Peak Inspiratory Flow as a Predictive Therapeutic Biomarker in COPD.

Biomarkers in COPD may be clinical (prior exacerbation history), physiologic (FEV1), or blood based (eosinophil count or fibrinogen level). Recent interest in using biomarkers to predict response to therapy in clinical practice has emerged. The benefits of inhaled therapy depend on the correct use of the inhaler, including an appropriate inspiratory flow. Of the available delivery systems, dry powder inhalers are unique because they have an internal resistance, are breath actuated, and are flow dependent. Ideally, the user inhales "forcefully" to generate turbulent energy (determined by an ind

Use of a portable device to record maximum inspiratory flow in relation to dyspnoea in patients with COPD.

Forced inspiratory measures have been described to reflect the reduction in dyspnoea upon bronchodilation in severe COPD. Based on this we evaluated the applicability and usefulness of a portable device for the assessment of forced inspiration. In 37 patients with COPD (GOLD II/II/IV n = 16/15/6, mean ± SD FEV(1) 46.2 ± 15.4%pred) lung function was recorded prior to inhalation of 24 μg formoterol and 30 min later. Assessments comprised spirometry including forced inspiration, body plethysmography, maximum inspiratory flow (InCheck, Clement Clarke), and changes in dyspnoea via visual analogue s

Respiratory dynamics and speech intelligibility in speakers with generalized dystonia.

This study investigated aspects of respiratory function, during quiet breathing and monologue, in six adult dystonic subjects and compared the findings to a control group of four neurologically intact adults. Additionally, breathing dynamics were compared with speech intelligibility. Respiratory inductive plethysmography was used to assess breathing rate, periodicity of the breathing pattern, and inspiratory lung volume. Ear oximetry was used to assess arterial blood oxygen saturation. Speech intelligibility was rated by a panel of five judges. Breathing patterns differed between groups; the d

Inspiratory muscle glycemic threshold in COPD: functional and clinical associations in incremental testing.

It is known that COPD is multifactorial and reduces the function of several body systems; however, its impact on inspiratory muscle metabolism and its possible clinical implications and for inspiratory muscle training are still unclear. Furthermore, the use of the glycemic threshold (GT) of inspiratory muscles as a viable and clinically useful metabolic measure in COPD is not yet addressed in the literature. This study aimed to verify whether there is an association between the GT of inspiratory muscles with Borg, functional capacity (FC) and cardiovascular autonomic balance in COPD. Included

Impact of inspiratory muscle training in patients with COPD: what is the evidence?

A meta-analysis including 32 randomised controlled trials on the effects of inspiratory muscle training (IMT) in chronic obstructive pulmonary disease (COPD) patients was performed. Overall and subgroup analyses with respect to training modality (strength or endurance training, added to general exercise training) and patient characteristics were performed. Significant improvements were found in maximal inspiratory muscle strength (P(I,max); +13 cmH₂O), endurance time (+261 s), 6- or 12-min walking distance (+32 and +85 m respectively) and quality of life (+3.8 units). Dyspnoea was significantl

Inspiratory pressure threshold training for upper airway limitation: a case of bilateral abductor vocal fold paralysis.

A single subject design was used to determine if pressure threshold training strengthens the inspiratory muscles in a subject with a limited glottal airway as well as diminish dyspnea and improve parameters of speech. The subject was a 19-year-old woman whose glottal airway was limited due to bilateral abductor vocal fold paralysis following a thyroidectomy. A 5-week inspiratory muscle strength-training program was implemented using a pressure-threshold trainer to strengthen the inspiratory muscles with the intent of enabling the generation of higher inspiratory pressures. The pressure thresho

Maximal shortening of inspiratory muscles: effect of training.

Normal subjects can increase their vital capacity by appropriate training. We tested whether that change can be achieved by greater maximal shortening of the inspiratory muscles without concomitant increases in peak static inspiratory pressures. Sixteen healthy volunteers participated in the study: eight were randomly assigned to make 20 inhalations to total lung capacity, held for 10 s with the glottis open, each day for 6 wk; the remainder served as nontraining controls. Before and after the 6-wk study period, we made multiple determinations of lung volumes and of curves relating lung volume

Postural vital capacity difference with aging in duchenne muscular dystrophy.

INTRODUCTION: A change in vital capacity (VC) from standing to supine can be an index of diaphragm paralysis if it exceeds 25%. We aimed to verify whether the postural VC difference increases with age and reflects diaphragm weakness in DMD. METHODS: VCs were measured in DMD. Postural VC difference and percentage were calculated from the VC data. Maximal inspiratory pressure (MIP) and MIP percentage were measured as an indirect index of diaphragm weakness. RESULTS: A total of 220 patients and 544 measurements were collected. MIP and MIP percentage decreased significantly with age (P < 0.001 f

Diagnostic methods to assess inspiratory and expiratory muscle strength.

Impairment of (inspiratory and expiratory) respiratory muscles is a common clinical finding, not only in patients with neuromuscular disease but also in patients with primary disease of the lung parenchyma or airways. Although such impairment is common, its recognition is usually delayed because its signs and symptoms are nonspecific and late. This delayed recognition, or even the lack thereof, occurs because the diagnostic tests used in the assessment of respiratory muscle strength are not widely known and available. There are various methods of assessing respiratory muscle strength during th

Acoustic rhinometry in persons recruited from the general population and diagnosed with chronic rhinosinusitis according to EPOS.

Chronic rhinosinusitis (CRS) is a disease related to the nose and the paranasal sinus as defined by the European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) criteria. The criteria include subjective symptoms, such as nasal obstruction, and objective findings by endoscopy. Acoustic rhinometry (AR) is an objective method to determine nasal cavity geometry. The technique is based on a sound pulse reflection analysis in the nasal cavity and determines cross-sectional areas as a function of distance as well as volume. AR measurements in persons recruited from the general population, wi

Effect of sustained inflations applied directly after cord clamping on lung function in premature newborn lambs.

We studied the possibility of improving lung volume and therefore clinical outcome in premature newborn lambs by increasing the inspiratory volumes during the first minute after birth. Sixteen lambs from eight were delivered by hysterotomy after 130-131 days' gestation. In eight lambs the lungs were inflated with a bag with a sustained inspiratory inflation (SI) of 5 s and expiratory time of 5 s during the first four inflations after cord clamping and then mechanically ventilated. Their siblings did not receive SI and served as a control group. At 8 h postnatally, the SI and control groups sho

Bracing arms increases the capacity for sustained hyperpnea.

Patients with severe chronic obstructive pulmonary disease (COPD) frequently lean forward, bracing their arms. We wondered whether the resulting shoulder girdle support improves the function of the ventilatory pump. We tested this possibility in 4 normal men by measuring the maximal ventilation that they could voluntarily sustain for 4 min while seated with their elbows braced firmly on a table and while seated with their elbows held just above the table. Bracing the arms increased ventilatory capacity significantly in all subjects, but the magnitude of the change was small (8%). We attribute

Phonatory assessments in patients with slowly progressive neuromuscular disorders: a new tool for evaluating inspiratory muscle dysfunction.

INTRODUCTION: Neuromuscular diseases (NMD) are a group of pathologies that affect the functionality of skeletal muscles and one of the main causes of morbidity and mortality in these patients is respiratory failure secondary to inspiratory muscle dysfunction. The diagnosis of inspiratory muscle dysfunction requires a combination of vital capacity (VC), maximum inspiratory pressure (MIP) and sniff nasal inspiratory pressure (SNIP). To assess whether it is possible using 3 techniques of peak phonation time (MPT) measurement to define lung function (VC < 50%ref) and inspiratory muscle dysfunction

Proximal airway function 8 to 16 years after laryngomalacia: follow-up using flow-volume loop studies.

Twenty children who had endoscopically confirmed laryngomalacia were reviewed at 8 to 16 years of age. All had developed stridor in the first 6 weeks of life; stridor had stopped or largely resolved by age 4 years. Proximal airway function was assessed by expiratory and inspiratory flow volume loops and compared with control values. These children as a group had variable extrathoracic obstruction to inspiratory airflow, as indicated by a significantly low maximal inspiratory flow at 50% of vital capacity and a high mean ratio of maximal expiratory to maximal inspiratory flow at 50% vital capac

Laryngomalacia and inspiratory obstruction in later childhood.

Pulmonary function and symptoms were reviewed in 20 children in whom laryngomalacia had been diagnosed by direct laryngoscopy in infancy. Most children developed stridor in the first 2 weeks, but 3 children did not develop it until age 3 months. The mean duration of stridor was 4 years 2 months, with a range of 4 months to 12 years 7 months. Six children had marked posseting as infants. Airway dynamics were assessed by flow volume loops. All except one had normal expiratory flow volume curves. Inspiratory abnormalities were detected in 7 children; 6 were assessed as having variable extrathorac

Neural respiratory drive measured during inspiratory threshold loading and acute hypercapnia in healthy individuals.

Understanding the effects of respiratory load on neural respiratory drive and respiratory pattern are key to understanding the regulation of load compensation in respiratory disease. The aim of the study was to examine and compare the recruitment pattern of the diaphragm and parasternal intercostal muscles when the respiratory system was loaded using two methods. Twelve subjects performed incremental inspiratory threshold loading up to 50% of their maximal inspiratory pressure, and 10 subjects underwent incremental, steady-state hypercapnia to a maximal inspired CO2 of 5%. Inspiratory muscle strength training with behavioral therapy in a case of a rower with presumed exercise-induced paradoxical vocal-fold dysfunction.

Paradoxical vocal fold dysfunction (PVFD) with high effort exercise can result in disruptions to ventilation, dyspnea, inspiratory stridor, elevated heart rate, and syncope. This single subject study experimentally tested an inspiratory muscle strength training (IMST) program with behavioral therapy on a 15-year-old male crew member. Outcome variables were maximum inspiratory pressure (MIP), and dyspnea ratings. Following 5 weeks of IMST, MIP increased by 93% from baseline function while dyspnea ratings substantially decreased. Outcome included successful competition with his high-school crew
