In patients with mild to severe hearing loss, conventional hearing aids offer limited benefits and several problems with feedback and cosmesis. Middle ear implants are a feasible option for patients with moderate to severe hearing loss who are unable to achieve adequate benefit from or cannot tolerate hearing aids for various reasons. Here we present a case of middle ear implant surgery using Vibrant Soundbridge with incus vibroplasty technique, and describe the hearing changes during postoperative follow-up.

HYPOTHESIS: The purpose of this study is to offer new anatomic data about the long process of incus (LPI) for stapes surgery.
BACKGROUND: Anatomic study of 50 human macerated incudes to measure different parameters of cross sections of the LPI.
METHODS: Step-by-step cutting of the LPI perpendicular to its axis starting from its free end next to the lenticular process. The layer thickness was 0.5 mm. The cutting surfaces were documented with Canon EOS 20D camera, and a standard software tool (MATLAB) was used for automated statistical analysis.
RESULTS: The LPI had a maximum diameter of 1.15

There is a correlation between macroscopic and histologic changes of incus in chronic otitis with cholesteatoma. The incidence and types of damage of auditory ossicles in chronic otitis media were extensively studied, but except some studies on reoperations there are no morphometric studies of human auditory ossicles in chronic otitis media. In order to define the rate of ossicular applicability for tympanoplasties in different macroscopic forms of damage, we analysed histologic changes of human incudes in correlation to their macroscopic alterations. Histomorphometric study of 82 intraoperati

OBJECTIVE: To describe a novel approach to manage malleus/incus fixation.
STUDY DESIGN: Retrospective review of 363 patients with conductive hearing loss operated on since 1996.
SETTING: Academic tertiary referral center.
PATIENTS: Three hundred sixty-three patients with conductive hearing loss, an intact tympanic membrane, and without history for chronic infection underwent middle ear exploration. Three hundred forty-three had otosclerosis and underwent laser stapedotomy; the remaining 20 patients had laser release of their malleus/incus fixation.
INTERVENTION: Twenty patients are present

Structural characteristics of human incudostapedial articulation (ISA) have been studied on a series of histological sections from temporal bone decalcified pyramids in three planes and by means of macro-micropreparation of tympanal structural elements from 96 human fetuses at various terms of gestation. It was established that the surface of the lenticular process has a spherical form. Its length surpasses its width. The articular surface on the head of the stapes presents a dome-shaped lacuna. The capsule is usually thicker in its posterior part than in the anterior one. It can endure deviat

OBJECTIVE: Type III-r ossiculoplasty (or incus reposition) is commonly used for transmastoid decompression of the facial nerve. Although this technique should also be applicable to cholesteatoma surgery with an intact ossicular chain, it has rarely been reported. The purpose of this study was to compare the postoperative outcomes of type III-r with other ossiculoplasty techniques in middle ear cholesteatoma surgery and to evaluate the efficacy of type III-r.
METHODS: A retrospective chart review was performed on 227 ears (227 patients) with middle ear cholesteatoma that underwent initial surg

OBJECTIVES: This study aims to evaluate our ossiculoplasty results using hydroxyapatite bone cement.
PATIENTS AND METHODS: Data of 29 patients (16 males, 13 females; mean age 28 years; range 17 to 57 years) who were performed ossiculoplasty using hydroxyapatite bone cement in İzmir Katip Çelebi University Atatürk Training and Research Hospital Department of Otorhinolaryngology between January 2010 and December 2013 were retrospectively evaluated. Of the 29 operated patients, bone cement was administered in 23 patients during tympanoplasty, in two patients during open technique tympanomastoide

OBJECTIVES: To report the rare case of a patient with complete incus dislocation after trauma showing normal hearing.
METHODOLOGY: Physical examination, audiometry, CT of temporal bone, and detection during operation.
RESULTS: The incus had become remotely located in the mastoid cavity, but the patient showed normal hearing because fibrous connections had preserved bony continuity.
CONCLUSION: This case demonstrates that disruption of the ossicular chain does not always require ossicular reconstruction.

OBJECTIVE: The purpose of this study is to evaluate our experience with endoscopic repair of ossicular discontinuity at the incudostapedial joint, with or without an intact stapes suprastructure, and present our hearing results. We classify results based on the causative pathology, the type of ossiculoplasty, and type of lesion. We demonstrate the ability to endoscopically place a total ossicular replacement prosthesis (TORP), measuring 4.25 mm, between the stapes footplate and the incus remnant to reestablish ossicular continuity.
METHODS: This was a retrospective case series conducted in te

BACKGROUND: The malleus-incus complex (MIC) plays a crucial role in the hearing process as it transforms and transmits acoustically-induced motion of the tympanic membrane, through the stapes, into the inner-ear. However, the transfer function of the MIC under physiologically-relevant acoustic stimulation is still under debate, especially due to insufficient quantitative data of the vibrational behavior of the MIC. This study focuses on the investigation of the sound transformation through the MIC, based on measurements of three-dimensional motions of the malleus and incus with a full six degr

OBJECTIVES: Upon direct inspection of surgically removed ossicles from the ears of patients with long-term post-mastoidectomy cavity problems, the extent of malleus destruction often appears greater in patients with a longer duration of cavity problems, whereas the extent of incus destruction does not appear to correlate with the duration of cavity problems. This study aimed to investigate this impression.
MATERIALS AND METHODS: As a result of total middle-ear reconstruction, 41 ossicles (21 malleus and 20 incus bones) were obtained from 31 patients with post-mastoidectomy cavity problems. Th

OBJECTIVE: To evaluate the feasibility, safety and effectiveness of malleostapedotomy based on malleus-oval window technique in otosclerosis with malleus/incus mobility disorder.
METHOD: Six cases with malleostapedotomy on stapes footplate fixation accompanied by malleus/incus movements disorder from March 2005 to March 2007 were analyzed retrospectively, when totally 78 cases of conventional stapes surgeries were performed on isolated otosclerosis . The surgical procedures, intraoperative findings and postoperative vertigo as well as pure tone gain were discussed.
RESULT: All of the 6 cases

Bilateral conductive deafness due to congenital absence of the long process of the incus (CALPI) is rare. The authors present the case of a patient with bilateral CALPI. To the best of our knowledge, this is the first reported case preoperatively diagnosed as bilateral CALPI by computed tomography. Multislice computed tomography is very useful for the diagnosis of minute abnormalities of the ossicles.

OBJECTIVE: The outcome of cartilage interposition ossiculoplasty was assessed in cases of incus necrosis after posterior malleus repositioning in the plane of the stapes, in terms of hearing gain after ossicular reconstruction.
METHODS: A retrospective observational study was conducted of 30 patients admitted to an Ain Shams University hospital from March 2021 to September 2021. All patients with ossicular disruption due to chronic suppurative otitis media and hearing loss of more than 40 dB were included in the study. Pure tone audiometry was conducted for each patient after three months, si

Cholesteatoma can lead to progressive destruction of the auditory ossicles along with conductive hearing loss but precise data on the microstructural, cellular, and compositional aspects of affected ossicles are not available. Here, we obtained incus specimens from patients who had cholesteatoma with conductive hearing loss. Incudes were evaluated by micro-computed tomography, histomorphometry on undecalcified sections, quantitative backscattered electron imaging, and nanoindentation. Results were compared with two control groups taken from patients with chronic otitis media as well as from sk
