BACKGROUND: The current literature on the specific phobia of urinary incontinence is limited, with no specific empirically established model or treatment protocol.
AIMS: This article consists of a case study of formulation-driven cognitive behaviour therapy (CBT) for phobia of urinary incontinence.
METHOD: Martin attended a total of 12 treatment sessions. The treatment included the development of an idiosyncratic formulation, and the use of well-established cognitive and behavioural treatment strategies from other anxiety disorders.
RESULTS: Both outcome measures and Martin's subjective rep

Fears of negative and positive evaluation (i.e., evaluative fears) manifest within performance-based situations (e.g., public speaking, group presentations), particularly among those experiencing social anxiety. Within these performance-based situations, individuals experiencing such evaluative fears frequently display a variety of impairments (e.g., avoidance, nervousness) that might manifest within and across various settings (e.g., employment, school). How do those who experience these fears react to in-the-moment feedback about their performance? We constructed the Fear of Evaluation About

Obsessive-compulsive disorder (OCD) is a common and disabling disorder. The most effective psychological treatment for OCD is currently exposure with response prevention (ERP). Although ERP is an effective therapy, recovery rates are relatively modest, so there is room for improvement. Metacognitive therapy (MCT) for OCD focuses primarily on modifying metacognitive beliefs about obsessions and compulsions, instead of their actual content. Based on a few small preliminary studies, there are some indications for the effectiveness of MCT for OCD. In the present article, the metacognitive model an

Exposure and response prevention (EX/RP) is a recommended psychotherapy for obsessive-compulsive disorder (OCD). Yet, not all patients benefit equally from EX/RP. Prior studies have examined EX/RP predictors by predicting endpoint symptoms and/or pre-post symptom change, rather than accounting for trajectories of symptom change across treatment. We pooled data from four NIMH-funded clinical trials, yielding a large sample (N = 334) of adults who received a standard course of manualized EX/RP. Independent evaluators rated OCD severity using the Yale-Brown Obsessive-Compulsive Scale (YBOCS). Dat

Pediatric obsessive-compulsive disorder (OCD) affects approximately .25%-3% of children, and if left untreated, can cause significant impairment for the child and family. Cognitive behavioral therapy with exposure and response prevention (ERP) is the gold-standard treatment for pediatric OCD. When using ERP with children and adolescents, it is essential to assess and target family factors in treatment in order to help the youth make therapeutic progress. The following case study illustrates the successful implementation of family-based ERP in a young female with contamination OCD. It further h

Post-traumatic stress disorder (PTSD) and psychosis may present as part of a continuum of responses to traumatic events. Despite emerging evidence on effectiveness of trauma interventions for PTSD in psychosis, implementation remains poor in Early Intervention for Psychosis (EIP) services around the world, partly due to scarcity of trauma‑adapted protocols and complex training requirements. Narrative Exposure Therapy (NET), a short intervention with minimum training requirements, offers scalability potential. This paper reports preliminary Patient and Public Involvement work with key stakehold

This article explores the potential of 3,4-methylenedioxymethamphetamine (MDMA)-assisted therapy to enhance exposure and response prevention in obsessive-compulsive disorder treatment. We discuss the mechanisms of MDMA, including fear extinction, psychological flexibility, and empathogenic effects that may improve adherence and efficacy, as well as highlighting important safety considerations for further research.

In anxiety and related disorders, Virtual Reality Exposure Therapy (VRET) was one of the first steps toward integrating technology into psychological treatments. In this article, we discuss crucial therapeutical skills and provide a case conceptualisation for the treatment of social anxiety disorder with VRET. The case conceptualisation is based on evidence-based cognitive-behavioural treatment approaches. Social anxiety can be very challenging to treat with exposure in vivo, and virtual reality exposure offers the added benefit of being able to create social situations and real-time interacti

Background: Trauma-focused cognitive behavioural therapy such as prolonged exposure is considered firsthand choice for treatment of posttraumatic stress disorder (PTSD) but is seldom available in regular care. Digital therapy is proposed to bridge this gap, but its effectiveness for severe and complex PTSD is uncertain. The primary objective of the current study was to examine the feasibility, acceptability, and preliminary effects of digital therapist-guided prolonged exposure (Huddinge Online Prolonged Exposure; HOPE).Method: Thirty participants with moderate to severe PTSD, with the majorit

Current psychotherapeutic treatments for OCD, while effective, have complex outcomes with mixed efficacy. Previous research has observed baseline brain activation patterns in OCD patients, elucidating some of the implications of this disorder. Observing the effects of evidence-based psychotherapeutics for OCD on brain activation (through MRI) may provide a more comprehensive outline of pathology. This systematic review and meta-analysis evaluated the effects of cognitive behavioural therapy (CBT) with exposure-response prevention (ERP) on brain activation in OCD patients. Irritability is a common, impairing transdiagnostic symptom in childhood psychopathology, though it has not been comprehensively studied in pediatric obsessive-compulsive disorder (OCD). Further, the central cognitive behavioral treatment component for OCD, exposure and response prevention therapy (ERP), has been recently proposed as a treatment for irritability. This study aimed to evaluate whether certain clinical characteristics are associated with irritability in pediatric OCD and whether irritability reduces following ERP. Participants were 161 youth (ages 7-17) with OCD and a caregiver p

BACKGROUND: Children with neurodegenerative diseases progressively lose skills and develop somatic and psychiatric symptoms. Obsessive-compulsive disorder (OCD) may occur, a disorder for which effective medical and psychological treatments are available. However, whether these treatments are useful for children with neurodegenerative disease is unknown.
CASE PRESENTATION: A child with an uncommon form of neurodegenerative disease (with loss of sight and incipient cognitive decline) had during the previous year developed time-consuming compulsive behaviours. The child spent much of the day at

Currently, it is unknown whether treatment outcomes derived from randomized controlled trials (RCTs) of cognitive-behavioural therapy (CBT) for problem gamblers still hold when applied to patients seen in routine practice. Thus, data from an RCT of cognitive therapy versus exposure therapy for problem gambling versus patients of a gambling help service were compared. Assessments of problem gambling severity, psychosocial impairment, and alcohol use were undertaken at baseline and post-treatment and evaluated within a counterfactual framework. Findings showed that the contrast between routine C

BACKGROUND: Problem gambling-specific cognitive therapy (CT) and behavioural (exposure-based) therapy (ET) are two core cognitive-behavioural techniques to treating the disorder, but no studies have directly compared them using a randomised trial.
AIMS: To evaluate differential efficacy of CT and ET for adult problem gamblers at a South Australian gambling therapy service.
METHODS: Two-group randomised, parallel design. Primary outcome was rated by participants using the Victorian Gambling Screen (VGS) at baseline, treatment-end, 1, 3, and 6 month follow-up.
FINDINGS: Of eighty-seven partic

The isolated effect of Virtual Reality Based Exposure Therapy (VRBET) for agoraphobia was analyzed through a comparative trial involving the first 10 agoraphobic participants. The participants were randomly assigned to two groups: VREBT only and VREBT combined with cognitive therapy. All the required Virtual Environments (VE) were created with an inexpensive Game Level Editor (GLE). Outcome measures supported the immersive effect of the VEs. Questionnaires, behavioral tests and physiological measures indicated a positive effect of VRBET alone. The addition of cognitive therapy to VREBT did not

Following a description of the paradoxical intention procedure and a brief review of the general behavioral literature relevant to this technique, the authors focus on the role of paradoxical treatment programs for agoraphobic clients. The cognitive and exposure components of paradoxical intention are discussed and the procedure is compared and contrasted with other cognitive behavioral methods on one hand, and with flooding on the other. Finally, the authors provide several ideas for future research.

BACKGROUND: Complex post-traumatic stress disorder (CPTSD) is characterized by the presence of post-traumatic stress disorder symptoms combined with disturbances in self-organization. However, treatment strategies for this newly defined condition remain lacking.
METHODS: In this systematic review of international studies exploring psychological interventions for CPTSD in children, adolescents, and adults, we screened PubMed and Google Scholar for relevant literature while adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines.

A recent survey of Americans found that the majority experienced stress during and after the 2016 United States presidential election. Psychosocial, environmental stressors can have a unique impact on symptom presentation in mental health disorders. This manuscript details a case illustration of an individual whose symptoms of obsessive-compulsive disorder were directly linked to the election, as well as how these symptoms were addressed via treatment with intensive exposure and response prevention.

J. P. Watson and I. M. Marks published a seminal article in Behavior Therapy entitled "Relevant and Irrelevant Fear in Flooding-A Crossover Study of Phobic Patients" in 1971 that paved the way for important theoretical developments and empirical studies that examined the mechanisms underlying extinction learning. Indeed, in the 44 years since their article was published, our knowledge about how exposure therapy works has increased considerably. In this review, we explore the progress our field has made in understanding extinction learning and how Watson and Marks' important work has influenced
