneal dialysis (PD) solutions, including neutral pH, low glucose degradation product (GDP) solutions and icodextrin, have previously been shown to favourably influence some patient-level outcomes, albeit based on generally sub-optimal quality studies. Several additional randomised controlled trials (RCT) evaluating biocompatible solutions in PD patients have been published recently. This is an update of a review first published in 2014. OBJECTIVES: This review aimed to look at the benefits and harms of biocompatible PD solutions in comparison to standard PD solu

Mechanisms of Crystalloid versus Colloid Osmosis across the Peritoneal Membrane.

Background Osmosis drives transcapillary ultrafiltration and water removal in patients treated with peritoneal dialysis. Crystalloid osmosis, typically induced by glucose, relies on dialysate tonicity and occurs through endothelial aquaporin-1 water channels and interendothelial clefts. In contrast, the mechanisms mediating water flow driven by colloidal agents, such as icodextrin, and combinations of osmotic agents have not been evaluated. Methods We used experimental models of peritoneal dialysis in mouse and biophysical studies combined with mathematical modeling to evaluate the mechanisms o
