CONTEXT: Endogenous hyperinsulinaemic hypoglycaemia in adults is most commonly caused by an insulinoma. Adult nesidioblastosis is rarely reported. To the best of our knowledge the presence of both insulinoma and nesidioblastosis has not been reported before.

Insulinoma is a rare neuroendocrine tumour infrequently reported in children. Operative excision is the treatment of choice, but requires a disproportionately large incision to remove a small and benign tumour located deep in the retroperitoneum. Laparoscopic resection of benign tumours of the pancreas has been increasingly reported in adults and seems to be ideal for pancreatic insulinomas. We present the case of pancreatic insulinoma in a 13-year-old boy treated by laparoscopic distal pancreatectomy.

A 29 year-old woman with a tumor of the pancreatic tail was referred to our institute. The tumor was confirmed to be a solitary benign insulinoma that protruded from the pancreas and was distant from the main pancreatic duct. A laparoscopic enucleation was performed with Laparoscopic Coagulating Shears (LCS). The postoperative course was uneventful. The laparoscopic enucleation for benign pancreatic tumor was thought to be a feasible procedure when the appropriate instruments were used.

There may exist patients, however, who present hypoglycemia in the absence of dumping syndrome. A survey of UK patients receiving the drug diazoxide, revealed 40 patients with insulinoma on this treatment. Mean age (+/- SD) was 67 +/- 18 years, and 74% were female. Duration of treatment was 7 +/- 6 years (range 1-22). Most (55%) patients were treated with diazoxide because of tumour non-localisation (including failed previous surgery).
