Local Adverse Events Associated with Peripheral Vasoactive Infusion in Children: A Systematic Review with Meta-analysis.

Prior meta-analysis suggested a low incidence of local adverse events after infusion of vasoactive agents via a peripheral venous catheter in children. However, the number of included patients was relatively low, and the vasoactive agents used were mostly dopamine. We performed an updated systematic review with meta-analysis using databases of MEDLINE (via PubMed) and Cochrane Central Register of Controlled Trials to explore the safety of infusing vasoactive agents, including epinephrine and norepinephrine, through peripheral venous catheters or intraosseous access in critically ill children.

Intraosseous versus intravenous vascular access during out-of-hospital cardiac arrest: a systematic review and meta-analysis.

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) is a significant global healthcare burden, with survival rates remaining alarmingly low despite resuscitation advancements. This study aimed to evaluate the effect of intraosseous (IO) versus intravenous (IV) access for resuscitation on outcomes in OHCA. METHODS: This systematic review and meta-analysis pooled data from studies retrieved from PubMed, CENTRAL, Scopus, and Web of Science up to August 2024. Risk ratios (RR) with 95% confidence intervals (CI) were calculated for all outcomes. RESULTS: Twenty-one studies with a total of 249,138 pa

Intraosseous approach to vascular access in adult resuscitation.

Establishing vascular access is vital to maximise resuscitation in critically ill children and adults (LaRocco and Wang 2003), and failure can result in delays in life-saving treatment (Nutbeam and Daniels 2010). The traditional intravenous access method can be difficult to achieve in patients with circulatory collapse (LaRocco and Wang 2003) and failure rates in emergency situations vary between 10 and 40 per cent (Lewis 1986). Other routes, such as endotracheal and intramuscular, do not provide controlled and reliable administration rates (Leidel et al 2009). This article focuses on the incr

Comparison of routes for achieving parenteral access with a focus on the management of patients with Ebola virus disease.

BACKGROUND: Dehydration is an important cause of death in patients with Ebola virus disease (EVD). Parenteral fluids are often required in patients with fluid requirements in excess of their oral intake. The peripheral intravenous route is the most commonly used method of parenteral access, but inserting and maintaining an intravenous line can be challenging in the context of EVD. Therefore it is important to consider the advantages and disadvantages of different routes for achieving parenteral access (e.g. intravenous, intraosseous, subcutaneous and intraperitoneal). OBJECTIVES: To compare t

The pharmacokinetics of intraosseous atropine in hypovolemic swine.

OBJECTIVE: Compare the pharmacokinetics of atropine administered via the intravenous (IV), intramuscular (IM), and intraosseous (IO) routes in a normovolemic and hypovolemic swine model. DESIGN: Prospective, between subjects, experimental study. SETTING: Vivarium. SUBJECTS: Yorkshire-cross swine (N = 36). INTERVENTION: Atropine was administered via IV, IM, or IO routes to normovolemic and hypovolemic swine. Blood samples were drawn at regular intervals after atropine administration and analyzed for plasma atropine concentration. Pharmacokinetic parameters were obtained from modeling the pl

Treatment of hemorrhagic shock with intraosseous administration of crystalloid fluid in the rabbit model.

Intravenous access can be very difficult to obtain in small hypotensive infants. We studied the ability of the intraosseous route to accept a large volume of resuscitation fluid administered to hypovolemic rabbits. Hypotension induced by withdrawing one-third of the blood volume of rabbits was treated by infusion of saline, IV or intraosseously. A control group was bled, but received no saline. All three groups experienced a drop in mean arterial blood pressure (MAP) to at least 50% of the prebleed baseline value. Also, all groups' heart rates dropped to between 90 and 95% of prebleed baseline

[Experimental studies on the effective factors and the neural control system of intraosseous pressure in the patella of dogs].

Physiological studies on the patellar intraosseous pressure gave the following results: The mean value of the intraosseous pressure was 12.5 mmHg (range 7-19.5 mmHg). The effects of femoral vascular occlusion and intravenous infusion of epinephrine or norepinephrine were similar to those reported on long bones. Not only the elevation of intraarticular pressure but also the compression of the infrapatellar fat pad increased intraosseous pressure during extension or flexion of the knee as a result of venous engorgement. The results of efferent nerve stimulation suggested that the femoral nerve c

Red cell haemolysis secondary to intraosseous (IO) blood transfusion in adult patients with major trauma: a systematic review.

INTRODUCTION: Intraosseous (IO) administration of medications and blood products is accepted practice in major trauma when intravenous access is not immediately available. However, there is a concern that the high infusion pressures required for IO transfusion may increase the risk of red cell haemolysis and its associated complications. The aim of this systematic review is to synthesise the existing evidence describing the risks of red cell haemolysis in IO blood transfusion. METHODS: We undertook a systematic search of MEDLINE, CINAHL and EMBASE using the search terms: "intraosseous transfu

Use of intra-osseous access in adults: a systematic review.

BACKGROUND: Indications for intra-osseous (IO) infusion are increasing in adults requiring administration of fluids and medications during initial resuscitation. However, this route is rarely used nowadays due to a lack of knowledge and training. We reviewed the current evidence for its use in adults requiring resuscitative procedures, the contraindications of the technique, and modalities for catheter implementation and skill acquisition. METHODS: A PubMed search for all articles published up to December 2015 was performed by using the terms "Intra-osseous" AND "Adult". Additional articles w

The Use of Intraosseous Fluid Resuscitation in a Pediatric Patient with Ebola Virus Disease.

BACKGROUND: Vomiting, diarrhea, and severe dehydration are common manifestations of Ebola virus disease (EVD), leading to its high mortality. Mortality is especially high in patients older than 45 years, younger than 5 years, and in pregnant women and their fetuses. The majority of patients with EVD are not able to tolerate the quantities of oral hydration solutions necessary to rehydrate properly. Although some have speculated that IV and intraosseous lines are not practical in the austere, resource-constrained settings of an Ebola treatment unit during an epidemic, it is necessary to provide

Root damage induced by intraosseous anesthesia. An in vitro investigation.

OBJECTIVES: The principle of the intraosseous anesthesia (IOA) relies on the perforation of the cortical plate of the bone for direct application of the local anesthetic solution into the underlying cancellous structures. During this procedure, IOA needles might accidentally come in contact with the tooth roots. The aim of the current in vitro study was to examine the consequences of this 'worst case scenario' comparing five commercially available IOA systems. MATERIAL AND METHOD: Extracted human roots were randomly perforated using five different IOA systems with a drilling time ≤5s. To simu

Finding the most suitable puncture site for intraosseous access in term and preterm neonates: an ultrasound-based anatomical pilot study.

The purpose of this prospective ultrasound-based pilot study was to identify the most suitable tibial puncture site for intraosseous (IO) access in term and preterm neonates, describe tibial dimensions at this site, and provide anatomical landmarks for rapid localization. We measured the tibial dimensions and distances to anatomical landmarks at puncture sites A (proximal: 10 mm distal to the tibial tuberosity; distal: 10 mm proximal to the malleolus medialis) and B (chosen by palpation of the pediatrician), in 40 newborns in four weight groups (< 1000 g; 1000-2000 g, 2000-3000 g, and 3000-400

Insights From a Tertiary Care Intraosseous Insertion Practice Improvement Registry: A 2-Year Descriptive Analysis.

INTRODUCTION: Few practice improvement registries exist that describe opportunities to improve intraosseous (IO) use. The goal of this project was to assess the success rate of the procedure by emergency nurses and identify opportunities to improvement. Secondary goals were to assess success rates based on clinician type, age of patient, and procedural factors. METHODS: Emergency nurses assigned to the resuscitation area of a tertiary care emergency department completed an education module and skill lab on IO placement. Tracking forms were completed whenever IO access was attempted, and the c

Towards evidence-based emergency medicine: Best BETs from the Manchester Royal Infirmary. BET 1: Local anaesthetics in intraosseous access.

A short-cut review was carried out to establish whether flushing an intraosseous needle with local anaesthetic or saline is more effective at reducing the pain injecting drugs or fluid via this route. Two studies were relevant to the question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of these papers are shown in table 1. The clinical bottom line is that injecting lidocaine both before and after flushing an intraosseous needle is an effective method of reducing the pain of fluid infusion via this route.

[Intraosseous device of perfusion. Apropos of 3 cases before hospitalization].

A renewal of interest in the intraosseous route has appeared lately in France. It concerns pediatric patients. After anatomophysiological and technical quotes, we report the indications, contraindications, method of supervision and complications of the placement of an intraosseous infusion. Our modest experience enables us to present three observations where intraosseous route has been used outside hospital in children suffering a cardiopulmonary arrest on arrival of the practitioner belonging to the mobile emergency unit. This rapid and easy to place technique seems to be attractive as an alt

Rapid and complete bioavailability of antidotes for organophosphorus nerve agent and cyanide poisoning in minipigs after intraosseous administration.

STUDY OBJECTIVE: Management of chemical weapon casualties includes the timely administration of antidotes without contamination of rescuers. Personal protective equipment makes intravenous access difficult but does not prevent intraosseous drug administration. We therefore measured the systemic bioavailability of antidotes for organophosphorus nerve agent and cyanide poisoning when administered by the intraosseous, intravenous, and intramuscular routes in a small study of Göttingen minipigs. METHODS: Animals were randomly allocated to sequentially receive atropine (0.12 mg/kg by rapid injecti

Comparing intraosseous versus intravenous access for resuscitation in out-of-hospital cardiac arrest: A systematic review and GRADE meta-analysis.

BACKGROUND: Out-of-hospital cardiac arrest, which poses a severe challenge due to high mortality rates in emergency cases, warrants the need for effective resuscitation measures. This meta-analysis evaluated survival outcomes for intraosseous (IO) versus intravenous (IV) drug administration. METHODS: Electronic databases, including PubMed, Cochrane Central, and ScienceDirect, were searched from inception until September 2024. Risk ratios (RRs) and 95 % confidence intervals (CIs) were pooled using Review Manager version 5.4.1 under a random-effects model for dichotomous outcomes. The quality o

Effects of Intravenous Versus Intraosseous Adrenalin Administration on Morbidity and Mortality After Out-of-Hospital Cardiac Arrest: A Systematic Review.

Background and Objectives: Out-of-hospital cardiac arrest (OHCA) is a common manifestation of heart disease and a leading cause of death in western societies with an overall survival rate of 10%. Guidelines generally prefer the peripheral intravenous (IV) access as the first option for OHCA patients, leaving the intraosseous (IO) route for patients in which IV access is not feasible or unsuccessful. This systematic review will purely focus on the clinical differences between adrenaline administered via the IO route compared to the IV route and its effects on morbidity and mortality after OHCA.
