Review of Mother-Infant Linkage Algorithms in Health Care Data Sources.

INTRODUCTION: Studies that use health care data to assess infant outcomes following maternal medication use in pregnancy require linkage of maternal and infant records. While such linkage is increasing among established and emerging data sources, linkage methods are heterogeneous and have not been thoroughly reviewed or evaluated to inform fitness for use. The objective of this study was to describe methods used to link maternal and infant records in health care databases for pharmacoepidemiology research. METHODS: Multiple Medline searches were conducted using structured terms and known moth

The Unequal Impact of the COVID-19 Pandemic on Infant Health.

The COVID-19 pandemic has taken a large toll on population health and well-being. We examine the consequences of prenatal exposure for infant health, through which the pandemic may have lasting intergenerational effects. We examine multiple pathways by which the pandemic shaped birth outcomes and socioeconomic disparities in these consequences. Analysis of more than 3.5 million birth records in California with universal information on COVID infection among persons giving birth at the time of delivery reveals deep inequalities in infection by education, race/ethnicity, and place-based socioecon

Long commutes to work during pregnancy and infant health at birth.

We conduct the first empirical study to examine the health impact of long commutes to work during pregnancy on fetuses and infants at birth, using unique data that contain information on not only a woman's home address but also her employer's address during pregnancy, which allows us to calculate the maternal travel distance during pregnancy. Our study contributes to the literature on the relationship between maternal stress during pregnancy and adverse birth outcomes by focusing on the understudied chronic stress induced by long commutes, rather than the stress triggered by a one-time signifi

Unresolved controversies in gestational diabetes: implications on maternal and infant health.

PURPOSE OF REVIEW: Gestational diabetes mellitus (GDM) is a major public health concern because of rising rates and offspring consequences; yet, expert panels are in complete disagreement on how to diagnose and optimally treat GDM. This review underscores why there remains no diagnostic standard, no agreement on whether excess dietary carbohydrate or fat should be reduced, and whether oral hypoglycemic therapy is safe given the unknown offspring effects on hepatic, pancreatic, or fat development. RECENT FINDINGS: New diagnostic criteria proposed by the American Diabetes Association would trip

Effect of maternal depression on premature infant health during initial hospitalization.

This study evaluates the effect of maternal depression on neonatal health status in premature infants during their initial hospitalization. Infants younger than 34 weeks' gestation born to nondrug abusing mothers were enrolled in the study. Thirty-one mother-infant pairs were identified. Maternal depression was evaluated by the Center for Epidemiologic Studies-Depression Scale (CES-D). Scores > or = 16 defined maternal depression. Initial infant physiologic health status was determined by the Score for Neonatal Acute Physiology (SNAP). In-hospital health status was assessed by the following va

Infant deaths in a health unit area of Northern Sri Lanka.

Ninety-seven infant deaths that occurred during 1 year were almost equally distributed in the neonatal and post-neonatal periods. The low infant mortality rate (IMR) reported for the area was found to be due to poor registration of deaths. The actual IMR is 35.4. Lower respiratory tract infections and gastroenteritis were the chief causes of death, with low birth weight contributing to it. Among these deaths, as a conservative estimate, 20 have been identified as preventable. Prevention of these deaths would reduce the IMR to 28.1. Practitioners of indigenous medicine need training on the mana

Beyond breastfeeding: a One Health Decalogue for nurturing the infant microbiota.

BACKGROUND: Early-life nutrition is a key determinant of infant gut microbiota development, immune maturation, and long-term health outcomes. Although breastfeeding is widely recognized as the optimal feeding strategy, many mothers are unable to breastfeed, underscoring the need for practical, evidence-based guidance to support infant health beyond breastfeeding. A One Health approach enables the integration of nutritional, microbial, clinical, environmental, and socio-cultural factors that influence maternal-infant dyads. METHODS: A narrative review of the literature was conducted using PubM

Unveiling the placental bacterial microbiota: implications for maternal and infant health.

The human placenta is a unique organ that forms under specific physiological conditions and plays a crucial role in nutrient and metabolite exchange between the mother and fetus. Research on the placenta is important for understanding maternal-fetal diseases. Traditionally, the placenta was considered "sterile," but advancements in detection techniques have revealed the presence of a low level of microorganisms. This discovery challenges the traditional notion that the uterine placenta is sterile. The revelation of this truth marks a significant breakthrough in medical research, prompting more

Public Health Impact of Potential Infant MenACWY Vaccination Strategies in Spain.

Background: The Spanish Interterritorial Council of the National Health System (a central government body) currently recommends vaccination against meningococcal serogroup C (MenC) at 4 and 12 months of age for prevention of invasive meningococcal disease (IMD). The Advisory Committee on Vaccines of the Spanish Association of Pediatrics (a professional medical association) and numerous Spanish regional bodies instead recommend quadrivalent vaccination against serogroups A, C, W, and Y (MenACWY) at 4 and 12 months of age. The central government and Spanish Association of Pediatrics also recomme

Navigating motherhood and maternal transitional infant feeding: Learnings for health professionals.

Given the lifelong associations between infant diet and weight, understanding maternal transitional infant feeding decisions can assist efforts to establish healthy eating habits early in life. Feeding experiences were explored using semi-structured interviews with 15 first-time mothers who were selected based on their concerns about their infants' weight in an infant feeding survey. Thematic synthesis of the interview transcripts identified three main themes: (1) Expectations of a "responsible mother", (2) Trusting oneself and trust in others, and (3) From stress to successful feeding. Optimizing Weight for Maternal and Infant Health - Tenable, or Too Late?

Obesity in pregnancy is the leading cause of maternal and fetal morbidity, and gestational weight gain (GWG) is one modifiable risk factor that improves pregnancy outcomes. Most pregnant women gain more than the 2009 Institute of Medicine recommendations, particularly overweight and obese women. GWG even less than the 2009 IOM guidelines in obese women may improve pregnancy outcomes and reduce large-for-gestational-age (LGA) infants, an independent risk factor for childhood obesity, without increasing small-for-gestational-age (SGA) infants. Unfortunately, despite the fact that over 50 interve

Dental and medical students' knowledge and opinions of infant oral health.

Primary care providers' involvement with and perceptions of the epidemic of early childhood caries could be related to attitudes and knowledge of the disease as well as to differences in discipline-based recommendations. A cross-sectional survey of demographics, opinions on infant oral health care visits and importance of infant oral health care, knowledge of tooth eruption, and knowledge of health care guidelines of the American Academy of Pediatrics (AAP) and American Academy of Pediatric Dentistry (AAPD) was administered by several methods to first- and fourth-year dental and medical studen

Infant/toddler oral health care: guidelines for the primary care physician.

Since primary care physicians treat infants and toddlers on a regular basis, they oversee the growth and development of children and are usually the first health care providers to evaluate their oral health status. When examining an infant or a toddler, certain oral health conditions should be considered, such as tooth eruption, fluoridation, preventive oral hygiene, diet, and orofacial development. This article provides pertinent information in these areas that can serve as a guide for primary care physicians in their routine care of the infant/toddler.
