Nonsurgical Removal of Improvised Nasogastric Tubes Entrapped in the Urethra in Two Male Dogs.

In two castrated male dogs, entrapment of a nasogastric feeding tube, which had been used as an improvised urethral catheter, occurred during intermittent urethral catheterization. In both cases, multiple failed attempts to remove the catheter raised concerns about catheter entrapment, which was subsequently confirmed via abdominal radiography. To facilitate removal, both dogs were placed under deep sedation using IV butorphanol (0.3 mg/kg) and propofol titrated to effect. A retrograde over-the-needle catheter was inserted alongside the entrapped nasogastric tube, and the urethral orifice was

Benzalkonium chloride induced corrosive balanoposthitis in a man requiring clean intermittent catheterization.

INTRODUCTION: Men who require intermittent catheterization may be susceptible to balanoposthitis. Benzalkonium chloride is commonly used to disinfect reusable catheters, but the concentration of this solution is critical. CASE PRESENTATION: A 40-year-old man presented with fever, pus around the glans and foreskin, and difficult catheterization. On physical examination, his urethral meatus was too narrow to insert a catheter. Suprapubic catheterization and 2 weeks of intravenous and topical treatment cured the ulcer of the glans. However, after restarting catheterization, the ulcer of the glan

Poor quality of life in patients with urethral stricture treated with intermittent self-dilation.

PURPOSE: We assessed patient perceptions of regular intermittent self-dilation in men with urethral stricture. MATERIALS AND METHODS: We constructed and distributed a visual analog questionnaire to evaluate intermittent self-dilation via catheterization by men referred for urethral stricture management at a total of 4 institutions. Items assessed included patient duration, frequency, difficulty and pain associated with intermittent self-dilation as well as interference of intermittent self-dilation with daily activity. The primary outcome was patient perceived quality of life. Multivariate an

[Urethral condyloma in a patient with neurogenic bladder dysfunction: a therapeutic challenge].

Urethral condyloma is a therapeutic challenge. This article reports the case of a patient with spinal cord injury with the incidental finding of a massive spread of urethral condyloma. After removal of the condyloma with biopsy forceps neither recurrence of the condyloma nor a urethral stricture occurred. In patients where intermittent catheterization is performed, condyloma may be dispersed into the bladder, therefore, prompt endoscopic removal is crucial. As condyloma frequently recurs even after complete removal regular controls are mandatory.

Advances in clean intermittent self-catheterisation: impact on trauma and urinary tract infections.

Intermittent self-catheterisation with hydrophilic coated catheters carries the risk of trauma, bleeding and infection. However, evidence suggest that these risks can be minimised with a new generation of catheters that stay lubricated over time, allowing for comfortable and safe insertion and withdrawal.

Modifications of and extended indications for the Pippi Salle procedure.

Total urinary incontinence is a difficult problem faced by the urologist. Several techniques to increase ureteral resistance have been described. The majority of them rely on intermittent catheterization for bladder emptying, especially in neurogenic incontinence. We have developed a new procedure in which a bladder flap is used to create a neourethra. This urethral extension acts as a flap valve to provide continence. Bladder emptying is accomplished by clean intermittent catheterization. Urethral lengthening with an anterior bladder-wall flap was performed in 18 patients aged a mean of 8.9 y

Detrusor-External Sphincter Dyssynergia: Review of Minimally Invasive and Endoscopic Management.

Detrusor-external sphincter dyssynergia (DSD) is a debilitating problem in patients with spinal cord injury. DSD carries a high risk of complications, and even life expectancy can be affected. Management of this condition includes the use of antimuscarinic agents in combination with intermittent catheterization, indwelling urethral catheterization, suprapubic catheterization, and a variety of surgical options, depending on patient and physician preference. This paper will review the current literature and data on minimally invasive and endoscopic management of DSD.

Management of posterior urethral valves on the basis of urodynamic findings.

PURPOSE: Abnormal urodynamic findings are common in boys with a history of posterior urethral valves. However, to our knowledge there are few reports on the results of treating these abnormal findings. We analyzed the treatment of abnormal urodynamic parameters and its outcome in 21 boys who underwent valve ablation. MATERIALS AND METHODS: After valve ablation multichannel urodynamic studies were performed in 31 boys, including 21 in whom studies were done before and after therapy was started for abnormal parameters. Detrusor instability and impaired bladder compliance were treated with antic

Bladder management and risk of bladder stone formation in spinal cord injured patients.

PURPOSE: We determined by statistical analysis whether method of management is associated with risk of bladder stone formation in spinal cord injured patients. MATERIALS AND METHODS: A retrospective cohort study was performed of 457 patients admitted to Stoke Mandeville Hospital Spinal Injuries Center between 1985 and 1990 with more than 6 months of followup. Analysis included Cox regression and Poisson regression. RESULTS: Relative to those patients treated with intermittent self-catheterization, the hazard ratio was 10.5 (p <0.0005, 95% confidence interval 4.0-27.5) for patients with supra

[Orthotopic neobladder in a female patient after radiotherapy and anterior exenteration with ileal conduit].

OBJECTIVE: To demonstrate the viability of a neobladder created after radiotherapy, anterior pelvic exanteration and previous ileal conduit. METHODS: We report the case of a female patient who underwent anterior pelvic exanteration, radiotherapy and ileal conduit as treatment for uterine cervix cancer. A transverse colon neobladder was performed and anastomosed to the urethral stump. It was also constructed an independent self-catheterizable conduct connected to the abdominal wall, with an adjacent segment of transverse colon using the Monti's technique. This was an alternative route for cath

[Clean vesical intermittent catheterization: lubrication of vesical catheter through lidocaine or gentamicine?].

INTRODUCTION: The patients who undergo intermittent catheterization are at high risk of urinary tract infection. A prophylaxis method for this infection can be represented by the lubrication of urethral catheters with 0.1% gentamycin cream. This study compared the lubrication of urethral catheters with 0.1% gentamycin cream and 2% lidocaine jelly in relation to the quantitative results of the urine culture and incidence of symptomatic urinary tract infection and asymptomatic bacteriuria. MATERIAL AND METHODS: Sixteen patients, divided in groups A and B, were studied. Patients in Group A were
