Insulin, incretins and other growth factors as potential novel treatments for Alzheimer's and Parkinson's diseases.

Recently, it has been shown that in patients with AD (Alzheimer's disease) and, to some degree, in patients with PD (Parkinson's disease) insulin signalling is impaired. This finding has initiated a range of research projects that showed remarkable improvements using treatments that initially had been developed to treat diabetes. Pre-clinical studies showed good neuroprotective effects when applying insulin or long-lasting analogues of incretin peptides. In transgenic animal models of AD and PD, analogues of the incretin GLP-1 (glucagon-like peptide 1) prevented neurodegenerative processes and

Effects of Incretin-Based Therapies on Testosterone Levels and Incretin Response in Men with Hypogonadism: A Systematic Literature Review Following PRISMA 2020 Guidelines.

Background/Objectives: Male hypogonadism affects 35-50% of men with type 2 diabetes mellitus (T2DM) and obesity. The relationship between testosterone deficiency and response to incretin-based therapies (GLP-1 and GIP receptor agonists) remains incompletely understood. This systematic literature review, conducted following PRISMA 2020 guidelines, examines whether male hypogonadism represents a risk factor for poor response to incretin therapy or, conversely, whether these agents offer therapeutic benefits for this population. Methods: A comprehensive systematic literature search was conducted

The incretin-bone axis: molecular mechanisms and clinical implications.

Incretin hormones, primarily glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1), are key regulators of postprandial glucose homeostasis and exert pleiotropic actions beyond pancreatic β-cell function. Increasing evidence identifies the incretin axis as a critical mediator linking nutrient intake to skeletal remodeling. Preclinical studies demonstrate that incretin signaling influences bone turnover through coordinated effects on osteoblasts, osteoclasts, osteocytes, and mesenchymal stromal cells. These actions involve modulation of major pathways, including

Incretin-Based Therapies: A Novel Pathway in Addiction Treatment.

Addiction poses a major global public health challenge. It is characterized by high prevalence, chronic relapse and limited efficacy of available pharmacotherapies across different substance use disorders. Increasing evidence demonstrates that incretin-based therapies directly modulate metabolic signaling pathways that intercross with central reward and motivational circuits, including hypothalamic-mesolimbic networks and dopaminergic neurotransmission. As a result, agents such as glucagon-like peptide 1 receptor agonists, originally developed for the treatment of type 2 diabetes and obesity,

Incretin effect is sufficient for glucose control in developing rats.

IN BRIEF: With an automated image analysis approach using artificial intelligence and machine learning, cytoarchitectural and spatial characteristics of prepartum cervix remodeling occur before term and preterm birth with loss of response to progesterone. Evaluation of local features of inflammation associated with prepartum cervix remodeling may distinguish pathophysiological from physiological processes to reduce risk of adverse pregnancy outcomes. ABSTRACT: The cervix functions as a gatekeeper barrier to maintain pregnancy and virtually vanishes for birth. Cervix remodeling occurs well bef

Obesity, incretin-based therapies, and cardiovascular disease in women.

Obesity in women is a growing and multifaceted public health challenge, driven by complex interactions among biological, hormonal, and psychosocial factors across the life course. From menarche to menopause, hormonal fluctuations influence fat distribution, energy balance, and metabolic risk, contributing to sex-specific trajectories of weight gain. Moreover, women-specific conditions, such as pregnancy-related weight retention, hypertensive disorders of pregnancy, and polyendocrine metabolic ovarian syndrome, further amplify long-term cardiometabolic risk. Women with obesity have an increased

Incretin-based drugs and intestinal obstruction: A pharmacovigilance study.

AIMS: To investigate the risk of intestinal obstruction associated with incretin-based drugs by performing a disproportionality analysis of adverse reaction reports in a global pharmacovigilance database. METHODS: We conducted a case/non-case analysis using VigiBase, the World Health Organization's adverse drug reactions (ADR) database, to assess intestinal obstruction reporting associated with incretin-based drugs (glucagon-like peptide 1 analogues [GLP-1a] and dipeptidyl peptidase 4 inhibitors [DPP-4i]. Cases were defined as reports of gastrointestinal stenosis and obstruction (MedDRA High

Rebirth of the Incretin Concept: Its conception and early development.

This paper describes the resurrection of the Incretin Concept in the early 1960s. It began with the more or less simultaneous discovery by three groups working independently in London. Dupre demonstrated that secretin given intravenously with glucose increased its rate of disappearance from the blood, McIntyre and co-workers established that hyperglycaemia evoked by oral glucose stimulated more insulin secretion than comparable hyperglycaemia produced by intravenous glucose and Marks and Samols established the insulinotropic properties of glucagon. The concept evolved with the discovery by Sam

Incretin secretion in humans is under the influence of cannabinoid receptors.

The mechanisms regulating incretin secretion are not fully known. Human obesity is associated with altered incretin secretion and elevated endocannabinoid levels. Since cannabinoid receptors (CBRs) are expressed on incretin-secreting cells in rodents, we hypothesized that endocannabinoids are involved in the regulation of incretin secretion. We compared plasma glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) responses during oral glucose tolerance test (OGTT) in 20 lean and 20 obese participants from the Baltimore Longitudinal Study of Aging (BLSA). Next,

From organophosphate poisoning to diabetes mellitus: The incretin effect.

Organophosphate (OP) poisoning induced disruption of glucose homeostasis is well established. OP poisoning leads to accumulation of acetylcholine (ACh) due to the inhibition of acetylcholinesterases (AChE). On the other hand the incidence of type 2 diabetes mellitus (T2DM) is shown to rise along with the use of pesticides in Southeast Asia. Attenuation of the 'incretin effect' is seen in T2DM. This effect is regulated by a complex loop of mechanism involving ACh driven muscarinic receptors. We hypothesize that OP poisoning leads to disruption of glucose homeostasis by attenuation of the incret

Bile enhances release of insulin: an incretin-mediated effect.

Ingestion of food stimulates secretion of bile and release of gut hormones that enhance nutrient-stimulated release of insulin. The extent of physiologic participation of bile in the enteroinsular axis was examined in seven conscious dogs (weight: 20 +/- 2 kg) that were prepared for study with chronic cannulas placed in the duodenum opposite the ampulla of Vater. On separate days, a meal consisting of 10 gm (MG-10), 25 gm (MG-25), or 62 gm (MG-62) of glucose (dextrose) was given orally in the presence of normal bile flow or during bile diversion. Bile diversion was achieved by catheterization

Endocrine implications of bariatric surgery: a review on the intersection between incretins, bone, and sex hormones.

Bariatric surgery is now the most widely used intervention for the treatment of human obesity. A large body of literature has demonstrated its efficacy in sustained weight loss and improvement in its associated comorbidities. Here, we review the effect of bariatric surgery in gut hormone physiology, bone remodeling and the reproductive axis. Rapid improvements in insulin release and sensitivity appear to be weight loss independent and occur immediately after surgery. These effects on pancreatic beta cells are mostly due to increased gut hormone secretion due to augmented nutrient delivery to t

A stewardship framework for incretin-based therapies in weight management.

BACKGROUND: Use of incretin-based therapies, including GLP-1 and GIP receptor agonists, for weight management and related cardio-kidney-liver-metabolic comorbidities has expanded rapidly, yet real-world practice is challenged by inconsistent prescribing, access barriers, limited long-term safety data, low treatment persistence, and concerns regarding equity and financial sustainability. These gaps highlight the need for a structured, coordinated stewardship model to guide safe and evidence-based use of incretin-based therapies. OBJECTIVES: To develop a scalable, multidisciplinary stewardship

Incretin polyagonists as an alternative to bariatric surgery to manage obesity.

Incretins are gut hormones involved in maintaining metabolic homeostasis in the human body, and disorders of the incretin system are recognized as contributing to the pathobiology of metabolic dysfunction and obesity. Incretin polyagonists are transforming the landscape of obesity treatment by offering potent, non-surgical alternatives to bariatric procedures. Acting on multiple incretin and related receptors, these novel pharmacological agents harness the synergistic effects of gut hormones such as glucagon-like peptide-1, glucose-dependent insulinotropic polypeptide, and glucagon to achieve

Incretin analogues as cardiovascular agents: a state-of-the-art review.

Glucagon-like peptide-1 (GLP-1) receptor agonists and the dual glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptor agonist tirzepatide have evolved over the past decade from glucose-lowering antidiabetic agents into a cardiovascular drug class with proven outcome benefit across multiple clinical scenarios. This state-of-the-art review synthesizes the cardiovascular evidence base for incretin analogues, organized by clinical scenario, and addresses the structural challenges that constrain their deployment in low- and middle-income countries (LMICs). We review the pharmacology

Advances in incretin therapies for targeting cardiovascular disease in diabetes.

The global prevalence of obesity is skyrocketing at an alarming rate, with recent data estimating that one-in-eight people are now living with the disease. Obesity is a chronic metabolic disorder that shares underlying pathophysiology with other metabolically-linked diseases such as type 2 diabetes mellitus, cardiovascular disease and diabetic cardiomyopathy. There is a distinct correlation between type 2 diabetes status and the likelihood of heart failure. Of note, there is an apparent sexual dimorphism, with women disproportionately affected with respect to the degree of severity of the card

Incretin system and glucagon secretion in patients with chronic pancreatitis.

BACKGROUND: Diabetes of the exocrine pancreas (DEP) is an underdiagnosed form of diabetes, prevalently caused by acute and chronic pancreatitis (CP). The contribution of incretin system dysfunction and the role of glucagon levels in the pathogenesis of DEP remain unclear. The aim of our study is to assess the secretion of glucagon like peptide-1 (GLP-1), glucose-dependent insulinotropic peptide (GIP) and glucagon, along with the incretin effect, in individuals with and without CP. By comparing these parameters within the same glucose tolerance class, we seek to elucidate specific hormonal alte
