Patients as peer preceptors for orthopedic oncology rehabilitation patients.

This article describes a peer preceptor program that trains patients who have experienced limb preservation surgeries and managed long-term rehabilitative therapy to support and teach newly diagnosed patients through their rehabilitation. This preceptor program was designed to complement the professional counseling and support that rehabilitation staff provide to patients and their families. Training includes communication techniques, practice sessions using videotaped scenes of patients and their families, as well as role-playing. Experienced patients who became preceptors as a result of this

Disseminating research evidence. A controlled trial in continence care.

The dissemination of research evidence from which clinical practice should be based poses many well-documented problems for nurses. This study set out to overcome some of the common barriers to dissemination by providing a research-based clinical handbook for continence care. The impact of the handbook on nurses' knowledge of both urinary and faecal incontinence was tested using an experimental and control group. The experimental group received the handbook while the control group did not. Both groups completed an assessment questionnaire at week 1, prior to the intervention in the experimenta

'Safety in the Grey Zone': Evaluation of a training program to improve the safety of incident responders on high-speed roads.

INTRODUCTION: Workers operating on high-speed roads (i.e., incident responders and emergency service workers) are at significant risk of being fatally injured while working. An identified gap in current prevention strategies is training focused on developing the skills of workers to effectively communicate and coordinate safety responses when operating on roads. METHODS: This study discusses the development of a program designed to optimize communication and coordination of safety practices at the scene of an incident on a high-speed road. The program is referred to as 'Safety in the Grey Zon

Toward an Emerging Role for Motivational Interviewing in Primary Care.

PURPOSE OF REVIEW: Implementing Motivational Interviewing (MI) in primary care settings has been problematic due in part to persistent gaps in knowledge. Examples include poor understanding of how to effectively train persons to conduct MI, or of which aspects of MI-related communication are associated with better outcomes for patients. This review describes how recent research findings addressing the knowledge gaps support a growing role for MI in primary care. RECENT FINDINGS: Two trials of MI training combined classroom time with ongoing coaching and feedback, resulting in enhanced MI abil

Implementation of a Comprehensive Program to Improve Child Physical Abuse Screening and Detection in the Emergency Department.

INTRODUCTION: Children often present to the emergency department for treatment of abuse-related injuries. ED providers-including emergency nurses-do not consistently screen children for abuse, which may allow abuse to go undetected and increases the risk for reinjury and death. ED providers frequently cite lack of knowledge or confidence in screening for and detecting child abuse. The purpose of this quality improvement project was to implement an evidence-based screening program that included provider education on child physical abuse, a systematic screening protocol, and use of the validated

Teamwork in multi-person systems: a review and analysis.

As the scope and complexity of modern task demands exceed the capability of individuals to perform, teams are emerging to shoulder the burgeoning requirements. Accordingly, researchers have striven to understand and enhance human performance in team settings. The purpose of this review is to summarize that research, from the theoretical underpinnings that drive it, to the identification of team-level elements of success, to the methodologies and instruments that capture and measure those characteristics. Further specified are three important avenues to creating successful teams: team selection

Rehabilitation of the expanded programme on immunization in Sudan following a poliomyelitis outbreak.

In 1993 a large outbreak of paralytic poliomyelitis occurred in Sudan as a result of an accumulation of large numbers of susceptible children that was accelerated by faltering immunization services. The extent of the outbreak led to the rapid rehabilitation of Sudan's Expanded Programme on Immunization (EPI); the government began financing vaccine purchase, operational aspects of EPI were decentralized, vaccine delivery was changed from a mobile to a fixed-site strategy, a solar cold chain network was installed, inservice training was resuscitated, and social mobilization was enhanced. Comparing two models of voice care delivery.

Voice care is an important aspect in the successful treatment of patients with non-organic dysphonia. No previous study has examined the specific effect of voice care programmes or the comparative effectiveness of the delivery of these programmes across two healthcare professionals (speech and language therapists and ENT nurses). We describe a pilot study that provides preliminary data that suggests a short, easily deliverable voice care programme is (i) effective in reducing patients' perception of their dysphonia and (ii) equally effectively administered by an ENT nurse compared with a speec

Exercise rhabdomyolysis in military aircrew: two cases and a review of aeromedical disposition.

INTRODUCTION: Exercise rhabdomyolysis (ER) is a rare, sometimes catastrophic condition where muscle fibers breakdown in response to exertion and release their breakdown products into the circulation. While ER was brought to the medical community's attention largely through reports from military training establishments we are unaware of ER having resulted from the activities of military aircrew. This paper reports two cases of ER in military aircrew and discusses the condition and the approaches taken in determining their future aeromedical disposition. CASE REPORTS: Rhabdomyolysis occurred in

Equipping an urban hospital police and security team with Narcan: part two.

In their article in the last issue of the Journal, the authors reported on their health system's decision to equip their security officers with Narcan, the nasal-spray form of naloxone, and train them to use the drug to revive patients and non-patients overdosed with heroin or other life-threatening opioids. The decision, they said, was prompted with an increase in individ- uals who may come to the hospital ' 32 buildings and over 130 offsitefa- cilities to "shoot up" themselves. In this follow-up report, they describe what has happened in the seven months since the policy was begun-- how many

Neurosurgical trainees operative experience before and after introduction of the New Deal for junior doctors.

To compare operative experience before and after implementation of the New Deal to reduce junior doctors working hours, operative data was audited over a 6-month period. The three registrars with national training numbers in our unit were placed on two virtual working patterns to determine what their operative experience would have been over that 6-month period. Comparison with a 1980s trainee over a similar period was also made. Trainees on a 1:3 rota would have performed more emergency operations [32 (28 - 38)], than those on a partial shift [1:6 with following day off, 16 (13 - 23)]. Partic

The development of technology competencies and training guidelines for occupational therapists.

The ability to use technology has become a survival skill in our society. This paper discusses technology trends related to the demands of the Information Age, the increasing availability of information and assistive technologies, and the impact of recent civil rights legislation mandating that persons with disabilities be given equal access to technologies that can enhance functional performance. Occupational therapists must become competent in the application and integration of these technologies into reasonable accommodation interventions if we are to meet the changing needs of persons with

Just-in-Time Video Laryngoscopy Versus Direct Laryngoscopy for Neonatal Intubation.

As neonatal endotracheal intubation (ETI) is a low-frequency, high-consequence event, it is essential that providers have access to resources to aid in ETI. We sought to determine the impact of video laryngoscopy (VL) with just-in-time training on intubation outcomes over direct laryngoscopy (DL) when performed by neonatal nurses. We conducted a prospective, randomized, crossover study with neonatal nurses employed at a level 2 neonatal intensive care unit (NICU). Nurses performed both DL and VL on a neonatal mannequin using a CMAC (Karl Storz Corp, Tuttlingen, Germany) either with the assista
