Role of Interleukin-6 in Atherothrombosis and Myocardial Infarction.

PURPOSE OF THE REVIEW: We aimed to summarize the pathogenic role of IL-6 in atherothrombosis and myocardial infarction (MI), with focus on novel pathogenic mechanisms and current IL-6 targeted therapy in clinical cohorts. RECENT FINDINGS: IL-6 plays a major role in the pathogenesis of cardiovascular disease interacting with aging- and metabolic-driven inflammation, two conditions with overlapping molecular mechanisms. A novel pathogenic mechanism in cardiovascular disease (CVD) is the interaction between IL-6 and clonal hematopoiesis of indeterminate potential, CHIP, that involve the ten-elev

A Multi-Scale Mechanistic Model of Ulcerative Colitis to Investigate the Effects of Selective Suppression of IL-6 Trans-Signaling.

Interleukin 6 (IL-6) has previously been identified as playing a role in ulcerative colitis (UC) by activating the signal-transducing element gp130 through ligation of either the membrane-bound or soluble IL-6 receptor (termed classic and trans-signaling respectively). It has been proposed that selective inhibition of trans-IL-6 signaling could ameliorate the deleterious, pro-inflammatory effects of IL-6, while preserving the homeostatic activity of classic IL-6 signaling. We developed an in silico, mechanistic model of UC in two stages to compare the biological effects that result from inhibi

Haemostatic changes and bleeding with anti-IL-6 directed therapy in autoimmune diseases.

Anti-IL-6 directed therapy, especially tocilizumab (TCZ), is widely used for the treatment of autoimmune diseases such as rheumatoid arthritis, giant cell arteritis and systemic juvenile idiopathic arthritis. Next to being a master regulator of inflammation, IL-6 also is an important regulator of haemostasis. Although generally well tolerated, increasing clinical experience has associated TCZ treatment with rare, yet clinically significant, bleeding complications. These complications reflect disruption of IL-6-regulated hepatic fibrinogen synthesis, thrombopoiesis and possibly FXIII expression

Review of Interleukin-6 and Cardiopulmonary Bypass-Related End-Organ Injury Along With the Potential for Mitigation With Tocilizumab.

Cardiopulmonary bypass (CPB) is essential for the conduct of open-heart procedures. While lifesaving, CPB can be associated with significant end-organ injuries believed to result from inflammatory responses triggered by the extracorporeal surfaces encountering cellular elements in the blood stream. In this review, we discuss the role of interleukin-6 (IL-6) and the potential for Tocilizumab, an anti-IL-6 receptor antibody, in mitigating these effects. We compare the inflammatory responses in CPB and cytokine storm, a clinical condition in which Tocilizumab has been effectively implemented. Fin

Interleukin-6: A Critical Cytokine in Cancer Multidrug Resistance.

Multidrug resistance (MDR) is a phenomenon through which tumor cells develop resistance against the cytotoxic effects of various structurally and mechanistically unrelated chemotherapeutic agents. The most consistent feature in MDR is overexpression and/or overactivity of ATP-dependent drug efflux transporters. Other mechanisms such as overexpression of drug-detoxifying enzymes and alterations in pro-survival or pro-death signaling pathways are also responsible for MDR. Inflammatory mediators including interleukin-6 (IL-6) play important roles in various events during inflammation and are also

IL-6 as a druggable target in psoriasis: focus on pustular variants.

Psoriasis vulgaris (PV) is a cutaneous inflammatory disorder stemming from abnormal, persistent activation of the interleukin- (IL-)23/Th17 axis. Pustular psoriasis (PP) is a clinicopathological variant of psoriasis, histopathologically defined by the predominance of intraepidermal collections of neutrophils. Although PP pathogenesis is thought to largely follow that of (PV), recent evidences point to a more central role for IL-1, IL-36, and IL-6 in the development of PP. We review the role of IL-6 in the pathogenesis of PV and PP, focusing on its cross-talk with cytokines of the IL-23/Th17 ax

Targeting interleukin-6: all the way to treat autoimmune and inflammatory diseases.

Interleukin (IL)-6, a cytokine featuring redundancy and pleiotropic activity, contributes to host defense against acute environmental stress, while dysregulated persistent IL-6 production has been demonstrated to play a pathological role in various autoimmune and chronic inflammatory diseases. Targeting IL-6 is thus a rational approach to the treatment of these diseases. Indeed, clinical trials of tocilizumab, a humanized anti-IL-6 receptor antibody have verified its efficacy and tolerable safety for patients with rheumatoid arthritis, Castleman's disease and systemic juvenile idiopathic arthr

Targeting interleukin-6 as a treatment approach for peritoneal carcinomatosis.

Peritoneal carcinomatosis (PC) is a complex manifestation of abdominal cancers, with a poor prognosis and limited treatment options. Recent work identifying high concentrations of the cytokine interleukin-6 (IL-6) and its soluble receptor (sIL-6-Rα) in the peritoneal cavity of patients with PC has highlighted this pathway as an emerging potential therapeutic target. This review article provides a comprehensive overview of the current understanding of the potential role of IL-6 in the development and progression of PC. We discuss mechansims by which the IL-6 pathway may contribute to peritoneal

Tocilizumab in pediatric rheumatology: the clinical experience.

During the last two decades, clinical use of novel biological therapy has led to increased mechanistic understanding of complex rheumatological diseases. Conversely, basic and translational studies have led to development of new and varied therapeutic agents. These new medications which "target" specific steps in one or more immune pathways have the potential to control disease symptoms, improve quality of life and long-term prognosis, and perhaps in some, restore immunological tolerance. Use of these agents in clinical trials, combined with post-marketing surveillance, has revealed both the b

Leveraging decagonal in-silico strategies for uncovering IL-6 inhibitors with precision.

Interleukin-6 upregulation leads to various acute phase reactions such as local inflammation and systemic inflammation in many diseases like cancer, multiple sclerosis, rheumatoid arthritis, anemia, and Alzheimer's disease stimulating JAK/STAT3, Ras/MAPK, PI3K-PKB/Akt pathogenic pathways. Since no small molecules are available in the market against IL-6 till now, we have designed a class of small bioactive 1,3 - indanedione (IDC) molecules for inhibiting IL-6 using a decagonal approach computational studies. The IL-6 mutations were mapped in the IL-6 protein (PDB ID: 1ALU) from thorough pharma

Interleukin-6 in rheumatoid arthritis - from the laboratory to the bedside.

Rheumatoid arthritis (RA) is a common and debilitating disease. Expanded therapeutic options, targeting pro-inflammatory cytokines such as tumour necrosis factor (TNF) and interleukin-6 (IL-6), have revolutionised RA treatment. To date, efficacy data shows superiority of IL-6 inhibition over placebo, conventional disease modifying anti-rheumatic drugs such as methotrexate, and TNF inhibition. Moreover, while demonstrating some key differences in the safety profile compared with TNF inhibition (e.g. hyperlipidemia and neutropenia), these safety concerns have not, at least to date, been found to

Therapeutic approach to giant cell arteritis.

Giant cell arteritis (GCA) affects middle-sized or large arteries in individuals over 50 years of age. GCA is characterized by a combination of focal inflammation responsible for arterial stenosis or occlusion and of systemic inflammation manifesting as polymyalgia rheumatica, a decline in general health, and inflammatory anemia. In addition to the typical involvement of the branches of the external carotid arteries, relatively common sites of involvement include the aorta, most notably in its thoracic segment, and the subclavian, axillary, brachial, vertebral, and femoral arteries. The treatm

Spotlight on sirukumab for the treatment of rheumatoid arthritis: the evidence to date.

Rheumatoid arthritis (RA) is a chronic autoimmune inflammatory disease primarily affecting synovial joints and is characterized by persistent high-grade systemic inflammation. Proinflammatory cytokines, particularly interleukin-6 (IL-6), are of crucial importance in the pathogenesis of the disease, driving both joint inflammation and extra-articular comorbidities. Tocilizumab, a humanized IL-6 receptor-inhibiting monoclonal antibody, has been the first, and, to date, the only, IL-6 inhibitor approved for the treatment of RA. Many studies have demonstrated the potency and effectiveness of tocil

Inhibition of interleukin-6 attenuates erectile dysfunction in a rat model of nerve-sparing radical prostatectomy.

INTRODUCTION: The precise mechanisms underlying erectile dysfunction (ED) occurring after cavernous nerve (CN)-sparing surgery remain to be determined. Aim.  To evaluate the expression of interleukin-6 (IL-6) and IL-6 receptor (IL-6R) after CN injury, and the effect of inhibiting IL-6 bioactivity on nerve injury-related ED. METHODS: Male Sprague-Dawley rats were divided into three groups: sham operation; bilateral CN dissection without crushing or cutting; and bilateral CN resection. In the interventional experiment, male rats underwent bilateral CN dissection, and anti-rat IL-6 antibody in p

Can we design a better anti-cytokine therapy?

Cytokine neutralization is successfully used for treatment of various autoimmune diseases and chronic inflammatory conditions. The complex biology of the two well-characterized proinflammatory cytokines TNF and IL-6 implicates unavoidable consequences when it comes to their global blockade. Because systemic cytokine ablation may result in unwanted side effects, efforts have been made to develop more specific cytokine inhibitors, which would spare the protective immunoregulatory functions of a given cytokine. In this article, we review current research and summarize new strategies for improved

Interleukin-6 in the pathogenesis of posterior capsule opacification and the potential role for interleukin-6 inhibition in the future of cataract surgery.

Posterior capsule opacification (PCO) is a debilitating and relatively common complication of cataract surgery despite modern medical and surgical advances. The pathophysiology of this condition has largely been attributed to peptide mediators, such as transforming growth factor-beta (TGFβ), epidermal growth factor (EGF) and matrix metalloproteinases (MMPs), with the inhibition of these and other related molecules showing promising results. Studies have also shown that the levels of interleukin-6 are elevated in the eyes following cataract surgery and in various ocular inflammatory disorders t

Effects of Janus kinase inhibition and interleukin 6 inhibition on serum cytokine/chemokine in idiopathic multicentric Castleman disease.

Idiopathic multicentric Castleman disease (iMCD) is a rare lymphoproliferative disease characterized by systemic inflammation. Although IL-6 receptor blockade with tocilizumab is an established treatment, Janus kinase (JAK) inhibition may offer broader immunomodulation by targeting multiple cytokine signaling pathways. This comparative longitudinal study evaluated 41 serum cytokines/chemokines in 10 patients with plasma-cell-type iMCD (five treated with filgotinib (JAK1 preferential inhibitor) for 52 weeks and five treated with tocilizumab for a median of 28 months) to compare the cytokine/che

Effects of biological and targeted synthetic DMARDs on bone loss in rheumatoid arthritis.

Bone loss is a typical consequence of Rheumatoid Arthritis (RA). It occurs not only locally, affecting the inflamed joints (erosions), but also systemically, leading to osteopenia and/or overt osteoporosis, with increased risk of fragility fractures. This complication, often underestimated, can worsen the burden of disability in RA patients. Moreover, systemic and local bone loss are closely intertwined as osteoporosis per se can facilitate the development of erosions. A fundamental role in this process is played by the osteoimmunologic dysregulation typical of RA and other chronic inflammator
