Home hospitalization in the spectrum of community geriatric care.

The Home Hospitalization Programme was initiated in Jerusalem in 1991 to provide intensive medical care at home in order to prevent or shorten hospitalizations. The programme was based upon regular home visits by physicians, and nursing assessment to determine the need for regular nursing care. Primary-care physicians and nurses were renumerated by a global monthly fee, and were on 24-h call in addition to their periodic visits. Patients were recruited by senior geriatric physicians from acute hospital wards, as well as from the community, at the family doctor's request. Ancillary services ava

Home health care.

Home health care is the fastest-growing expense in the Medicare program because of the aging population, the increasing prevalence of chronic disease and increasing hospital costs. Patients and families are choosing the option of home care more frequently. Medicare's regulations are often considered the standard of care for all home health agency interactions, even when a patient does not have Medicare insurance. These regulations require patients who receive home health care services to be under the care of a physician and to be homebound. The patient must have a documented need for skilled n

Barriers to Discharge for Nursing Home Residents With Serious Mental Illness.

IMPORTANCE: Nursing homes deliver rehabilitative and long-term care for people with serious medical illnesses, functional impairment, and/or cognitive impairment. The population of nursing home residents with serious mental illness (ie, bipolar disorder, schizophrenia, other psychotic disorders) has grown substantially. Other than prisons and jails, nursing homes are the largest institutional care setting for adults (aged ≥18 years) with serious mental illness. Nursing home residents with serious mental illness become long-term residents despite less functional impairment, which may be a funct

Utilization of rehabilitation services among older persons with physical disabilities: an analysis of its association with socioeconomic status stratified by gender and age in China.

BACKGROUND: Rehabilitation is a critical method for preventing and controlling disabilities. However, the majority of the global demand for rehabilitation services remains unmet, particularly in low- and middle-income countries. This study aims to explore the complex and concealed stratified relationships between socioeconomic status and the utilization of rehabilitation services among older persons with physical disabilities (PD) from gender and age perspectives. METHODS: A total of 19,782 observations of older persons with PD were included from the 2007–2013 China National Disabled Persons C

Prediction of disability levels and research on nursing economic costs for elderly people in China.

This study aims to solve the problem of the shortage of professional nursing staff and the inability of the quality of nursing services to meet the needs of the elderly. Therefore, a longitudinal research type was adopted for long-term tracking and observation, with a time span from 2017 to 2020. The disability status of the elderly in China was analyzed, and appropriate sample data were selected to construct a comprehensive disability level assessment system. Then, a prediction method for disability scale and level based on the queue element method was proposed. Finally, based on the predicti

A Comprehensive Evaluation of Feasibility and Acceptability of a Nurse-Managed Health Clinic for Homeless and Working Poor Populations: A 3-Year Study.

Background/Objectives: Homeless populations have higher rates of chronic illness and mortality than more advantaged peers but have low primary care engagement. Nurse-managed clinics emerged as a possible solution to increase healthcare access for marginalized populations. This paper presents a comprehensive evaluation of feasibility (conceptualized as patient recruitment and retention) and acceptability (conceptualized as patient satisfaction) of a nurse-managed primary care clinic tailored to people experiencing homelessness and poverty. Methods: This is a three-year retrospective chart revie

Over- and undersupply in home care: a representative multicenter correlational study.

BACKGROUND/AIMS: Quality assurance and funding of care become a major challenge against the background of demographic changes in western societies. The primary aim of the study was to identify possible misclassification, respectively over and undersupply of care by comparing the Barthel Index of clients of home care service with the level of care (Stage 0, I, II, III) according to the statutory German long-term care insurance. METHODS: In 2012, a multi-center point prevalence study of 878 randomly selected clients of 100 randomly selected home care services across Germany was conducted. Accor

Primary healthcare policy and vision for community pharmacy and pharmacists in the United States.

The United States (US) has a complex healthcare system with a mix of public, private, nonprofit, and for-profit insurers, healthcare institutions and organizations, and providers. Unlike other developed countries, there is not a single payer healthcare system or a national pharmaceutical benefits scheme/plan. Despite spending over USD 10,000 per capita in healthcare, the US is among the worst performers compared to other developed countries in outcomes including life expectancy at birth, infant mortality, safety during childbirth, and unmanaged chronic conditions (e.g., asthma, diabetes). Payment for procedural sedation.

PURPOSE OF REVIEW: As surgical procedures become less invasive and medical procedures become more invasive, the role of procedural sedation has become more important. The resulting proliferation of settings for procedural sedation and providers rendering sedation has brought attention to the economics of this service from nonanesthesia specialists and payers. Rapidly escalating expenditures for sedation have led to scrutiny from public and private insurance carriers. The review will summarize these trends and predict changes in coding and payment for procedural sedation. RECENT FINDINGS: Chan

Activities of daily living in nursing home and home care settings: a retrospective 1-year cohort study.

OBJECTIVE: Korea introduced universal long-term care insurance (LTCI) for physically dependent older adults in 2008. Older adults, their family members, and policy makers in Korea want to know patient outcomes in different care modalities because older adults who have a similar functional status and LTC needs can choose either nursing home care or home care. The aim of this study was to compare activities of daily living (ADLs) in nursing home care and home care settings for physically dependent older adults in Korea. DESIGN: A retrospective 1-year cohort study using national LTCI data. SETT

Injectable Contraceptives as an Underutilized Option for Women's Reproductive Health: An Exploratory Qualitative Study.

BACKGROUND: Injectable contraceptives (IC) provide a highly effective, reversible method of preventing conception, yet discontinuation rates are high. Health workers play a crucial role in the successful implementation of family welfare services. Adding up the basket of choices without knowing the community's needs can lead to poor utilization of services. OBJECTIVES: To explore the facilitators and barriers to the utility of injectable contraceptives among reproductive women from the user's point of view and to understand solutions from the key informants. METHODOLOGY: The study was conduct

Health problems of Asian and Latino immigrants.

Many different circumstances influence Asian and Latino immigration to the United States, including poverty, war, educational opportunities, and protection of financial assets. Such varying circumstances point clearly to a different set of expected health problems. Immigrants often lack resources necessary to acquire quality health care. These resources involve language skills, knowledge of US health care and social services, and insurance. Risk factors to which immigrants may have been exposed include poor nutrition, lack of immunizations and vaccinations, inadequate or inappropriate treatmen

Polypharmacy and potentially inappropriate medications in older adults who use long-term care services: a cross-sectional study.

BACKGROUND: Older adults requiring care often have multiple morbidities that lead to polypharmacy, including the use of potentially inappropriate medications (PIMs), leading to increased medical costs and adverse drug effects. We conducted a cross-sectional study to clarify the actual state of drug prescriptions and the background of polypharmacy and PIMs. METHODS: Using long-term care (LTC) and medical insurance claims data in the Ibaraki Prefecture from April 2018 to March 2019, we included individuals aged ≥ 65 who used LTC services. The number of drugs prescribed for ≥ 14 days and the num
