[Treatment of late stages of anthracic infection].

Ways for increasing the anthrax treatment efficacy by combined use of antibiotics, toxin neutralizing, immunomodulating, symptomatic and pathogenetic agents are considered. The dynamics of the infection development was studied on rabbits by the microbiological, cytochemical, biochemical and cytomorphologic criteria. The efficacy of monoantibacterial and complex schemes of the etiopathogenetic therapy of the disease at the early and late stages (phases) of the infection generalization was estimated.

Atopic dermatitis and fungi.

Atopic dermatitis (AD) is a chronic, itching, inflammatory skin disease which is associated with asthma and/or hay fever and a familial occurrence of these conditions. Genetic factors are important in the development of AD, but the exact hereditary pathway is still unknown. Dry skin and the weakened barrier function in patients with AD is very important for the patient's reactions to irritants and other external trigger factors including microorganisms. The standard treatments are topical corticosteroids, topical immunomodulating agents, and emollients. If AD cannot be controlled by this type

Immunomodulatory therapy in non-infectious Uveitis: Current landscape, gaps, and future directions.

Non-infectious uveitis (NIU) is a potentially sight-threatening intraocular inflammatory condition that may arise idiopathically or in association with systemic immune-mediated diseases. While corticosteroids remain essential for rapid suppression of inflammation, their long-term use is limited by significant systemic and ocular side effects. Thus, immunomodulatory therapy (IMT)-including antimetabolites, calcineurin inhibitors, biologics, and emerging small molecules-has become central to achieving sustained control with a reduced corticosteroid burden in chronic cases. Despite a range of the

Preventing Infectious Complications of Immunomodulation in COVID-19 in Foreign-Born Patients.

Immunomodulating therapies for COVID-19 may carry risks of reactivating latent infections in foreign-born people. We conducted a rapid review of infection-related complications of immunomodulatory therapies for COVID-19. We convened a committee of specialists to formulate a screening and management strategy for latent infections in our setting. Dexamethasone, used in severe COVID-19, is associated with reactivation of latent tuberculosis, hepatitis B, and dissemination/hyperinfection of Strongyloides species and should prompt screening and/ or empiric treatment in appropriate epidemiologic con

Paradoxical adverse drug reactions: descriptive analysis of French reports.

INTRODUCTION: Paradoxical adverse drug reactions (ADRs) are defined as being opposing reactions to the pharmacological effect of drugs in relation to its pharmacodynamic properties. Their diagnosis is difficult as they are relatively rare with atypical clinical presentation (with the possibility of being confused with drug ineffectiveness or the worsening of the underlying disease). This kind of ADR may be particularly subject to under-notification. The aim of the present study is to describe paradoxical ADRs using the French PharmacoVigilance DataBase (FPVDB). METHOD: We analysed all reports

Multivariate analysis suggests improved perioperative outcome in Crohn's disease patients receiving immunomodulator therapy after segmental resection and/or strictureplasty.

BACKGROUND: Medical management of moderate to severe Crohn's disease (CD) using immunomodulator agents has not eliminated surgical treatment of disease complications. The effect of improved medical treatment on perioperative CD surgical outcome is not known. We analyzed the impact of immunomodulator therapy on the rate of intraabdominal septic complications (IASC) in CD patients undergoing bowel reanastomosis or strictureplasty. METHODS: Surgical outcome was reviewed in 100 consecutive CD patients who underwent segmental resection with primary anastomosis or strictureplasty between 1998 and 2

Factors for progressive pulmonary fibrosis in connective tissue disease-related interstitial lung disease.

BACKGROUND: Progressive fibrosis can occur in connective tissue disease (CTD)-related interstitial lung disease (ILD) and make the prognosis worse. OBJECTIVES: This study aimed to investigate factors related to progressive pulmonary fibrosis (PPF) phenotype in CTD-ILDs. DESIGN: Medical records of patients diagnosed as CTD and ILD at a single, tertiary hospital in South Korea were retrospectively reviewed. METHODS: Patients whose lung functions were followed up for more than a year were included in analysis. PPF was defined as forced vital capacity (FVC) declined ⩾10% or diffusion capacity o

Successful therapy with rituximab in three patients with probable neurosarcoidosis.

BACKGROUND: Neurosarcoidosis occurs in about 5-15% of patients with sarcoidosis. Therapy with corticosteroids is generally accepted as the first-line medication, followed by various immunomodulating and cytotoxic agents or combined therapy. However, some patients show an unsatisfactory outcome or have adverse events and require novel treatment strategies. METHODS: We describe three patients with systemic sarcoidosis and central nervous system involvement who received CD20-targeted B-cell depletion with rituximab. RESULTS: Treatment with rituximab was well tolerated and followed by marked rem

Janus kinase inhibitors in the treatment of pyoderma gangrenosum: case report and review.

Pyoderma gangrenosum (PG) is a rare inflammatory dermatologic condition with neutrophilic infiltration of the skin that causes pustules and ulcerations. Janus kinase (JAK) inhibitors are immunomodulating agents that have been recently described in the literature as an effective treatment for PG. We describe a patient with PG on the lower extremities successfully treated with baricitinib. We also conducted a narrative review of the literature of PG patients treated with JAK inhibitors who were refractory to other treatments.

Treatment and clinical research in myasthenia gravis: how far have we come?

Although once an often fatal illness, myasthenia gravis can now be well managed with relatively safe and effective therapies. Prior to 1960 mortality for myasthenia gravis was approximately 30%. In the current era, mortality should be less than 5%. Major therapeutic advances by decade were: physostigmine and thymectomy (1930s); mechanical ventilation (1950s); corticosteroids and plasmapheresis (1960s); azathioprine (1960s-1970s); cyclosporine (1980s); intravenous gamma globulins (1980s-1990s); and most recently mycophenolate mofetil (1990s-2000s). Modern management involves a graded approach,

Peripheral Neuropathy: Common Side Effect.

For peripheral neuropathy, standards of care are based on established evidence-based practice. Peripheral neuropathy is disease or dysfunction of one or more peripheral nerves (i.e., motor, sensory, or autonomic), resulting in numbness or weakness. Peripheral neuropathy occurs outside of the brain and spinal cord and is caused by cancer, treatment, or both. Chemotherapeutic agents that can cause peripheral neuropathy include epothilones, platinum analogs, taxanes, and vinca alkaloids, as well as immunomodulating drugs and proteasome inhibitors.

Infliximab and other immunomodulating drugs in patients with inflammatory bowel disease and the risk of serious bacterial infections.

BACKGROUND: There remain concerns about the safety of infliximab therapy in patients with inflammatory bowel disease (IBD). AIM: To assess the association between the initiation of infliximab and other immunomodulating drugs and the risk of serious bacterial infection in the treatment of IBD. METHODS: We assembled a cohort study of patients with IBD, including Crohn's disease (CD) and ulcerative colitis (UC). All patients initiating an immunomodulating drug between January 2001 and April 2006 were identified in British Columbia from linked health care utilization databases. Exposure of inter

Vedolizumab in inflammatory bowel disease: pharmacokinetics and the role of immunomodulator co-therapy.

Vedolizumab (VED) is a gut-selective monoclonal antibody and an effective biological agent for many patients with inflammatory bowel disease (IBD). Therapeutic drug monitoring has been used to optimise treatment with other biological agents; however, the clinical applicability of measuring vedolizumab trough concentrations is unclear. Initial registration trial data demonstrated a positive exposure-efficacy relationship of vedolizumab in IBD. However, there is conflicting data in more recent studies, with no definitive benefit of vedolizumab dose-escalation and no consistent target vedolizumab

Mycophenolate Mofetil Use in Pediatric Immune Thrombocytopenia Refractory to First-line Therapy: a Single-center Experience.

Most children treated for immune thrombocytopenia remit during the first year following diagnosis. For the ∼40% who develop persistent or chronic disease, second-line treatment options include immunomodulation and thrombomimetic agents. While immunomodulators target the underlying mechanism, prolonged immunosuppression may increase the risk of infection. We report the use of the reversible immunomodulating agent mycophenolate mofetil (MMF) in 16 pediatric patients with immune thrombocytopenia refractory to first-line treatment. Using escalating doses up to 2400 mg/m 2 /d, MMF treatment resulte

Immunomodulators: current and future development and application.

It has been amply demonstrated that immunomodulators have a place in the armamentarium with other therapeutic modalities for the treatment of various diseases. They presently are, and in the future will be, most effective in preventing diseases which cause, or are the result of, immunodeficiencies. For future development the biological agents that are potent immunomodulators can be more purified and their molecular structures defined and synthesized, such as muramyldipeptides are products of BCG, etc. The active moiety of Picibanil (OK 432), a very powerful immunostimulator should be defined.

Therapeutic Approaches with Intravitreal Injections in Geographic Atrophy Secondary to Age-Related Macular Degeneration: Current Drugs and Potential Molecules.

The present review focuses on recent clinical trials that analyze the efficacy of intravitreal therapeutic agents for the treatment of dry age-related macular degeneration (AMD), such as neuroprotective drugs, and complement inhibitors, also called immunomodulatory or anti-inflammatory agents. A systematic literature search was performed to identify randomized controlled trials published prior to January 2019. Patients affected by dry AMD treated with intravitreal therapeutic agents were included. Changes in the correct visual acuity and reduction in geographic atrophy progression were evaluat

Treatment of Pyoderma gangrenosum with low-dose colchicine.

Pyoderma gangrenosum (PG) is a neutrophilic dermatosis of unknown origin. Systemic agents occasionally administered provide either incomplete long-term control of the disease or have been associated with serious adverse side effects after chronic administration. We present two patients with PG successfully treated with low-dose colchicine. Antimitotic, anti-inflammatory and immunomodulating properties of colchicine might account for its beneficial effects in PG patients. Colchicine is effective and well tolerated in low doses by most patients. In addition, it is inexpensive and safer for long-

Case Report: Use of extracorporeal support to treat a fulminant Mycobacterium bovis infection and complex broncho-oesophageal fistula in a child.

Tuberculosis (TB), caused by Mycobacterium tuberculosis remains a significant public health issue. Human TB caused by Mycobacterium bovis (M. bovis) is rare accounting for less than 1% of TB cases in UK annually. Tuberculosis secondary to immunomodulating agents is well described. We present a case of airway and pulmonary TB caused by M. bovis, likely due to zoonotic transmission, in an immunocompromised child due to medical management of Crohn's disease. Management required extracorporeal membrane oxygenation for complex surgical interventions on airway and oesophagus.
