Acceptance and commitment therapy for breast cancer survivors with fear of cancer recurrence: A 3-arm pilot randomized controlled trial.

BACKGROUND: Fear of cancer recurrence (FCR) has a profound negative impact on quality of life (QOL) for many cancer survivors. Breast cancer survivors (BCS) are particularly vulnerable, with up to 70% reporting clinically significant FCR. To the authors' knowledge, evidence-based interventions for managing FCR are limited. Acceptance and commitment therapy (ACT) promotes psychological flexibility in managing life's stressors. The current study examined the feasibility and preliminary efficacy of group-based ACT for FCR in BCS. METHODS: Post-treatment BCS (91 patients with stage I-III disease)

Using decision tree models and comprehensive statewide data to predict opioid overdoses following prison release.

PURPOSE: Identifying predictors of opioid overdose following release from prison is critical for opioid overdose prevention. METHODS: We leveraged an individually linked, state-wide database from 2015-2020 to predict the risk of opioid overdose within 90 days of release from Massachusetts state prisons. We developed two decision tree modeling schemes: a model fit on all individuals with a single weight for those that experienced an opioid overdose and models stratified by race/ethnicity. We compared the performance of each model using several performance measures and identified factors that w

Regenerative potential of human dental pulp stem cells in the treatment of stress urinary incontinence: In vitro and in vivo study.

OBJECTIVES: To evaluate the regenerative potential of human dental pulp stem cells (hDPSCs) in an animal model of stress urinary incontinence (SUI). SUI, an involuntary leakage of urine, is due to physical stress involving an increase in bladder pressure and a damage of external urethral sphincter affecting muscles and nerves. Conventional therapies can only relieve the symptoms. Human DPSCs are characterized by peculiar stemness and immunomodulatory properties and might provide an alternative tool for SUI therapy. MATERIALS AND METHODS: In vitro phase: hDPSCs were induced towards the myogeni

Building an Integrated Data Infrastructure to Examine the Spectrum of Suicide Risk Factors in Philadelphia Medicaid.

While there are many data-driven approaches to identifying individuals at risk of suicide, they tend to focus on clinical risk factors, such as previous psychiatric hospitalizations, and rarely include risk factors that occur in nonclinical settings, such as jails or emergency shelters. A better understanding of system-level encounters by individuals at risk of suicide could help inform suicide prevention efforts. In Philadelphia, we built a community-level data infrastructure that encompassed suicide death records, behavioral health claims, incarceration episodes, emergency housing episodes,

The use of outpatient commitment in Norway: Who are the patients and what does it involve?

PURPOSE: Despite one of the longest histories of using Outpatient Commitment (OC), little is known about the use in the Norwegian context. Reporting from the Norwegian Outpatient Commitment Study, this article aims to: establish the profile of the OC population in Norway; ascertain the legal justification for the use of OC and what OC involves for patients; investigate possible associations between selected patient and service characteristics and duration of OC, and; explore potential differences based on gender or rurality. METHODS: A retrospective multi-site study, extracting data from the

Factors associated with psychiatric admission from observation beds in the psychiatric emergency room.

BACKGROUND: Psychiatric emergency services frequently utilize observation units to support disposition decisions when diagnostic uncertainty exists, allowing for extended psychiatric supervision to inform medical decision-making. Yet there is limited data describing predictors of inpatient admission following observation, and understanding which factors are associated with this outcome may improve clinical decision-making and resource utilization. METHODS: We conducted a retrospective chart review of adult patients placed in an observation unit within a high-volume urban psychiatric emergency

On the recording of mental illness for civil commitment.

The survey of 102 consecutive certificates for involuntary admission and a review of the case material demonstrated that in 78 cases the form was completed adequately. In 13 cases full description of evidence of mental disorder was lacking on the certificate but after reviewing the cases it would appear that clear evidence of mental disorder was probably present at the time of certification. In this survey, suicidal or homicidal risk alone was not considered evidence of mental disorder. In the full 11 cases the grounds for diagnosing mental disorder rested upon the presence of phenomena like a

Behavioral changes of patients after orthognathic surgery develop on the basis of the loss of vomeronasal organ: a hypothesis.

We introduce a hypothesis which presumes that damage to the vomeronasal organ during a Le Fort I osteotomy of the maxilla for the purpose of orthognathic surgical treatment of congenital or acquired jaw deformities affects the patient's social life in terms of the selection of mates and establishment of relationships. The vomeronasal organ is chemosensory for pheromones, and thus registers unconscious olfactory information which might subsequently act on the limbic system of an individual and influence the selection of mates. We believe it is connected to an inhibitory feedback mechanism which

The Use of Telepsychiatry Services in Emergency Settings: Scoping Review.

BACKGROUND: Telepsychiatry (TP), a live video meeting, has been implemented in many contexts and settings. It has a distinct advantage in the psychiatric emergency department (ED) setting, as it expedites expert assessments for psychiatric patients. However, limited knowledge exits for TP's effectiveness in the ED setting, as well as the process of implementing TP in this setting. OBJECTIVE: This scoping review aimed to review the existing evidence for the administrative and clinical outcomes for TP in the ED setting and to identify the barriers and facilitators to implementing TP in this set

Life, liberty, and the pursuit of happiness: reframing inequities experienced by people with severe mental illness.

TOPIC: In the Declaration of Independence, Thomas Jefferson stated that citizens of the new republic had fundamental and inalienable rights to "life, liberty and the pursuit of happiness." PURPOSE: The purpose of this article is to reframe inequities experienced by people living with severe mental illnesses in terms of violations of Jeffersonian rights to "life, liberty and the pursuit of happiness." We simultaneously consider governmental and clinical efforts to address identified inequities and uphold these rights. SOURCES USED: The broad research literature on severe mental illnesses was

Making the case for a model mental health advance directive statute.

Acute episodes of mental illness temporarily destroy the capacity required to give informed consent and often prevent people from realizing they are sick, causing them to refuse intervention. Once a person refuses treatment, the only way to obtain care is as an involuntary patient. Even in the midst of acute episodes, many people do not meet commitment criteria because they are not likely to injure themselves or others and are still able to care for their basic needs. Left untreated, the episode will likely spiral out of control. By the time the person finally meets strict commitment criteria,

Dual harm: Violent behaviour to others and self-harm behaviour in adults compulsorily admitted to a Dutch psychiatric hospital.

BACKGROUND: Verbal and physical violence in psychiatric hospitals can have harmful consequences for staff members, such as physical injury, traumatisation, and sick leave, and they often accompany involuntary admission. Harm to others may co-occur with self-harm, i.e., dual harm. However, little is known about the association between dual-harm and violent behaviour towards staff members and its clinical outcomes, such as seclusion and rapid tranquilisation after involuntary admission to a psychiatric inpatient unit. METHOD: A convenience sample of patients admitted involuntarily (N = 384; mea

Anti-aggressive effects of clozapine in involuntarily committed black patients with severe mental illness.

INTRODUCTION: Patients with severe mental illness are falsely characterized as aggressive by the media, perpetuating stigma. While exaggerated, some patients with severe mental illness are more aggressive without treatment. Clozapine may have a unique anti-aggressive effect in patients with schizophrenia-related disorders, independent of antipsychotic or sedative effects. Limited data in forensic and involuntary committed patients is currently available. PURPOSE: This study evaluates clozapine's effects on hostility and aggression in court-ordered Black patients. METHODS: This study analyzes

Importance of Capgras syndrome in shared psychotic disorder: a case report.

Shared psychotic disorder characterized by Capgras syndrome is an extremely rare condition. To our knowledge, there are only a few published papers on this condition. This paper presents a case of shared Capgras syndrome in two sisters. The inducer was a younger sister with schizophrenia, who passed on her Capgras delusion to her older sister after the death of their father. After committing a violent offense caused by Capgras delusion, a court ordered the sisters' involuntary admission to a psychiatric hospital. After being separated and receiving antipsychotic treatment, the sisters showed s

Ethical and Legal Considerations in the Refusal of Antipsychotic Treatment During Pregnancy: A Case Report.

Acute psychosis during pregnancy creates a complex clinical scenario that requires simultaneous consideration of maternal psychiatric stabilization and fetal well-being. Guidance remains limited on the management of acutely psychotic pregnant patients who lack decision-making capacity and refuse care, particularly in settings without on-site obstetric resources. We describe the case of a 35-year-old woman with schizoaffective disorder and post-traumatic stress disorder who was involuntarily committed for acute behavioral disturbance and found to be approximately 14 weeks pregnant during evalua

Considerations of Dissociation, Betrayal Trauma, and Complex Trauma in the Treatment of Incest.

Child sexual abuse committed by a parent (incest) is related to particularly severe physical and psychological symptoms across the life span. Incest is associated with low self-esteem, self-loathing, feelings of contamination, worthlessness, and helplessness, as well as somatization and low self-efficacy. A child's negative constructions often are attempts to derive some meaning that justifies the incest, such as, "it is because of my badness that it happens to me." Survival often involves voluntary or involuntary disconnection from self, others, and the environment, or compartmentalization of

Prevalence of aggression and associated factors among inpatients with mental illness at tertiary hospitals in Southwestern Uganda.

BACKGROUND: In mental health treatment facilities around the world, aggression aimed towards medical personnel and other patients is a serious issue. Types of aggression include; verbal aggression, aggression towards property, self-harm/auto-aggression, and physical aggression. Studies show 1 in 5 patients admitted to acute mental health care wards in high-income countries commit an act of physical violence during admission. In Uganda, there is limited literature on aggression among patients with mental illness admitted in psychiatric wards in our setting. This study aimed to investigate preva

The Role of Lifestyle on Adherence to Treatment in a Sample of Patients with Unipolar and Bipolar Depression.

Introduction: Poor adherence to treatment is currently stated to be one of the causes of depression relapse and recurrence. The aim of the present study is to assess potential differences in terms of clinical and lifestyle features related to adherence to treatment in a sample of patients with unipolar and bipolar depression. Methods: One hundred and eight patients with a diagnosis of unipolar or bipolar depressive episode were recruited from January 2021 to October 2022. Adherence to psychopharmacological treatment was assessed using the clinician rating scale. Descriptive and association ana

Navigating the Complexity of a Bipolar Pregnant Patient With Multiple Comorbidities.

Bipolar disorder I (BDI) is a psychiatric disorder characterized by the occurrence of at least one manic episode. Within the scope of neurological disorders, epilepsy and psychogenic nonepileptic seizures (PNES) share clinical features and can be differentiated using electroencephalogram (EEG). Substance use disorder is a condition defined by impaired control, risky use, social impairment, and addictive behaviors. We present the case of a 20-year-old pregnant woman with BDI associated with a history of epilepsy, PNES, and polyvalent substance use. The patient presented to the emergency departm
