To determine whether the incidence of nausea and vomiting in patients with acute myocardial infarction (AMI) varies with infarct location, we studied 180 patients who had been admitted to our hospital for ST-segment elevation AMI or AMI associated with left bundle branch block. The presenting symptoms (chest pain, nausea, and vomiting), initial electrocardiographic findings, and additional demographic, clinical, laboratory, and outcome data were extracted from the medical records and correlated with the infarct location. Of the 180 patients with AMI, 108 (60%) had inferior and 72 (40%) had ant

BACKGROUND: Acute pancreatitis can rarely present with electrocardiographic changes that imitate myocardial ischemia. Even rarer is for acute pancreatitis to present with ST segment elevation in contiguous leads, suggestive of an acute coronary syndrome. In this comprehensive review article, we highlight diagnostic challenges and examine possible pathophysiological causes as seen through 34 total cases in which acute pancreatitis has been found to mimic an acute myocardial infarction.
SUMMARY: It has been shown that regardless of the severity of acute pancreatitis, it can be associated with m

Coronavirus disease 2019 (COVID-19) is a recently recognised pandemic spreading rapidly from Wuhan, Hubei, to other provinces in China and to many countries around the world. The number of COVID-19-related deaths is steadily increasing. Acute ST-segment elevation myocardial infarction (STEMI) is a disease with high morbidity and mortality rates, and primary percutaneous coronary intervention is usually recommended for the treatment. A patient with diabetes mellitus and hypertension for five years was admitted to the emergency unit with symptoms of fever, cough and dyspnoea. These symptoms were

Anterior and inferior ST elevation on electrocardiography (ECG) in patients with acute myocardial infarction is uncommon. ST-segment elevation due to right ventricular infarction induced by right coronary occlusion may extend from V1 to V3/V4, resembling the pattern of transmural ischemia of the anterior wall of the left ventricle. In addition, a wraparound left anterior descending (LAD) artery can produce ischemia manifesting as ST-segment elevation in the anterior and inferior leads. Our case report reveals dynamic ST-segment changes in acute inferior myocardial infarction, including the app

Myocardial infarction (MI) happens when blood stops circulating to an explicit segment of the heart causing harm to the heart muscles. Vectorcardiography (VCG) is a technique of recording direction and magnitude of the signals that are produced by the heart in a 3-lead representation. In this work, we present a technique for detection of MI in the inferior portion of heart using short duration VCG signals. The raw signal was pre-processed using the median and Savitzky-Golay (SG) filter. The Stationary Wavelet Transform (SWT) was used for time-invariant decomposition of the signal followed by f

PURPOSE: Inferior attenuation is a common problem in the interpretation of myocardial perfusion SPECT. We explored whether cardiovascular magnetic resonance (CMR) was a useful adjunct in differentiating between artifactual attenuation of the inferior wall and the presence of myocardial infarction and/or ischemia.
METHODS: We used CMR to assess resting wall motion, myocardial perfusion, and the presence of infarction with late gadolinium enhancement in 30 patients with presumed inferior attenuation on ungated myocardial perfusion SPECT, but where uncertainty was present over interpretation of

Atrioventricular block and cardiogenic shock are critical complications of inferior wall myocardial infarction with high mortality, especially in resource-limited settings lacking percutaneous coronary intervention. We report a 53-year-old male smoker presenting to a rural hospital with acute chest pain, dyspnea, and profound hemodynamic instability with electrocardiogram showing inferior wall ST-segment elevation myocardial infarction with Mobitz type I atrioventricular block. This case highlights reversal of atrioventricular block with return of hemodynamic stablility using Streptokinase and…

INTRODUCTION: Left Ventricular twist (LV twist) is defined as the apical counter-clockwise rotation relative to the clockwise basal rotation. It has been shown that LV twist decreases after myocardial infarction (MI) and that it is well correlated with left ventricular ejection fraction. Most studies have only evaluated anterior wall MI. The aim of our study was to determine whether LV twist is dependent on the infarct territory (anterior vs. inferior) and whether there is a correlation between LV twist and matrix metalloproteinase-9, a marker of LV remodeling.
METHODS: We measured LV twist u

The incidence of acute myocardial infarction (AMI) is increasing. Acute papillary muscle rupture is one of the serious and rare mechanical complications of AMI, which occurs mostly in inferior and posterior myocardial infarction. A patient with acute inferior myocardial infarction developed pulmonary edema and refractory shock, followed by cardiac arrest. After cardiopulmonary resuscitation (CPR), revascularization of criminal vessels was carried out by emergency percutaneous transluminal coronary angioplasty (PTCA) under the support of intra-aortic balloon pump (IABP) and extra corporeal memb

PURPOSE: To determine whether the recently applied technique of acquisition-weighted 31P-MR spectroscopy (AW-MRS) allows for the detection of depressed energy metabolism in patients with inferior wall myocardial infarctions.
MATERIALS AND METHODS: Eight patients with subacute myocardial infarction and wall motion abnormalities restricted to the inferior wall were examined with a 1.5-T MR scanner. Global and regional left ventricular (LV) function was assessed by cine MRI, and the size and extent of myocardial infarction was assessed by late enhancement (LE). MRS was performed with an AW three…

An infarction in the right coronary artery affects the inferior wall of the heart and can also cause impedance to the cardiac conduction system. The right coronary artery perfuses the sinoatrial and atrioventricular nodes, and a loss of blood flow contributes to a breakdown in the communication system within the heart, causing associated bradycardias, heart blocks, and arrhythmias. This case report details the prehospital and emergency care of a middle-aged man who experienced an inferior myocardial infarction, concomitant third-degree heart block, and subsequent cardiogenic shock, with succes

INTRODUCTION: Right ventricular infarction (RVI) is most commonly associated with inferior wall infarction (20-50% of cases). Clinical presentation of RVI may vary.
AIM: Assessment of outcome and clinical course of myocardial infarction in patients with inferior wall myocardial infarction with or without RVI. Additionally, risk stratification was attempted in the above-mentioned groups of patients.
METHODS: The analysis involved 181 consecutive patients (pts) with inferior wall myocardial infarction hospitalised between 1 July 2000 and 1 July 2002.
RESULTS: Nineteen in-hospital deaths were …

Most left ventricular inferior wall aneurysms are classified as false, and they have a narrow neck and exhibit rapid flow. Inferior wall pseudoaneurysms that develop soon after primary percutaneous intervention and coronary artery bypass grafting for acute myocardial infarction are rare. We report the case of a 64-year-old man who had a wide-necked left ventricular inferior wall pseudoaneurysm that developed soon after surgery for an acute myocardial infarction and post-infarction mitral regurgitation that occurred as a mechanical complication. The surgery consisted of coronary artery bypass g

The volumes of the right and left ventricles were measured in 78 patients with first acute transmural myocardial infarction at days 1, 3, 7, 14, and 28 of the disease. Thirty four patients were diagnosed as having anterior myocardial infarction, 35 presented with inferior myocardial infarction, and 9 had a concurrent right and left ventricular inferior wall myocardial infarction. A high incidence of right ventricular dysfunction was confirmed both in anterior and inferior myocardial infarction. The most profound right ventricular contractile dysfunctions were detected in patients with proximal…

Radionuclide imaging was useful for noninvasively visualizing a location and size of infarction in two patients with acute myocardial infarction. In one patient, inferior subendocardial myocardial infarction was detected by 99mTc-PYP myocardial scintigraphy and then inferior transmural infarction was observed as more intense radioactivity with a larger size. In the other patient with acute extensive anterior myocardial infarction, akinesis of the left ventricular wall and left ventricular aneurysm were confirmed by first-pass radionuclide angiocardiography and multigated equilibrium cardiac po

We have studied the relationship between left ventricular segmental wall motion evaluated by means of biplane left ventricular cineangiography and the myocardial infarct size estimated using Selvester's scoring system based on the QRS of the electrocardiogram. Seventy consecutive patients with old myocardial infarction were recruited. All patients underwent diagnostic retrograde left heart catheterization including left ventriculography and selective coronary arteriography. Nineteen patients were excluded from the study due to intraventricular conduction disturbances and left ventricular hyper

We observed transient myocardial bridging of left anterior descending coronary artery (LAD) in 18.75% (12 of the total 64) of the patients during acute inferior myocardial infarction (MI). Myocardial bridging occurred only in the acute phase of inferior MI and not in the chronic phase. In the acute phase of inferior MI, compensatory hypercontraction of the anterior wall is assumed to occur in response to the decrease in the movement of the infarct-related walls. In the chronic phase, disappearance of the myocardial bridging observed due to the resolution of compensatory anterior wall hypercont

BACKGROUND: Acute myocardial infarction (AMI) type is an important distinction to be made in both clinical and health care research context, as it determines the treatment of the patient as well as affecting outcomes. The aim of the paper was to determine the feasibility of distinguishing AMI type, either ST elevation myocardial infarction (STEMI) or non-ST elevation myocardial infarction (NSTEMI), using ICD10 codes.
METHODS: We carried out a retrospective descriptive analysis of hospital administrative data on AMI emergency patients in England, for financial years 2000/1 to 2009/10. We used
