The feasibility of dexmedetomidine-led anesthesia maintenance strategy during major abdominal surgery.

BACKGROUND: Dexmedetomidine is known for its selective action on α2-adrenoceptor sites and is recognized for its neuroprotective capabilities. It can improve postoperative cognitive function. Commonly used anesthetics, such as sevoflurane and propofol, have been reported to affect postoperative cognitive function. Therefore, it could be valuable to explore dexmedetomidine-led anesthesia strategy. This study was designed to assess the performance, safety, and effective infusion rate in anesthesia maintenance, to explore a feasible dexmedetomidine-led anesthesia maintenance protocol, and to prov

Investigation of Possible Intraoperative Transmission of Brucella melitensis, Slovenia.

We report possible intraoperative transmission of Brucella melitensis in Slovenia, likely caused by aerosolized particles during wound irrigation. Whole-genome multilocus sequence typing revealed that isolates from the patient and the surgeon belonged to the same transmission cluster, differing by 1 allele. Our findings raise awareness of occupational risks faced by orthopedic surgeons.

Middle Colic Artery Originating from the Gastroduodenal Artery Discovered during a Whipple.

Hepatic artery variations occur in about one-fourth of the population, are frequent questions on standardized surgery exams, and are often discussed on teaching rounds with an attending surgeon. Intraoperatively, it is important to be aware of possible vascular anomalies in order to prevent ligation or injury of an essential vessel. This case report describes an extremely rare vascular anomaly encountered during a pancreaticoduodenectomy (Whipple operation). Our patient was a middle-aged woman who was incidentally diagnosed with a cystic pancreatic lesion. During the operation, an aberrant mid

Hand injuries during hand surgery: a survey of intraoperative sharp injuries of the hand among hand surgeons.

An e-mail survey comprising 19 questions was directed towards members of the American Society for Surgery of the Hand (ASSH) to investigate the prevalence and nature of intraoperative injuries to hand surgeons during hand surgery. The responses were collected, statistical analysis was done and trends were extrapolated. Two hundred members of the ASSH completed the e-mail survey. A hand surgeon in practice for greater than 10 years has a 97% chance of sustaining an intraoperative "sharps" injury. The injury is self-inflicted (88%) in most cases and the index finger (94%) of the left hand (87%)

Postoperative recovery of patients with differential requirements for sevoflurane after abdominal surgery: A prospective observational clinical study.

An association between animals and volatile anaesthetic requirements has been shown; however, evidence related to the postoperative outcome of human patients is lacking. Our aim was to investigate whether there is a difference in the requirement for sevoflurane among people undergoing gastrointestinal surgery. We observed 390 adult patients who underwent gastrointestinal surgery with an American Society of Anesthesiologists physical status of I or II with an expected surgery duration of > 2 hours. We used the bispectral index (BIS) to guide the regulation of end-tidal sevoflurane concentration

Conscious awareness during general anaesthesia: patients' perceptions, emotions, cognition and reactions.

We interviewed 45 patients, who answered advertisements (n = 21) or were referred by colleagues (n = 24), about their experience of intraoperative awareness using a standardized questionnaire. Auditory perceptions, hearing sounds or voices were mentioned by all patients (45 of 45): 33 of 45 patients understood and recalled conversations; 21 of 45 patients had visual perceptions; 12 of 21 recognized things or faces; 29 of 45 patients felt being touched; three patients had the sensation of moderate pain; and eight patients were in severe pain. Patients' feelings were mostly related to paralysis

Anaphylaxis due to latex allergy.

An anaphylactic reaction to latex is reported in two patients. One occurred intraoperatively with exposure to the surgeon's latex gloves and the other during vaginal contact with a latex condom. Radioallergosorbent test (RAST) and scratch test with latex material identified the allergen. With the increasing use of latex gloves and condoms, one should be aware of the possibility of a severe immediate hypersensitivity reaction to latex products. Latex allergy should be considered when evaluating intraoperative and idiopathic anaphylaxis.

The incidence of intra-operative awareness during general anesthesia in China: a multi-center observational study.

BACKGROUND: The incidence of awareness in patients undergoing general anesthesia is 0.1-0.2% in Western countries. The medical literatures about awareness during general anesthesia are still rare in China, but some previous studies have reported a higher incidence (1.4-6%) of intra-operative awareness. To find out the reason why the incidence reported in China is much higher than that in Western countries, we performed a prospective, multicenter, non-randomized observational study to determine the true incidence of intra-operative awareness in China. METHODS: This is a prospective, non-random

Clinical Trial Comparing Remimazolam with Propofol During Intravenous Anesthesia: A Prospective Randomised Clinical Trial.

BACKGROUND AND OBJECTIVE: Remimazolam is a water-soluble sedative-anesthetic with short-acting properties and less hemodynamic effects. Currently, it is primarily used for gastroenteroscopy sedation. AIM: The aim of this study is to investigate the effectiveness and safety of Remimazolam as an alternative intravenous anesthetic agent in surgical patients, in order to expand clinical options beyond Propofol. METHODS: Eighty patients aged 20-69 and classified as an American Society of Anesthesiologists physical status I-II were randomly assigned to either the Remimazolam group (RM group) or th

CT-override: endoscopic updates to preoperative anatomical models during ablative surgery.

PURPOSE: Preoperative CT provides a detailed anatomical basis for navigation in sinus surgery, guiding surgeons through intricate nasal structures while providing 3D awareness of critical anatomy. However, this static representation lacks the capacity to adapt to intraoperative tissue ablation. Efforts toward endoscope-based navigation reconstruct anatomy from visual cues alone, often without incorporating the CT prior that anchors surgical context. We propose a method to ground both CT and endoscope in a unified representation that bridges pre- and intraoperative domains, enabling dynamic upd

Asystole in Orthognathic Surgery: A Case Report.

A previously healthy 26-year-old man presented for elective Le Fort I osteotomy, bilateral mandibular osteotomy, and genioplasty. During the intraoperative course, the patient developed sudden bradycardia and asystole on placement of a bite block. Asystole is an extremely rare consequence of the trigeminocardiac reflex, and awareness of this potentially life-threatening complication is pivotal in its management.

Acute gastric variceal bleeding during orthotopic liver transplant.

We present a case of intraoperative gastric variceal bleeding during liver transplant. After an uneventful induction and surgical dissection, our patient developed hemodynamic instability during the anhepatic phase. We believe that an increase in portal pressures, owing to clamping of the portal system, led to spontaneous variceal rupture; however, placement of an oral gastric tube or transesophageal echocardiography probe may have contributed to this also. After intraoperative banding, the patient was stabilized and surgery proceeded uneventfully. The patient had no long-term sequelae. Anesth

Cardiac Arrest During Spine Surgery in the Prone Position: Case Report and Review of the Literature.

OBJECTIVE: Intraoperative cardiac arrest (CA) is usually attributable to pre-existing disease or intraoperative complications. In rare cases, intraoperative stress can demask certain genetic diseases, such as catecholaminergic polymorphic ventricular tachycardia (CPVT). It is essential that neurosurgeons be aware of the etiologies, risk factors, and initial management of CA during surgery with the patient in the prone position. METHODS: We present a case of CA directly after spinal fusion for lumbar spondylolisthesis and review the literature on cardiac arrests during spinal neurosurgery in t

A rare pattern of acute type A aortic dissection: circumferential intimal invagination.

A 59-year-old patient with chest pain was transferred to our emergency unit. Computed tomography depicted a dissection membrane limited to the aortic root and a seemingly normal ascending aorta. Further downstream stenosis of the descending aorta appeared. Intraoperatively, complete intimal absence in the ascending aorta with intussusception into the descending aorta was evident. Ascending aortic and hemiarch replacement was performed. This rare case should raise the surgeons' awareness that preoperative imaging can differ considerably from intraoperative findings.
