Randomised crossover trial of neurally adjusted ventilatory assist (NAVA) for neonates with congenital diaphragmatic hernias: the NAN-C study.

Retrospective studies comparing NAVA to assist control ventilation (ACV) in neonates with congenital diaphragmatic hernia (CDH) have shown that ventilatory mode may improve respiratory parameters. The aim of this study is to determine if infants with CDH studied post-operatively had a lower oxygenation index (OI) on NAVA compared to ACV. This dual-centre randomised cross-over trial compared post-operative NAVA with ACV in infants with CDH. Infants were randomised to receive either NAVA or ACV first in a 1:1 ratio for a 4-h period. At the end of each 4-h period, blood gas analysis was performed

Neurally Adjusted Ventilatory Assist Versus Pressure Support Ventilation: A Comprehensive Review.

Mechanical ventilation serves as crucial life support for critically ill patients. Although it is life-saving prolonged ventilation carries risks and complications like barotrauma, Ventilator-associated pneumonia, sepsis, and many others. Optimizing patient-ventilator interactions and facilitating early weaning is necessary for improved intensive care unit (ICU) outcomes. Traditionally Pressure support ventilation (PSV) mode is widely used for weaning patients who are intubated and mechanically ventilated. Neurally adjusted ventilatory assist (NAVA) mode of the ventilator is an emerging ventil

Attaining Low Tidal Volume Ventilation During Patient Triggered Ventilation in Sedated Subjects.

BACKGROUND: Low tidal volume (VT) ventilation has become the preferred approach in patients in the ICU. Sedation reduces VT by attenuating respiratory drive. Even in deep sedation, some patients exhibit high VT. We aimed to determine factors associated with low VT ventilation in deeply sedated subjects who exhibited an inspiratory effort by examination of the acid/base balance using the Stewart model. METHODS: The medical records of 630 consecutive subjects admitted to the ICU over 1 y were reviewed retrospectively, and daily data sets of patients with a persistent inspiratory effort, PaO2 /F

Modes of Neonatal Ventilation: Breathe Deeply!

The art and science of neonatal ventilation continue to evolve with advances in technology and as a result of evidenced based research. Although some historically administered therapies remain such as nasal continuous positive airway pressure, newer therapies have emerged in the neonatal intensive care unit such as pressure regulated volume control and neurally adjusted ventilatory assist. The challenge for clinicians continues to be which mode will support the patient's medical diagnosis with minimal barotrauma or lung injury. Vigilance and collaborative discussions among the treatment team r

Nonconventional ventilation techniques.

PURPOSE OF REVIEW: Mechanical ventilation is one of the most important life support tools in the ICU, but it may also be harmful by causing ventilator-induced lung injury (VILI) and other deleterious effects. Advances in ventilator technology have allowed the introduction of numerous ventilator modes in an effort to improve gas exchange, reduce the risk of VILI, and finally improve outcome. In this review, we will summarize the studies evaluating some of the nonconventional ventilation techniques and discuss their possible use in clinical practice. RECENT FINDINGS: Proportional assist ventila

Correlation between diaphragmatic electrical activity and clinical respiratory distress in infants: a prospective observational study.

UNLABELLED: The electrical activity of the diaphragm (EDi), measured via neurally adjusted ventilatory assist (NAVA), reflects neural respiratory drive and may provide an objective marker of respiratory distress in infants. However, its relationship with clinically assessed respiratory distress remains uncertain, particularly across different levels of maturity. To evaluate the correlation between EDi peak values and respiratory distress severity, as measured by the Silverman-Anderson Score (SAS), in infants with NAVA catheters in situ. A prospective observational study was conducted at a tert

Impact of Neurally Adjusted Ventilatory Assist Management on Neurodevelopmental Outcomes in Extremely Preterm Infants.

INTRODUCTION: Neurally adjusted ventilatory assist (NAVA) improves patient-ventilator synchrony using diaphragmatic electrical activity. However, its long-term impact on neurodevelopment in extremely preterm infants remains unclear. This study examined the association between NAVA management and neurodevelopmental outcomes. METHODS: We retrospectively compared infants born at ≤ 27 weeks' gestation who were admitted before (2016-2017, n = 38) and after (2019-2021, n = 44) NAVA implementation. The primary outcome was the developmental quotient (DQ) at 18 months, assessed using the Kyoto Scale o

Effects of Different Modes of Mechanical Ventilation on Aerodynamics of the Patient-specific Airway: A Numerical Study.

Mechanical ventilation (MV) is an effective management strategy for neonates with critical congenital heart disease or congenital tracheal stenosis (CTS). However, there is no standard for patient-specific mode selection. This study numerically investigated the aerodynamic effects of tracheal model with severe stenosis when by different levels of ventilator assist during Pressure Support Ventilation (PSV) and Neurally Adjusted Ventilatory Assist (NAVA). Based on medical images, a three-dimensional (3D) tracheal model with insertion of a cuffed endotracheal tube was reconstructed. The technolog

Optimizing synchronized non-invasive support: Clinical management guidelines for non-invasive neurally adjusted ventilatory assist.

Neurally Adjusted Ventilatory Assist (NAVA) is an innovative ventilation mode that empowers patients to control both the timing and level of ventilatory support. By utilizing the electrical activity of the diaphragm (Edi) as the control signal, NAVA enables synchronized non-invasive ventilation (NIV-NAVA) even in the presence of leaks, while also providing continuous monitoring of the patient's respiratory pattern and drive. NIV-NAVA offers several advantages compared to conventional non-invasive ventilation, including enhanced patient-ventilator interaction and synchrony, reliable respiratory

Study protocol for a randomised cross-over trial of Neurally adjusted ventilatory Assist for Neonates with Congenital diaphragmatic hernias: the NAN-C study.

BACKGROUND: Neurally adjusted ventilatory assist (NAVA) is a mode of mechanical ventilation that delivers oxygen pressures in proportion to electrical signals of the diaphragm. The proportional assistance can be adjusted by the clinician to reduce the patient's work of breathing. Several case series of infants with congenital diaphragmatic hernias (CDH) have shown that NAVA may reduce oxygenation index and mean airway pressures. To date, no clinical trial has compared NAVA to standard methods of mechanical ventilation for babies with CDH. METHODS: The aim of this dual-centre randomised cross-

Cognitive function and mood at high altitude following acclimatization and use of supplemental oxygen and adaptive servoventilation sleep treatments.

Impairments in cognitive function, mood, and sleep quality occur following ascent to high altitude. Low oxygen (hypoxia) and poor sleep quality are both linked to impaired cognitive performance, but their independent contributions at high altitude remain unknown. Adaptive servoventilation (ASV) improves sleep quality by stabilizing breathing and preventing central apneas without supplemental oxygen. We compared the efficacy of ASV and supplemental oxygen sleep treatments for improving daytime cognitive function and mood in high-altitude visitors (N = 18) during acclimatization to 3,800 m. Each

Non-invasive neurally adjusted ventilatory assist versus nasal intermittent positive-pressure ventilation in preterm infants born before 30 weeks' gestation.

BACKGROUND: Non-invasive neurally adjusted ventilatory assist (NIV-NAVA), a mode of non-invasive ventilation (NIV) controlled by diaphragmatic electrical activity, may be superior to other NIV as a respiratory support after extubation in preterm infants, but no report has compared NIV-NAVA with other NIV methods. We evaluated the effectiveness and adverse effects of NIV-NAVA after extubation in preterm infants <30 weeks of gestation. METHODS: This retrospective study involved patients who were born before 30 weeks of gestation. We mainly used NIV-NAVA or nasal intermittent positive-pressure v

Comparison of diaphragmatic function between pressure support and NAVA using ultrasonography.

BACKGROUND: Pressure support (PS) is the most widely used mode for weaning from mechanical ventilation, yet 20-30% of ICU patients fail to wean successfully. Neurally adjusted ventilatory assist (NAVA), which triggers and supports breathing based on diaphragmatic electrical activity, improves patient-ventilator asynchrony and diaphragmatic efficiency. However, its effects on diaphragmatic function and weaning outcomes remain unclear. OBJECTIVES: To compare diaphragmatic function between PS and NAVA using ultrasonographic indices sensitive to predicting weaning failure. We hypothesized that NA
